Available in PDF, EPUB and Kindle. Book excerpt: At a time when the world’s wealthiest nations struggle to make health care and medicine available to everyone, why do resource-constrained countries make costly commitments to universal health coverage and AIDS treatment after transitioning to democracy? Joseph Harris explores the dynamics that made landmark policies possible in Thailand and Brazil but which have led to prolonged struggle and contestation in South Africa. Drawing on firsthand accounts of the people wrestling with these issues, Achieving Access documents efforts to institutionalize universal healthcare and expand access to life-saving medicines in three major industrializing countries. In comparing two separate but related policy areas, Harris finds that democratization empowers elite professionals, such as doctors and lawyers, to advocate for universal health care and treatment for AIDS. Harris’s analysis is situated at the intersection of sociology, political science, and public health and will speak to scholars with interests in health policy, comparative politics, social policy, and democracy in the developing world. In light of the growing interest in health insurance generated by implementation of the Affordable Care Act (as well as the coming changes poised to be made to it), Achieving Access will also be useful to policymakers in developing countries and officials working on health policy in the United States.

Available in PDF, EPUB and Kindle. Book excerpt: This book empirically examines health care financing reforms and popular responses in three major cities in East Asia: Shanghai, Singapore, and Hong Kong. It adopts a new revised version of the theory of historical institutionalism to compare and explain the divergent reform paths in these three places over the past three decades. It also examines forces that propel institutional change. The book provides three detailed case studies on the development of health care financing reforms and the politics of implementing them. It shows that health care systems in Shanghai, Singapore, and Hong Kong were the products of Western presence in the nineteenth century. It illustrates how greater attention is paid to the roles played by ideas, actors, and environmental triggers without abandoning the core assumptions that political institutions and policy feedback remain central to impact health care financing reforms. It shows that health care financing reform is shaped by a complex interplay of forces over time. It also provides the most updated material about health care financing reforms in Shanghai, Singapore, and Hong Kong. The central argument of this book is that health care financing reform is both an evolving process responding to changing circumstances and a political process revealing an intricate interplay of power relationships and diverse interests. It shows that institutional changes in health care financing system can be incremental but transformative in nature. It argues that social policies will continue to develop and welfare states will continue to adapt and evolve in order to cope with new risks and needs. This book sheds new lights on understanding the politics of health care financing reform and sources and modes of institutional change.

Available in PDF, EPUB and Kindle. Book excerpt: Following the detection of the first HIV infections in the early 1980s, by the 1990s Thailand was routinely depicted as having the world’s fastest moving HIV/AIDS epidemic. However, by the early 2000’s the bulk of scholarly and medical AIDS literature portrayed the epidemic as being largely under control, and claimed that Thai AIDS prevention efforts during the 1990s had been successful. Based on long-term ethnographic research conducted in Northern Thailand this book makes an in-depth study of the social construction of Thailand’s HIV/AIDS epidemic over this period. In addition to his own field research the author draws on an extensive corpus of English and Thai language social science and medical HIV/AIDS literature to examine the modeling of Thailand’s AIDS epidemic, and addresses concepts and issues such as risk groups, risk behaviour, alcohol use, gender and class, masculinity, the scapegoating of female prostitutes and men in the underclass, the reporting of the HIV/AIDS epidemic in Thailand’s indigenous Thai language media, and sexual activity amongst Thai youth. The analysis demonstrates the contribution of anthropology as an interpretative social science, and the use of anthropological theory and research methods, to finding alternative ways of framing the problems of Thai AIDS and of posing new questions that will lead to more effective points of intervention. It emphasises the necessity for critically reflexive approaches that question the ‘taken for granted’ and demonstrates how qualitative research techniques guided by social theory have the potential to take account of local meanings in complex social contexts where traditional values and cultural practices are rapidly transforming due to economic and social change. The book offers a sustained and powerful criticism of the limitations of the normative model of the Thai AIDS epidemic and, in its aim of promoting critically reflexive AIDS research techniques in order to produce a better understanding of issues ‘on the ground’ and hence better health policy and more effective AIDS interventions, speaks not only to the Thai AIDS epidemic but to AIDS epidemics throughout Southeast Asia and elsewhere. This is the only English language study of Thailand’s HIV/AIDS epidemic to draw on long-term qualitative research in Northern Thailand as well as on a broad range of Thai (and some Khmer language) materials. Its contextualised and subtly nuanced analysis of the AIDS epidemic and of the impact of AIDS control initiatives, in concert with the theoretical and methodological contributions it makes to AIDS research and policy and behavioural interventions, makes it a timely publication of vital interest to scholars in the social sciences, as well as to the members of non-governmental organisations and international organisations working in the HIV/AIDS, health and development fields.

Book excerpt: Hariti is the ancient Indian goddess of childbirth and women healers, known at one time throughout South and Southeast Asia from India to Nepal and Bali. Daughters of Hariti looks at her 'daughters' today, female midwives and healers in many different cultures across the region. It also traces the transformation of childbirth in these cultures under the impact of Western biomedical technology, national and international health policies and the wider factors of social and economic change. The authors ask what can be done to improve the high rates of maternal and infant deaths and illnesses still associated with childbirth in most societies in this area and whether the wholesale replacement of indigenous knowledge by Western biomedical technology is necessarily a good thing.
