Book excerpt: "Drawing on a comparative study of hospital records, Charity and the London Hospitals investigates how and why Victorians contributed in order to show that benevolence was rarely amenable to a single form or reason. Whilst charity remained central to the hospitals' raison d'etre, philanthropy's contribution was modified at a financial and administrative level as hospitals shifted from being philanthropic to medical institutions. Why this process occurred and the impact of professionalisation and scientific medicine are assessed."--BOOK JACKET.

Book excerpt: Many doctors do not receive training early in their careers on the broad range of non-clinical aspects of their work, and confront day-to-day issues for which initial medical education has failed to prepare them. Experienced doctors and consultants can also experience a similar lack of accessible reference material on these aspects of their role and for the non-clinical training of their juniors. This book and its companion volume The Doctor's Handbook Part 1: managing your role beyond clinical medicine, have been written to address these and other needs. Previously published as The Specialist Registrar and New Consultant Handbook, these completely revised and reconfigured volumes reflect the changing everyday work of specialist trainees, registrars and consultants. Topics covered in Volume 2 include: *

Book excerpt: To maximise the effectiveness of their work, NHS employees need a clear understanding of the structures and systems of the organisation in which they work. However, this information can be widely spread, hard to access and difficult to gain a working overview of, and the pace of changes and initiatives can seem almost dizzying. This book draws together a clear picture of the modern NHS, from funding and governance to reports, inquiries and overarching legislation. The book is clear and easy to understand and crucially includes full references to provide a one-stop point of access to the most detailed and up-to-date information available. This book is essential reading for workers in the NHS at all levels, including managers, administrators and clinical professionals. It is vital reading for managers and staff at commercial companies working with the NHS. It will also be of interest to campaigners, patient interest groups, researchers and journalists with an interest in the NHS. Specialty registrars and consultants can also find the information from the book, and much more, in The Doctor's Handbook Parts 1 & 2, by the same author. 'Rather than long narrative histories or complex explanations, the author signposts readers to sources of further information, making this book the quick guide so many of us need' - from the Foreword by Sir Ian Carruthers

Available in PDF, EPUB and Kindle. Book excerpt: The NHS came into existence in an atmosphere of conflict centred on the strong ideological commitment of the Post-war Labour Government and the opposition of the Conservative Party of that time to the idea of a universally available and centrally planned medical care service. There was also opposition from some sections of the medical establishment who feared the loss of professional autonomy. Setting health policy in both an historical and modern context (post 1997) Carrier and Kendall weigh up the successes and failures of the National Health Service and examine the conflicts which have continued for over sixty years, in spite of efforts to solve financial problems in the NHS through increases in funding as well as structural and organisational change. After looking at recent responses to supposed failures of the NHS, they conclude that the NHS has successfully faced the challenges before it and is likely to continue to meet the changing health needs of the population. Financial stresses, concerns about the quality of care and demographic change, with consequent issues for the elderly and the chronically ill, continue to be urgent and politically contentious issues. This book is appropriate for a wide range of undergraduate and postgraduate students studying health policy and the NHS.

Book excerpt: Although the number of elderly people in Britain is increasing steadily, the law in relation to their particular needs is a relatively neglected field. The Law and Elderly People was the first text to provide easily accessible information for those involved in advice-giving and service provision in this rapidly developing field. This second edition has been fully revised and completely updated to reflect the major legal and social changes which have recently occurred. Concerned for the independence and autonomy of both young elderly and the old elderly, the book covers employment and income, accomodation and housing, community and residential care, health provision and delivery and family relationships, thus providing an important guide to the most important legal issues. The Law and Elderly People will be of practical help to all those concerned with the welfare of the elderly and to undergraduates and lecturers in social work, law and gerontology.

Available in PDF, EPUB and Kindle. Book excerpt: This book analyses changes which have occurred in the organization and management of the UK public services over the last 15 years, looking particularly at the restructured NHS. The authors present an up-to-date analysis around three main themes: the transfer of private sector models to the public sector, the management of change in the public sector, and management reorganization and role change. In doing so they examine to what extent a New Public Management has emerged, and ask whether this is a parochial UK development or of wider international significance. This is a topical and important issue in management training, professional, and policy circles. Important analytic themes include: an analysis of the nature of the change process in the UK public services, characterization of quasi markets, the changing role of local Boards, and possible adaptation by professional groupings. The book also addresses the important and controversial question of accountability, and contributes to the development of a general theory of the New Public Management.

Book excerpt: Surgery in London and The Royal College of Surgeons of England 'Opportunities and Pitfalls' The Royal Commission on Medical Education (the Todd Report) in 1968 supported by the Flowers Report in 1980 recommended major changes to undergraduate and postgraduate medical education. London was considered separately because of the disproportionate number of teaching hospitals, medical schools, specialist hospitals and research institutes. After the Second World War, extraordinary advances in the medical sciences, engineering, high technology and surgical techniques dramatically expanded the spectrum of disease amenable to surgical treatment. The UK remained non-compliant with EU directives on the 'harmonisation' of specialist training until the 1990s and this revolutionary era which culminated in a transformation of surgical training and practice, is described. An increasing number of elderly patients, and others with comorbidities, are now candidates for complex procedures and at higher risk of complications and death. The ongoing controversy regarding the optimal organisation of operative and postoperative care for these patients is discussed. An overview, currently lacking, of more than 700 years of recorded surgical history provides requisite historical context including details of commissioned reports and some of the most consequential post-war innovations. The sometimes adversarial relationship of diplomates with the Royal College of Surgeons of England and its predecessors is examined. In the summer of 2020, Professor Neil Mortensen the newly elected President of the Royal College of Surgeons of England invited Baroness Helena Kennedy, QC to produce a 'Diversity and Inclusion Review' that was published on 18 March 2021. Sadly, the report concluded that the college and the profession are systemically 'sexist, racist and homophobic. The college responded with its 'Diversity, Equity and Inclusion Action Plan' (DEI) dated 16 September 2021 and the subject is considered. The purpose of this mini-monograph is to provide a concise account of the evolution of surgical training and practice as it affected London and the rôle of the Royal College of Surgeons of England and its predecessors.
