Book excerpt: ABSTRACT: Background: Osteoporosis is a bone disease that causes decreased bone mass and bone density, increasing the risk of fracture and decreasing health-related quality of life (HRQoL). Strength training and other exercise interventions are often prescribed to treat low bone mineral density. The purpose of this review is to determine if strength and resistance training can decrease pain levels or increase HRQoL. Methods: A comprehensive search was performed in the following databases using a pre-selected string: PubMed, CINAHL, Trip, Pedro, EBSCO, and MEDLINE. The inclusion criteria consisted of: articles that used strength or resistive training methods as an intervention on postmenopausal women with either osteoporosis or osteopenia, and applied an outcome measure incorporating HRQoL and/or pain. Articles were excluded if participants presented with interfering health conditions, the exercise intervention did not include a strength training aspect, patients were being treated pharmacologically, with exceptions of calcium and vitamin D supplements, and if the articles were not written in English. Results: Six articles were included in this systematic review. Analysis of this literature indicates that strength training improves HRQoL and decreases pain in postmenopausal women with osteoporosis. Discussion/Conclusion: Little peer-reviewed evidence exists for strength training interventions in women with osteoporosis. All evidence supported interventions of at least 10 weeks in length, with the longest intervention spanning 2 years. Strength training alone was found to significantly decrease pain in 2 studies. Interventions combining strength training and other forms of exercise were found to increase HRQoL significantly more than strength training alone in women with osteopenia and osteoporosis.

Book excerpt: A meta-analysis was done to determine what effect exercise has on the bone mineral density (BMD) of the lumbar spine, proximal femur, and distal radius in postmenopausal women, utilizing data from 18 studies. Treatment effects (TE) were calculated for each of the skeletal sites, aerobic and strength training, as well as methods of BMD measurement (dual energy x-ray absorptiometry and dual photon absorptiometry). The resulting TEs found exercise, in general, to significantly increase (p c 0.05) the BMD of the lumbar spine (0.73%), and the proximal femur (0.35%); however, the distal radius showed a decrease ( -0.91%) in BMD. When looking for the effect of aerobic versus strength training, significant (p c 0.05) increases in BMD were found in the spine and femur. Since positive changes were not seen in all sites, the benefits of exercise may be specific to the skeletal sites which the working muscles are attached. When the % change for exercise and control groups were analyzed separately, the exercise group increased 2.31% at the lumbar spine and 0.61% at the proximal femur. However, the exercise group decreased -2.21% at the radius. The control group decreased at the lumbar spine -1.09%, and at the femur -1.1 I%, however, increased at the radius 0.36%. The BMD % changes were significantly larger for the spine and femur, therefore, vigorous exercise along with resistance training may help slow the decrease in BMD as one ages.

Book excerpt: We studied the role of body composition on C-reactive protein (CRP) and interleukin-6 (IL-6); determine the differences in CRP and IL-6 among HT users and non-users; determine the effect of 12-months of resistance-training and resulting body composition changes on IL-6 and CRP; determine the relationship between BMD and IL-6, CRP and creatine kinase (CK). Sedentary women (N=208, age 44-66, postmenopausal) taking HT (N=106) or not taking HT (N=102) were randomly assigned to resistance-training: HT-exercise (N=55), HT-no exercise (N=45), no HT-exercise (N=53), and no HT-no exercise (N=49). The program included three weekly 60-75 minute sessions of 8 exercises performed in 2 sets of 6-8 repetitions at 70-80% of 1RM. Total fat mass (TFM), lean soft tissue mass (LSTM) and BMD were assessed by dual-energy X-ray absorptiometry (DXA). For each subject, baseline and 1-year IL-6, CRP and CK were measured. High TFM, high IL-6 and HT use were independently associated with high CRP levels. A stronger relationship between IL-6 and AFM compared to TFM was found in HT users. High TFM and LSTM were significantly related to higher IL-6 levels. A stronger relationship between CRP and AFM compared to TFM was found in HT non-users. CRP was higher in HT users (5.47ł5.40 mg/L) compared to non-users (2.70ł3.05 mg/L) and was higher in oral (5.76ł5.29 mg/L opposed; 6.14ł5.97 mg/L unopposed) compared to transdermal HT users (2.65ł4.44 mg/L). CRP increased slightly (p=0.077) in exercisers (0.54 ł 0.34 mg/L) not taking HT compared to controls ( -0.39 ł 0.35 mg/L). Reductions in TFM were associated with reductions in IL-6 and CRP in HT users. CK was positively associated to all BMD sites at baseline. IL-6 change was positively associated with change in femur neck BMD. CRP change was inversely correlated with change in lumbar spine BMD. CK change was directly related to change in total body and femur trochanter BMD. Reductions in TFM were accompanied by reductions in IL-6 and CRP; AFM was more strongly correlated with inflammation than TFM; 12-months of resistance-training did not decrease IL-6 or CRP; IL-6 and CK were positively related to BMD, and CRP was inversely associated with BMD.

However, there are limited data on the long-term effects of weight-bearing exercise on BMD in adult males, particularly in men with osteopenia. PURPOSE: To 1) determine the effects of six months of resistance training (RT) or plyometrics (PLYO) exercise on changes in BMD in males with osteopenia; and 2) determine the effects of six months of RT or PLYO exercise on changes in bone turnover in males with osteopenia. METHODS: Twenty-one recreationally active (>4 h/wk of activity) healthy males (25-55 y) with osteopenia in the hip or lumbar spine (L1-L4) were randomized into a 6-mo RT (N = 9) or PLYO (N = 8) exercise program. Dual energy X-ray absorptiometry was used to measure bone area, BMC, and BMD of the lumbar spine (L1-L4), total left hip, and whole body (WB). Serum concentrations of osteocalcin (OC), bone-alkaline phosphatase (BAP), tartrate-resistant acid phosphatase isoform 5b (Trap5b) and carboxyterminal telopeptide of type I collagen (CTX) were measured determine bone turnover activity. RESULTS: Two-factor repeated measures ANOVA showed that WB BMD increased after the 6-mo exercise interventions (p=0.070) with no differences between RT and PLYO groups (+1.32 and 0.52 %, respectively). Although there were no statistically significant changes in bone turnover markers, the BAP/CTX (p=0.036) and OC/CTX ratios (p=0.077) were significantly increased after the exercise intervention with no differences between RT and PLYO. CONCLUSION: We are the first to show that participation in 6 mo of RT or PLYO can improve WB BMD and bone turnover ratios in osteopenic men.

It is also associated with maintenance of bone mineral density (BMD) - critical to minimizing fracture risk and associated healthcare costs that accompany advancing age. bioDensity(TM) training is a low-volume high-intensity RT approach that involves four exercises performed for 5-seconds each once per week. The four exercises engage most major muscle groups, are performed at voluntary-maximal intensity over a limited-range and are intended to elicit multiple body weight loading of the skeleton. PURPOSE: To determine whether a 24 week bioDensity(TM) intervention improves BMD and health/fitness factors in osteopenic/osteoporotic adults. METHODS: 15 participants are currently training with bioDensity(TM) 1x/week; nine females (59.8±5.0 yrs; 24.0±4.4 kg/m2) have completed 24-week testing. Baseline and 24-week testing includes BMD and body composition (DEXA), Y-balance test, Senior Fitness Test, floor-to-stand test, and muscular force production measurements.
