Book excerpt: Significant progress in the field of implant dentistry has led to various trend shifts with regard to treatment indications, implant parameters as well as surgical procedures. Compared to edentulous jaws in the past, the single-tooth gap represents the primary indication for implant placement today. Advances in oral regeneration have enabled implant therapy for more and more patients, and new implant designs as well as novel surgical techniques have broadened the spectrum of implant therapy.Recognizing changes in the field implant dentistry is crucial for the development of the field. Thus, we aimed to perform an analysis of a large patient pool to identify current trends and tendencies as well as possible risk factors for complications and early implant failure.We performed a retrospective analysis of patients referred to the Department of Oral Surgery of the Medical University of Vienna, University Clinic of Dentistry between January 2016 and November 2018. First, we evaluated subject characteristics (e.g., demographics, systemic conditions, medication). We then analyzed indications for implant placement (single-tooth gap, extended gap, distal extension, edentulous jaw), implant parameters (length, diameter, type, material), the timing of surgery (immediate, early, late) as well as the exact surgical procedures (e.g., bone augmentation, computer-aided navigation). Finally, we assessed postoperative complications (hematoma, local infection, swelling, nerve damage) as well as early (i.e., before loading) implant failures.A total of 1,132 patients (16u201390 years, mean 50.6 u00b1 16.5 years, 55.4% female, 19.3% smokers) received 2,413 implants. The majority of patients had 1 implant (n = 525, 46.7%). The maximum number of implants per patient was 10. A total of 1,028 implants (42.6%) were used in single-tooth gaps, 379 (15.7%) in extended edentulous gaps, 469 (19.4%) as distal extensions, and 537 (22.2%) in completely edentulous jaws. Implants 11.5 mm in length and 4.3 mm in diameter were the most common (19.6% of all implants). No implants shorter than 8 mm were used, and 86.4% of all implants were between 3.5 and 4.5 mm in diameter. A total of 1,879 (77.87%) implants were placed late (i.e., more than 6 months after first edentulousness) and 732 implants (30.3%) were supported by an augmentative measure. Computer-aided navigation was used for the placement of 226 implants (9.4%). A total of 71 patients (6.27%) reported a postoperative complication. Early implant failure occurred in 16 implants (0.7%).The data show a largely conservative approach to implant dentistry, as evidenced by the absence of short implants, the late time point of surgery, the low prevalence of computer-aided navigation as well as the reservation of bone augmentation for cases where they were indispensable. Our findings highlight a trend shift towards single-tooth replacements, similar to the recent literature. However, our data show a significantly lower prevalence of augmentative measures than comparable studies.

The sample size consisted of 464 consecutively treated completely and partially edentulous patients who had 1852 implants placed between 1979 to 1999. The effect of cigarette smoking on implant survival in relation to the time of implant failure, gender, age, implant surgeon, date of implant placement, length of implant, implant diameter, site of implant placement, design of the prosthesis and occlusal loading considerations was assessed in bivariate and multivariate survival analyses. An overall implant failure rate of 143 implants (7.72%) was found. Patients who were smokers at the time of implant placement surgery had a significantly higher implant failure rate (23.08%) as compared to non-smokers (13.33%) when the data were assessed on a patient basis. Furthermore, a multivariate survival analysis showed that early implant failure is significantly associated with smoking at the time of stage one surgery, implant length and site of implant placement and late implant failure is significantly associated with a positive smoking history of 25 cigarette years, implant length and site of implant placement. This study indicated that cigarette smoking has a negative effect on implant survival even after accounting for potential confounding variables. Other risk factors for implant failures exist and must be identified when considering the causes of dental implant failure.

However, although dental implant can achieve a fair rate of survival, still, some of them fail. The risk for implant failures is not evenly distributed among patients, which indicates that there are biological, mechanical, or behavioral risk factors that can cause implant failure. Among biological risk factors, drugs taken by patients are of special interest, due to the fact that most implant patients are elder adults that are often polymedicated.The success of implants relies mainly on osseointegration, a direct functional and structural interlocking between implants and bone. The process of osseointegration around implants is similar to the biological events occurring during bone repair and fracture healing. Dysregulation of bone metabolism is known to have a negative impact on bone healing and implant osseointegration. Many of the patients receiving implant treatments are taking different kinds of medications. Some drugs interfere with biological processes involved in bone metabolism.We hypothesize that medications interfering with bone metabolism could influence implant survival. The aim of our research is to identify risk associated with implant failure. Some example of such drugs are antihypertensive medications, selective serotonin reuptake inhibitors (SSRIs), and proton pump inhibitors (PPIs), etc., which are commonly prescribed for patients suffering from highly prevalent conditions such as hypertension, depression, and gastric acid reflux, respectively. The impact of these drugs on osseointegration is unexplored and important to know whether they could affect bone, osseointegration, and dental implants. In fact, our research is looking into the relationship between pharmacology, osseointegration and dental implants. It also may help to open the door for new pharmacological innovation that might improve the success of osseointegrated implants, and avoid unnecessary complications. This research project is of great relevance in health care science in general, and in dentistry in particular, as well as for dental implant patients taking certain medications. Accordingly, we believe that this project is of great importance to healthcare researchers, practitioner and patients.The three retrospective cohort studies included a cohort of patients with osseointegrated dental implant placement at the East Coast Oral Surgery clinic in Moncton, Canada, from January 2007 to September 2015. Kaplan-Meier estimator was used to describe the hazard function of dental implant failure in terms of the use of SSRIs, antihypertensive drugs, and PPIs, respectively. Multilevel mixed effects parametric survival analyses were used to test the association between medication exposure and risk of implant failure adjusting for potential confounders. The findings suggest that treatment with SSRIs or PPIs may be associated with an increased risk of osseointegrated dental implant failure, whereas antihypertensive drugs usage is associated with higher survival rate of dental implants.The publication of "Selective Serotonin Reuptake Inhibitors and the Risk of Osseointegrated Implant Failure: A Cohort Study" has been the most-read article of 2014 in the Journal of Dental Research.
