Pour certains, des systèmes de Communication Alternative et Améliorée (CAA) peuvent être mis en place dès le plus jeune âge, notamment par le biais de l'orthophoniste et en collaboration avec la famille. Notre recherche s'intéresse au point de vue des parents dans ce processus d'appropriation. L'objectif est de recueillir leurs perceptions, leurs expériences et de dégager la place de l'orthophoniste dans cet accompagnement. Pour cela, nous avons interrogé six parents d'enfant autiste utilisant un outil de CAA et deux orthophonistes associés, par le biais d'entretiens semi-directifs individuels. Après retranscription, nous avons procédé à une analyse qualitative transversale qui a permis de développer trois thèmes principaux : les représentations parentales du handicap et du métier d'orthophoniste, le partenariat parent/orthophoniste dans l'appropriation de la CAA au quotidien et la difficulté à généraliser son utilisation à toutes les sphères de la vie de l'enfant. Les résultats montrent que la mise en place d'une CAA s'apparente à un réel défi pour les familles et les orthophonistes. Malgré l'impact positif de la CAA sur l'enfant et son entourage, des difficultés externes et internes persistent. Le manque de connaissances, de soutien et de collaboration entre les partenaires de communication et les professionnels, notamment en milieu scolaire, représente un frein majeur au développement d'une CAA fonctionnelle et généralisée.

En effet, lorsque l'enfant doit disposer d'un outil de Communication alternative ou augmentée (CAA), ils sont les plus aptes à leur apprendre à s'en servir à la maison. Toutefois, les orthophonistes ne sont pas encore très à l'aise avec l'accompagnement parental et ne disposent pas toujours des outils adaptés pour aider les parents à mettre en place cette CAA dans leur quotidien. Pour répondre à ce besoin, nous avons créé un questionnaire et un livret. Les parents peuvent puiser dans ce support des conseils pour les aider au quotidien. A la suite de cette démarche, nous avons créé un questionnaire à destination de cinq orthophonistes. Il nous permet de vérifier la pertinence de notre outil par rapport aux besoins de ces professionnels du langage et surtout de ceux des parents. Les données récoltées ont mis en évidence un manque d'aisance dans l'accompagnement parental, ce qui motiverait les orthophonistes à disposer d'un outil pour aider les parents et faire le relais entre leur lieu de travail avec l'enfant et la maison. Les données concernant le fond et la forme de notre outil ont donné des résultats encourageants, bien que le support ait à profiter de quelques modifications. Enfin, la majorité des réponses se sont accordées pour dire que les parents s'accommoderaient bien de ce support pour les aider à la maison. Cet outil pourrait donc porter ses fruits auprès des parents de communicateurs émergents avec TSA mais pourrait également être élargi à une plus grande population nécessitant un outil de CAA.

Nous supposions que le Y'a CAA permettrait le recensement des informations indispensables au choix et à l'installation d'une CAA. Aussi, nous présagions qu'il serait pratique et facile d'utilisation pour les orthophonistes, quel que soit leur mode d'exercice. L'analyse des données a montré la pertinence des items du Y'a CAA, permettant de recenser des critères inhérents à l'enfant et à son environnement. Toutefois, certains items étaient plus ou moins accessibles et faciles à renseigner selon la situation institutionnelle et familiale de l'enfant. Aussi, certains critères spécifiques, rapportés dans la littérature, se sont révélés absents de l'inventaire. L'analyse de la concordance des réponses des partenaires sociaux, a prouvé l'intérêt d'une co-élaboration de l'inventaire, pour obtenir une vision globale de l'enfant. Par ailleurs, nous avons soulevé l'importance d'informer le grand public sur la CAA et de réfléchir sur la formation des professionnels à la modélisation du dispositif. L'intérêt d'intégrer des essais techniques au Y'a CAA, a été souligné. Cet inventaire pourrait faire l'objet d'une autre étude, permettant de le parfaire puis de le valider sur le plan statistique.

On notait un apport de connaissances présent mais modéré, limité dans ses applications pratiques. Les participants mettaient en avant, bien qu'inattendu le bénéfice important perçu dans la rencontre avec d'autres parents.

Brown applies this theory of transformational processes to a variety of topics, including the analyst’s receptivity, countertransference as transformation, the analytic setting, the paintings of J.M.W. Turner, "autistic transformations" and other clinical situations in the analysis of children and adults. Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians.
