These hospital acquired infections can be costly and lethal but most importantly, preventable. This research project's purpose is to analyze several studies demonstrating how central line-associated infections are preventable. The methods used for this project was a collaborative study examining the interventions that are incorporated in the On The CUSP: Stop BSI program through various research articles and how these interventions decreased central line-associated bloodstream infections in the Intensive Care Unit. This comprehensive unit-based safety program was developed with the goal of preventing bloodstream infections and provides a framework to educate health care professionals that are in direct care of patients with central lines. Physicians, physician assistants, registered nurses, radiology technicians, and other bedside care providers follow a collaborative five step process. The five step process as designed by On the CUSP: Stop BSI are to educate regarding the safety techniques, implement teamwork and communication tools, identify defects, address the defects, and collaborate with a unit leader for guidance. The infection rates are monitored before, during, and up to twelve months after the interventions were initiated. When these improvement efforts were evaluated, the data showed a significant decrease in central line-associated bloodstream infections. The conclusion is that implementing the On the CUSP: Stop BSI program produced significant and sustaining data that central line-associated bloodstream infections decreased in diverse ICU settings through multiple cohort studies that utilized these evidence-based prevention bundles over a year to two year time frame.

In Intensive Care Units (ICU), Central Venous Catheters (CVCs) are more common for rapid fluid, Total Parenteral Nutrition (TPN) and multiple drug administrations. As per new health care reform laws, hospital acquired infections and conditions (HAI) are no longer paid by Medicare (Agency for Health Care Research and Quality, 2011). To overcome this situation health care setting aims to provide cost effective, customer satisfied and safe care to minimize the HAIs. Implementation of bundle care is one of the important steps to gain this outcome. Healthcare professionals are responsible for maintaining central line free from infection. Based on the evidences from the 15 selected articles author could prove that bundle care is very helpful to prevent CLABSI in health care facility. At a minimal cost, each steps in central line bundle care and maintenance log was created as a checklist as per the Centers for Disease Control and Prevention (CDC) guidelines. Through this program it is expected to have zero CLABSI rate in the unit and decrease in secondary complications. The purpose of this program would be positively impacted by the dissemination of implementation strategies and evidences. It is proved that by providing education and reinforcing medical professionals about care and maintenance of central line has drastically reduced the rates of CLABSI occurrence, related complications, cost of health care and thereby increasing the satisfaction rates of the patients.

Adverse consequences to patient health resulting from these infections can negatively impact patient outcomes, prolong patient length of stay, and increase the overall cost of care. The author implemented an evidence-based practice protocol using a team approach in a 12 bed PICU using a care bundle to prevent central line infections. The purpose of this change project was to educate PICU staff caring for patients with central lines about utilization of bundle elements to reduce risk of CRBSIs. The goal was to implement each step of the bundle when caring for every patient with a CVL during the eight week change project. The author measured staff compliance to bundle elements including hand hygiene, maximal barrier precautions, chlorhexidine skin antisepsis, and daily review of line necessity. Weekly audits demonstrated 100% provider adherence to care bundle interventions over the implementation phase. In accordance with evidence from similar studies, these findings suggest compliance with bundle interventions can lead to significant reduction in the rate of catheter-related bloodstream infections. Central intravenous lines on patients admitted to this critical care setting provide intravenous access for medications, total parenteral nutrition, and for hemodynamic monitoring. Although CVCs aid hospital staff in providing efficient care for these critically ill patients, these tools also place the patients at risk for central line-associated bloodstream infections (CLABSI). CLABSI, a nosocomial infection, is a major source of morbidity and mortality. It is also associated with increased cost of care for healthcare facilities across the country, averaging $45,000 per single infected patient. (Hakko, Guvenc, Karaman, Erdem, and Cakmakci, 2015) This project proposal calls for an implementation of a six-month pilot study to be conducted in the facility's medical intensive care unit to adopt a new updated central line management bundle. The new bundle will incorporate the utilization of disinfectant port protectors or alcohol-impregnated port protectors as a measure to prevent bloodstream infections. The utilization of disinfectant port protectors, which are saturated with alcohol, has been proven by several published research studies in preventing central line-associated bloodstream infections. In addition to the pilot program, facility-wide education will be conducted to educate nurses and other allied healthcare personnel about central line-associated bloodstream, central line management bundle, and prevention strategies. Educating healthcare workers about CLABSI and proper maintenance of central intravenous lines is primary prevention strategy, which can aid decrease the facility's CLABSI rate, and also decrease the facility's unnecessary expenditures of treating these infections. It is also important to disseminate these possible results, so that nurses know the value of compliance and strict adherence to protocols of central line management care in preventing CLABSI.

Book excerpt: More than fifty percent of all patients admitted to the intensive care unit will acquire a central venous catheter (CVC) in order to provide a more efficient treatment plan. While very beneficial for treatment, they often come with significant costs that include high bloodstream infection rates, increased mortality rates, increased lengths of stay, and increased cost of additional resources. With quality of care initiatives leading the industry, decreasing catheter-associated bloodstream infections (CA-BSI) has become one focus for improvement. Research shows improvement in adult populations; however, few research reports address implementation of the recommendations among pediatric populations. The National Association of Children's Hospitals and Related Institutions (NACHRI) group called upon pediatric institutions to develop a standard of care for central line maintenance to reduce infection rates. This secondary analysis investigated the effects of a central line bundle on reducing CA-BSI in a pediatric intensive care unit (PICU). The conceptual framework was based on The Improvement Model developed by Langley et al. (1996) to test and evaluate new changes. Based on data collected from one PICU's performance improvement practices and for the NACHRI collaborative, this study evaluated rates of infection before and after the collaborative as well as nursing compliance with the bundle. Data was not statistically significant during the initiation year of implementation, however, the first complete year after initiation showed statistically significant results. Nursing compliance with the bundle was variable across the years and with each bundle element. Further research is recommended as improving CA-BSI rates impact pediatric patient outcomes.

Book excerpt: " Central line-associated bloodstream infection (CLABSI) is among the groups of hospital-acquired infections (HAIs). CLABSI carries an increased risk of morbidity, mortality, increased hospital length of stay, and is very expensive to treat. The literature reviewed recommended the Centers for Disease Control and Prevention (CDC) guidelines and checklist bundle system as an effective method in CLABSI prevention; however, this required strict adherence and compliance to be effective. The project manager (PM) implemented the quality improvement project in a 14-bed surgical intensive care unit (SICU) for ten weeks. A pre-test questionnaire based on CDC guidelines tested the nurses' knowledge. A CDC checklist was provided to the nurses to monitor compliance with the bundle and followed by an educational intervention presentation. A post-test questionnaire score was used to assess knowledge gained. Compliance with the insertion and maintenance bundle was collected and tabulated. CLABSI rate for SICU for the same period was 1 case for a total of 197-line days, equal to an infection rate of 5.08 higher than the national benchmark of 1.53, compared to the other units was 5 cases for a total of 1,497 line days resulting in an infection rate of 3.34 for level 1a hospital. Results indicated a post-test knowledge of 91.6%, exceeding the facility benchmark of 80% for competency education. Compliance with all the bundle components was 100%. However, SICU had one CLABSI case in January and none in February. These findings indicate that CLABSI prevention using the bundle system is effective in reducing and preventing hospital-acquired infections. Gaining knowledge on all the bundle components fostered compliance and increased quality patient care outcomes. Nurses played an integral part in CLABSI prevention.
