Book excerpt: Introduction : les carcinomes épidermoïdes de la cavité buccales représentent 95% des causes des pertes de substances mandibulaires. Les retentissements fonctionnel et esthétique de cette chirurgie ne sont pas à méconnaître. Les résections osseuses sont plus ou moins complètes en fonction de la classification TNM de la tumeur. La reconstruction mandibulaire par un lambeau de fibula est devenue [t9]le gold standard. Malheureusement toutes les pertes de substances mandibulaires ne sont pas réhabilitées. Le but de cette étude était d'évaluer les réhabilitations orales dans les suites d' une perte de substance mandibulaire chez les patients ayant un carcinome épidermoïde. Matériel et méthode : une étude rétrospective a été menée afin d'évaluer les réhabilitations orales en fonction du type de résection chirurgicale et de reconstruction. Les critères secondaires d'évaluation étaient : le type de réhabilitation, le taux de succès implantaire, le taux de succès de réhabilitation, les délais postradiothérapie et les causes de non-réhabilitations. Résultats : l'étude a inclus 126 patients ayant eu une résection mandibulaire. 27,9% ont bénéficié d'une réhabilitation orale implantaire ou par prothèse amovible simple. Toutes les réhabilitations implantaires ont été fonctionnelles. Les principales causes de non-réhabilitations étaient les décès ou récidives liées à l'évolution de la maladie, les difficultés anatomiques post-opératoires et le coût d'une réhabilitation orale. Conclusion : les réhabilitations orales après chirurgie de résection mandibulaire apparaissent quantitativement insuffisantes. La nécessité d'une unité de réhabilitation comprenant chirurgien maxillo-facial, chirurgien oral et dentiste est primordiale. Celle-ci doit être envisagée dès que possible. Le coût de la réhabilitation ne devrait pas être un facteur limitant.
