Updating information previously presented in the 1990 publication, Health Effects of Exposure to Low Levels of Ionizing Radiation: BEIR V, this book draws upon new data in both epidemiologic and experimental research. Ionizing radiation arises from both natural and man-made sources and at very high doses can produce damaging effects in human tissue that can be evident within days after exposure. However, it is the low-dose exposures that are the focus of this book. So-called “late” effects, such as cancer, are produced many years after the initial exposure. This book is among the first of its kind to include detailed risk estimates for cancer incidence in addition to cancer mortality. Book excerpt: NCRP Report No. 116 is the latest in the long series of reports on basic radiation protection criteria that began in 1934. It supersedes the predecessor in the series, NCRP Report No. 91, which was published in 1987. The current Report takes advantage of new information, evaluations and thinking that have developed since 1987, particularly the risk estimate formulations set out in NCRP Report No. 115. While the recommendations set out in this Report do not constitute a radical revision of the basic criteria, they do represent a refinement of the system enunciated in Report No. 91. Important changes include the utilization of revised tissue/organ weighting factors and the introduction of radiation weighting factors. Also noteworthy is the introduction of an allowable reference level of intake. Noteworthy too is the recommendation of an age-based lifetime limit for control of occupational exposures and a major simplification of limits aimed at controlling the exposure of the embryo and fetus. This Report, after outlining the goals and philosophy of radiation protection and the basis for exposure limits, goes on to review, in some detail, absorbed dose, equivalent dose, radiation weighting factors, and effective dose. Committed equivalent dose and committed effective dose are also introduced. Risk estimates for radiation exposure are presented and then the dose limits are enunciated. The Report also covers exposure in excess of the limits, limits for unusual occupational situations, guidance for emergency occupational exposure, and remedial action levels for naturally occurring radiation.

Although these 'recently' published materials have essentially reached the same numerical results for radiation levels at which observations are available, there have been differences in philosophy and divergent conclusions regarding the estimation of the risk associated with low-level, low dose rate radiation, for which little or no data are available. Further, and perhaps more importantly, widely varying interpretations of the significance and meaning of these reports and conclusions have been formulated. In an endeavor to consider carefully the implications of the various 'new' recommendations in relation to 'current' NCRP radiation protection philosophy, the Board of Directors of the Council determined to undertake an intensive analysis of the published materials and the underlying background. To initiate this work, the NCRP constituted the Ad Hoc Committee on Comparison of Radiation Protection Philosophies. The Committee developed background material for consideration of this topic by the Board of Directors of the Council. The Board determined that a formulation of the NCRP's position would constitute an important contribution and, utilizing the material developed by the Ad Hoc Committee, together with review by the Council, produced this report." --From Preface, page iii.

Topics addressed in this Report include: (a) uncertainties associated with extrapolation of dose-response relationships observed in primary epidemiological studies to estimate the risk per unit dose (i.e., organ dose or whole-body dose) in the U.S. population and other exposed populations; (b) applications of meta-analyses or pooled analyses to increase the statistical power in evaluating uncertainties in dose-response relationships for exposed human populations; (c) uncertainties associated with extrapolation of dose-response relationships observed for populations exposed to acute doses of high-energy gamma rays to estimate the risk per unit dose in populations exposed to fractionated or low-dose rate chronic exposures; (d) uncertainties associated with extrapolation of the dose-response relationships observed for populations exposed to high-energy gamma rays to estimate the risk per unit dose in populations exposed to low-energy photons, low-energy electrons, alpha particles, and neutrons with various energies; (e) comparison of uncertainties associated with risk estimated for individual tissue or organ sites with the uncertainties associated with estimating risk of all tumors combined due to whole-body exposure; (f) evaluation of opportunities for using additional epidemiological and laboratory-based biological information to modify estimates of uncertainty in risk estimation for cancer, noncancer effects, and severe heritable disorders; (f) procedures for accounting for dose uncertainty in epidemiological dose-response analyses; and (g) evaluation of the combined effect of uncertainty in dose estimation with the uncertainty in estimation of risk per unit dose in estimating the overall risk. This Report also provides a comprehensive analysis of uncertainties on the estimation of probability of radiation-induced disease, including: (1) application of organ doses and associated uncertainties in estimation of probability of disease causation; (2) evaluation of inherent uncertainties in calculating the probability of disease causation (in an individual), or assigned share of excess relative risk for various types of cancer attributable to radiation exposure; and (3) methods of improving existing procedures for estimating disease probability based on input organ doses and their uncertainties. The issue of uncertainty in estimation of radiation-induced risks of cancer, noncancer diseases, and heritable genetic effects analyzed in this Report is of great importance in evaluating the effects of ionizing radiation on human health, in decisions involving the safe use of ionizing radiation and in addressing public controversy. Uncertainty analyses should become increasingly important in the future as the sophisticated methodologies continue to develop and become more available.

Book excerpt: NASA's current missions to the International Space Station (ISS) and potential future exploration missions involving extended stays by astronauts on the lunar surface, as well as the possibility of near- Earth object (NEO) or Mars missions, present challenges in protecting astronauts from radiation risks. These risks arise from a number of sources, including solar particle events (SPEs), galactic cosmic rays (GCRs), secondary radiation from surface impacts, and even the nuclear isotope power sources transported with the astronauts. The serious early and late radiation health effects potentially posed by these exposures are equally varied, ranging from early signs of radiation sickness to cancer induction. Other possible effects include central nervous system damage, cataracts, cardiovascular damage, heritable effects, impaired wound healing, and infertility. Recent research, much of which has been sponsored by NASA, has focused on understanding and quantifying the radiation health risks posed by space radiation environments. Although many aspects of the space radiation environments are now relatively well characterized, important uncertainties still exist regarding biological effects and thus regarding the level and types of risks faced by astronauts. This report presents an evaluation of NASA's proposed space radiation cancer risk assessment model, which is described in the 2011 NASA report, Space Radiation Cancer Risk Projections and Uncertainties-2010. The evaluation in Technical Evaluation of the NASA Model for Cancer Risk to Astronauts Due to Space Radiation considers the model components, input data (for the radiation types, estimated doses, and epidemiology), and the associated uncertainties. This report also identifies gaps in NASA's current research strategy for reducing the uncertainties in cancer induction risks.
