Book excerpt: Introduction : Depuis quelques années le végétarisme est en plein développement, ce qui implique que les professionnels de santé sont amenés à voir des patients végétariens en consultation. L "objectif de ce travail était d"étudier le ressenti des patients végétariens lors de leur confrontation au monde médical et plus particulièrement en consultation de médecine générale. Matériel et méthodes : ous avons mené une étude qualitative par entretiens individuels semidirigés auprès de 17 patients végétariens. Les entretiens ont été enregistrés, retranscris puis coder à l'aide du logiciel RQDA. Résultats : Les personnes interrogées rapportaient des réactions souvent négatives de la part des professionnels de santé. ainsi que la persistante de nombreux préjugés, un manque de dialogue et la sensation que ces derniers avaient peu de connaissances concernant leur alimentation. Même si dans la plupart des cas, el les n · ont pas changé de médecin traitant, cene situation pouvait avoir des conséquences sur la qualité de la relation médecin-patient, notamment J" omission de certaines informations. Discussion : Deux autres thèses se sont intéressées aux patients végétariens et au point de vue des médecins. et elles constataient également un manque dïnformations et de compréhension de la part des professionnels de santé. Cene situation est probablement liée au fait que contrairement à d'autres pays (comme le Canada et les Etats-Unis), les institutions référentes de la santé, ne sont pas très favorables au végétarisme. Mais c'est une position qui tend à évoluer. Conclusion : Les personnes interrogées étaient globalement déçues de !"accueil et de la prise en charge de leur médecin traitant. Une plus grande tolérance et une mise à jour des connaissances sur le végétarisme pourrait permettre une amélioration des relations médecin-patient.

Nous avons choisi de connaître le positionnement des médecins généralistes libéraux en Franche-Comté dans la prise en charge des patients suivant un de ces régimes.Matériels et méthodes: Une analyse descriptive quantitative a été réalisée auprès de 107 médecins généralistes et internes ayant déjà réalisé leur SASPAS, via la diffusion d'un questionnaire durant 4 mois.Résultats: De manière générale, le régime alimentaire des patients est abordé principalement en fonction du contexte. Concernant le suivi, les médecins ruraux ou formés ou encore suivant un régime ont tendance à suivre ces patients plutôt qu'à l'adresser vers un confrère spécialiste. Enfin, devant un patient mineur ou végétalien, les médecins s'estiment plus en difficultés, prescrivent des biologies de manière plus systématique et préfèrent un suivi conjoint ou exclusif avec un spécialiste. Discussion: L'abord de la nutrition est souhaité par les patients alors que les médecins craignent la réticence de ces derniers. De plus, le manque de connaissances et de temps, autant en consultation que pour la formation, limite son évocation. Si certains de ces régimes appartiennent déjà à l'arsenal thérapeutique, par exemple le cas du diabète, dans certains pays, il n'existe pas encore de recommandation en France. Conclusion: La place de la nutrition en médecine générale est encore à renforcer. Une meilleure sensibilisation des médecins et des recommandations claires donneraient une réelle place à cette dernière.

Ce travail étudie l'avis des MG de Franche-Comté sur le végétarisme et le végétalisme en pédiatrie, leurs connaissances, et l'influence sur leur pratique. Matériel et méthodes : L'étude était qualitative. Trois focus group, et un entretien avec une pédiatre ont été réalisés. Ceux-ci ont été enregistrés, retranscrits avec le logiciel Word, et doublement codés. Résultats : l'alimentation exclusivement végétale est imposée et motivée par éthique anitnale, écologique, ou effet de mode ; avec de graves risques carentiels. Les MG expriment un 1nanque de connaissances par défaut de formation 1nédicale, et le besoin de reconunandations validées. ·une bonne communication est nécessaire pour sauvegarder la relation de confiance et le suivi. Discussion : les données de la science confinnent l'avis des MG et le manque de formation. Néanmoirts, le GFHGNP et la SFP ont publié des reco1nmandations. Et alors que les MG de l'étude n'orientent vers un spécialiste qu'en cas de signe d'alerte, celles-ci recommandent un suivi spécialisé systé1natique. Conclusion : des recommandations claires et adaptées à la pratique en médecine générale sont nécessaires ; ainsi que! 'accessibilité à des fonnations. II est pertinent d'améliorer la prévention, le carnet de santé pourrait en être un outil clé.
