Book excerpt: Introduction : en France, les troubles mentaux représentent 15 à 40 % des consultations chez le médecin généraliste (MG). Toutefois, la stigmatisation autour de la santé mentale persiste. L’objectif principal de notre étude était de déterminer les représentations des MG de l’Oise sur la psychiatrie. Ensuite, nous avons évalué leurs pratiques face aux patients présentant des troubles mentaux et leur collaboration avec les psychiatres. Méthode : étude quantitative descriptive effectuée sur un échantillon de 290 MG tirés au sort. Les questionnaires, anonymes, étaient envoyés par courrier postal ou courriel. Le taux de réponses était de 34,67 %. Résultats : la majorité des répondants portait un regard négatif sur les psychiatres et les patients présentant des troubles mentaux. 84 % des MG pensaient que les patients psychiatriques étaient éprouvants et 60 % jugeaient que les psychiatres n’étaient pas intéressés par la médecine somatique. La proximité avec une personne présentant des troubles mentaux et l’expérience professionnelle en psychiatrie auraient un impact positif sur les représentations. La coopération entre MG et psychiatres s’avérait complexe. Les principaux obstacles seraient liés à la question des courriers et aux difficultés d’orientation des patients. Discussion : nos résultats sur les représentations vont dans le même sens que ceux de Stuart et al (2015). Nos conclusions sur la collaboration entre les MG et les psychiatres sont retrouvées dans de nombreuses études. Conclusion : changer les représentations en santé mentale nécessite une meilleure connaissance mutuelle entre la médecine générale et la psychiatrie ainsi qu’une amélioration de la communication et de la formation théorique et pratique.

Les patients souffrant de pathologies psychiatriques ont une morbi-mortalité plus élevée que la population générale. Le médecin généraliste est souvent le premier professionnel de santé consulté dans ce contexte, il est particulièrement exposé aux soins psychiatriques : 30% de sa patientèle présenterait une souffrance mentale ou une pathologie psychiatrique. Une collaboration de qualité entre médecins généralistes et psychiatres est importante pour améliorer la prise en charge des patients. Cependant, la collaboration est jugée insuffisante du fait d'une communication pauvre voire inexistante. Cette mauvaise collaboration a un effet négatif sur la prise en charge des patients du fait du risque de perte d'information. Le développement de cette collaboration est important pour améliorer la prise en charge des patients. L'objectif de l'étude était de déterminer comment améliorer la collaboration entre médecins généralistes et psychiatres et plus particulièrement dans la prise en charge de patient dépressifs. Matériel et méthodes : étude qualitative réalisée à partir de 10 entretiens individuels et semi-dirigés réalisés auprès de médecins généralistes installés dans l'Oise, analysés selon la méthode de théorisation ancrée. Résultats : les résultats mettent en évidence un besoin important de collaboration les médecins de l'étude estimaient prendre en charge une quantité croissante de patients dépressifs au sein de leur patientèle. Ils expliquaient cette situation par la diminution des psychiatres libéraux. Les médecins généralistes jugeaient que le recours au psychiatre leur paraissait plus difficile que pour les autres spécialités. La qualité des relations entre médecin généraliste et psychiatre apparaissaient mauvaise dans l'ensemble. Ils avaient l'impression de ne pas être pris en compte dans la prise en charge du patient par l'absence de partage de stratégie thérapeutique ou l'absence de sollicitation. La qualité de leur collaboration était jugée insuffisante. Ils exprimaient un besoin de reconnaissance sur leur rôle de coordonnateur et de partenaire de soin. Discussion : les médecins généralistes sont conscients du partenariat qu'ils doivent établir avec les psychiatres pour une prise en charge de qualité du patient. Une majorité des médecins généralistes se déclare prêt à s'investir dans un projet multi partenarial de prévention et de promotion de la santé mentale. Notre étude pourrait être étendue aux ressentie des psychiatres. Conclusion : les échanges entre le médecin généraliste et le psychiatre sont pauvres voire inexistant. Au vu de l'impact économique et de la prévalence de la dépression en France, cette collaboration doit être améliorée. A une époque où le médecin semble être le référent principal en pathologie mentale, son manque de formation et son sentiment d'abandon rendent difficile une prise en charge de qualité. Cette étude effectuée auprès de généralistes de l'Oise semble être le reflet d'une situation nationale.

