Dès le début des années 90, la situation sanitaire Française en la matière est particulièrement préoccupante. Les premières actions pour lutter contre ces résistances sont alors engagées en milieu hospitalier. A partir de 2001 et la validation du plan national pour préserver l'efficacité des antibiotiques, la lutte s'organise aussi en milieu communautaire. L'Assurance Maladie lance alors une campagne d'information visant à responsabiliser les médecins pour des prescriptions plus rationnelles, et à informer le grand public sur le bon usage des antibiotiques. Le pharmacien a un rôle prépondérant à jouer dans cette mission de santé publique. De part sa proximité et sa disponibilité, et en tant qu'éducateur de santé qu'il représente, un relais idéal auprès de la population pour une diffusion des informations, et un acteur efficace dans les actions de préventions.

D'ici à 2050, elle pourrait devenir l'une des principales causes de décès dans le monde. Face à l'émergence de ce phénomène, de nombreux plans de prévention nationaux et internationaux se sont succédés depuis le début du siècle. L'antibiorésistance est principalement lié au mésusage des antibiotiques, prescription inadéquate et sur consommation. Les patients ont également un rôle à jouer en respectant les posologies et les durées de traitement recommandées par le médecin ainsi qu'en évitant l'automédication. Afin d'évaluer les connaissances des patients sur les antibiotiques et connaitre leurs habitudes par rapport aux traitements, un questionnaire a été rédigé et distribué dans plusieurs officines franc-comtoises. Des comportements inadaptés ont été mis en évidence et ceux-ci peuvent impacter la résistance bactérienne. Le pharmacien d'officine peut agir face à l'antibiorésistance en mettant en place diverses actions de prévention. Lors de la délivrance des traitements, en rappelant le bon usage des antibiotiques, en collectant les médicaments non utilisés afin d'éviter l'automédication, en recommandant la vaccination ou grâce à la dispensation à l'unité de certains antibiotiques qui devrait être développée prochainement. Il est possible de limiter le recours aux antibiotiques grâce aux tests d'orientation diagnostique angine qui peuvent être réalisés en officine.

Many soon became dependent on routine antibiotic use to sustain and increase production. The resulting growth of antibiotic infrastructures came at a price. Critics blamed antibiotics for leaving dangerous residues in food, enabling bad animal welfare, and selecting for antimicrobial resistance (AMR) in bacteria, which could no longer be treated with antibiotics. Pyrrhic Progress reconstructs the complicated negotiations that accompanied this process of risk prioritization between consumers, farmers, and regulators on both sides of the Atlantic. Unsurprisingly, solutions differed: while Europeans implemented precautionary antibiotic restrictions to curb AMR, consumer concerns and cost-benefit assessments made US regulators focus on curbing drug residues in food. The result was a growing divergence of antibiotic stewardship and a rise of AMR. Kirchhelle’s comprehensive analysis of evolving non-human antibiotic use and the historical complexities of antibiotic stewardship provides important insights for current debates on the global burden of AMR.
