Ce sont des maladies inflammatoires chroniques non transmissibles associées à un déséquilibre du microbiote buccal. Ce microbiote correspond aux bactéries retrouvées dans le biofilm communément appelé plaque dentaire. Ce microbiote entraîne une inflammation au niveau de la gencive c'est la gingivite, puis si elle s'étend au parodonte profond (tissu conjonctif et osseux) elle devient la parodontite. L'inflammation est nécessaire à l'élimination des bactéries, mais si elle est excessive ou persiste après le traitement, elle peut être délétère pour les tissus. Des facteurs favorisants, comme l'état de stress émotionnel chronique du patient, participent aussi à l'évolution de la maladie et perturbent la cicatrisation après traitement, en agissant sur la réponse inflammatoire et par conséquent sur le microbiote. Les mécanismes qui entrent en jeux sont complexes et ne sont pas encore complètement connus. Ce travail de thèse est une revue de la littérature dont le but principal est d'explorer ces mécanismes. Nous évoquerons les effets pro et anti-inflammatoires de l'axe hormonal du stress, son rôle dans l'apparition du stress oxydatif, les liens entre stress microbiote oral et intestinal, ainsi que le concept de charge allostatique en lien avec l'apparition ou l'aggravation d'une maladie parodontale. La compréhension de ces mécanismes est un départ important pour faire évoluer la prise en charge des patients atteints de parodontite en prenant plus en compte leur état psychologique.
