Ultimately, the goal of these guidelines is to help reduce levels of air pollutants in order to decrease the enormous health burden resulting from the exposure to air pollution worldwide.

The challenges described include air pollution, climate change, extreme weather, and related health impacts that range from heat stress, vector-borne diseases, food and water insecurity and chronic diseases to malnutrition and mental well-being. The influence of humans on climate change has been established through extensive published evidence and reports. However, the connections between climate change, the health of the planet and the impact on human health have not received the same level of attention. Therefore, the global focus on the public health impacts of climate change is a relatively recent area of interest. This focus is timely since scientists have concluded that changes in climate have led to new weather extremes such as floods, storms, heat waves, droughts and fires, in turn leading to more than 600,000 deaths and the displacement of nearly 4 billion people in the last 20 years. Previous work on the health impacts of climate change was limited mostly to epidemiologic approaches and outcomes and focused less on multidisciplinary, multi-faceted collaborations between physical scientists, public health researchers and policy makers. Further, there was little attention paid to faith-based and ethical approaches to the problem. The solutions and actions we explore in this book engage diverse sectors of civil society, faith leadership, and political leadership, all oriented by ethics, advocacy, and policy with a special focus on poor and vulnerable populations. The book highlights areas we think will resonate broadly with the public, faith leaders, researchers and students across disciplines including the humanities, and policy makers.

Book excerpt: A study on air quality monitoring carried out in the 1980s indicated that around 40% of the high concentrations of air pollutants adversely affecting residents and the natural environment were contributed by coal-fired home stoves and boiler houses using solid fuels. These heat sources discharging particulate matters at low heights, basically lacking any pollution control devices, are called the ''low emission sources.'' The American-Polish Program on Clean Fossil Fuels and Energy Efficiency in Krakow, which began in 1992, was preceded by a detailed inventory of low emissions sources completed in 1991. In the American-Polish program it has been accepted that all possible technical means would be used to reduce pollution, mainly those which enable utilization of reserves present in the existing networks of: district heating; gas distribution; and power distribution. The analysis indicated that the following activities can bring about realistic progress in reducing air pollution: elimination of the existing solid fuel boiler houses by connecting the buildings they serve to the municipal district heating network; elimination of solid fuel boiler houses by converting them to natural gas; elimination of solid fuel-fired home stoves by replacing them with electric heating; modernization of those solid fuel boiler houses which cannot by eliminated because of their function; and reducing the adverse environmental effects of home stove use throughout the city by changes in the combustion process, eliminating coal and replacing it with natural gas or electricity, and/or looking for combustion devices that can burn coal without producing air pollutants.

Clean air is a basic condition for health. Air pollution aggravates respiratory problems, leading to increased sickness absenteeism, increased use of health care services and even premature mortality. Air pollution is under intensive discussion in the United States and Europe. In The Netherlands, a wide range of policy instruments have been formulated which have reduced air pollution. For example; since 1975, sulphur dioxide and lead emissions have been reduced. However, emission reduction figures for many other substances are more modest. Many air pollution problems persist because progress in countering these problems is nullified by growth in the economy and traffic. Another important target is the prevention of climate change. The international community is agreed that the increasing concentration of greenhouse gases in the atmosphere has led to a gradual increase in the earth's temperature. In terms of the environmental consequences and social implications, the greenhouse problem surpasses all other air quality problems. Across Europe, strategies are being developed to reduce acidification and photochemical air pollution. An air emission ceiling for each country in the European Union is being agreed. In the area of climate change, there is good co-operation between the United States, The Netherlands and other EU Members States in the ongoing global negotiations. This is the start of a new movement. In the last century economies and societies developed through increasing human productivity. In the next century they must develop through increasing the productivity of fuel and natural resources.
