Thus, all parts of the health system must be involved; policy makers, healthcare professionals and users of the services have been forced to think differently about how services are financed, how resources are allocated, how systems are structured and organized, how services are delivered to patients, and how the resulting activity is monitored and evaluated in order to improve the effectiveness, efficiency, equity and acceptability of the response. This book is unique in attempting to describe and assess a range of responses across the globe by situating them within the characteristics of each country and its health system. Most chapters combine a health policy expert with an HIV specialist, allowing both a 'top down' health system approach and a 'bottom up' HIV-specific perspective. Book excerpt: "The presence, or absence, of neglected tropical diseases (NTDs) can be seen as a proxy for poverty and for the success of interventions aimed at reducing poverty. Today, coverage of the public-health interventions recommended by the World Health Organization (WHO) against NTDs may be interpreted as a proxy for universal health coverage and shared prosperity - in short, a proxy for coverage against neglect. As the world's focus shifts from development to sustainable development, from poverty eradication to shared prosperity, and from disease-specific goals to universal health coverage, control of NTDs will assume an important role towards the target of achieving universal health coverage, including individual financial risk protection. Success in overcoming NTDs is a "litmus test" for universal health coverage against NTDs in endemic countries. The first WHO report on NTDs (2010) set the scene by presenting the evidence for how these interventions had produced results. The second report (2013) assessed the progress made in deploying them and detailed the obstacles to their implementation. This third report analyses for the first time the investments needed to achieve the scale up of implementation required to achieve the targets of the WHO Roadmap on NTDs and universal coverage against NTDs. INVESTING TO OVERCOME THE GLOBAL IMPACT OF NEGLECTED TROPICAL DISEASES presents an investment strategy for NTDs and analyses the specific investment case for prevention, control, elimination and eradication of 12 of the 17 NTDs. Such an analysis is justified following the adoption by the Sixty-sixth World Health Assembly in 2013 of resolution WHA6612 on neglected tropical diseases, which called for sufficient and predictable funding to achieve the Roadmap's targets and sustain control efforts. The report cautions, however, that it is wise investment and not investment alone that will yield success. The report registers progress and challenges and signals those that lie ahead. Climate change is expected to increase the spread of several vector-borne NTDs, notably dengue, transmission of which is directly influenced by temperature, rainfall, relative humidity and climate variability primarily through their effects on the vector. Investments in vector-borne diseases will avoid the potentially catastrophic expenditures associated with their control. The presence of NTDs will thereby signal an early warning system for climate-sensitive diseases. The ultimate goal is to deliver enhanced and equitable interventions to the most marginalized populations in the context of a changing public-health and investment landscape to ensure that all peoples affected by NTDs have an opportunity to lead healthier and wealthier lives."--Publisher's description.

Book excerpt: This paper has analyzed the cost-effectiveness of interventions aimed at scaling up HIV/AIDS treatment in three sub-Saharan African countries: Mozambique, Tanzania and Uganda. This issue is of particular importance as all three countries are now in a time of flux, both in terms of their general outlook and with respect to HIV/AIDS response. Despite the impressive successes of global attempts to scale up HIV/AIDS treatment in recent years, driven by strong involvement of United States through its flagship PEPFAR program, HIV/AIDS financing has stagnated and there is risk that it may become victim of its own success - with widespread voices that we are witnessing the beginning of the end of AIDS, combatting the epidemic does not figure prominently among the new post-2015 development priorities. Since Mozambique, Tanzania and Uganda have been among the largest recipients of PEPFAR support, these recent developments are particularly worrisome. At the same time, all three countries have recently discovered vast fossil fuel reserves with potential to massively increase their economic growth and governments' budgetary capacity. Given the tremendous toll taken by HIV/AIDS in three countries in question, this paper advocates for substantial part of this potential fossil fuel rents to be invested in public intervention to curtail the epidemic. Using the AIDSCost simulation program developed by Garnett and Over (2012), it has found that scale-up efforts, both moderate and ambitious, have strong benefit-to-cost ratios between 3:1 and almost 6:1, providing evidence in favor of more ambitious interventions to mitigate HIV/AIDS.