Book excerpt: Selon une étude rétrospective menée au sein du Centre Hospitalier Le Vinatier en 2014, 59% des patients suivis sur l'hôpital n’ont pas de médecin généraliste (MG). Suite à cette constatation, une consultation médico-infirmière est expérimentée sur deux secteurs psychiatriques pour réaliser une synthèse médicale et orienter ces patients vers un MG. Afin d'améliorer le fonctionnement de la consultation réseau, une étude transversale descriptive par questionnaire retournés par voie postale ou électronique a été entreprise auprès des 224 MG des deux secteurs concernant les freins et difficultés qu'ils rencontrent. 78 questionnaires réponse ont été obtenus (taux de participation de 34,8%). 76% des MG ont des difficultés pour prendre en charge leurs patients porteurs de pathologies psychiatriques. 83% ont des difficultés d’orientation pour ces patients. 80 % ne sont pas satisfaits de la collaboration avec les professionnels de santé mentale. Les difficultés rencontrées sont liée à une mauvaise communication interprofessionnelle, aux délais d'attente en CMP, à la complexité des problématiques en santé mentales et à une mauvaise connaissance du système de soin en santé mentale. La consultation réseau est un outil pertinent et efficace pour faciliter un lien collaboratif de qualité entre les acteurs de la prise en charge globale des patients suivis en santé mentale. L'outil actuel est destiné à orienter les patients de la file active de l'hôpital du Vinatier vers un suivi ambulatoire par un MG. Dans un second temps nous avons imaginé et proposons un dispositif de soutien aux soins en santé mentale plus élaboré et destiné à faciliter la prise en charge d'un maximum de situations problématiques, destiné à tous les acteurs impliqués dans la prise en charge globale de ces patients

The Cognitive Behavioural Tsunami: Managerialism, Politics, and the Corruptions of Science provides a powerful critique of CBT’s understanding of human suffering, as well as the apparent scientific basis underlying it. The book argues that CBT psychology has fetishized measurement to such a degree that it has come to believe that only the countable counts. It suggests that the so-called science of CBT is not just "bad science" but "corrupt science". The rise of CBT has been fostered by neoliberalism and the phenomenon of New Public Management. The book not only critiques the science, psychology and philosophy of CBT, but also challenges the managerialist mentality and its hyper-rational understanding of "efficiency", both of which are commonplace in organizational life today. The book suggests that these are perverse forms of thought, which have been institutionalised by NICE and IAPT and used by them to generate narratives of CBT’s prowess. It claims that CBT is an exercise in symptom reduction which vastly exaggerates the degree to which symptoms are reduced, the durability of the improvement, as well as the numbers of people it helps. Arguing that CBT is neither the cure nor the scientific treatment it claims to be, the book also serves as a broader cultural critique of the times we live in; a critique which draws on philosophy and politics, on economics and psychology, on sociology and history, and ultimately, on the idea of science itself. Histories, novels, and films have cloaked his life in hagiography and legend. Jacalyn Duffin's fascinating new biography relies on a vastly expanded foundation of primary source material, including thousands of pages of handwritten patient records, lecture notes, unpublished essays, and letters. She situates Laennec, the scientist and teacher, within the broader social and intellectual currents of post-Revolutionary France. Her work uncovers a complex character who participated actively in the dramatic changes of his time. Laennec's famous Treatise on Mediate Auscultation was his only published book, but two lesser known works were left in manuscript: an early treatise on pathological anatomy and a later set of lectures on disease. The three parts of Duffin's biography correspond to these books. First, she examines Laennec's student research on the emerging science of pathological anatomy, the background for his major achievement. Second, she uses his clinical records to trace the discovery and development of "mediate auscultation" (listening through an instrument, or mediator, to sounds within the human body). The stethoscope allowed clinicians to "see" the organic alterations inside their living patients' bodies.
