This book was released on 2014-04-01 with total page 385 pages. Available in PDF, EPUB and Kindle. Book excerpt: This User’s Guide is intended to support the design, implementation, analysis, interpretation, and quality evaluation of registries created to increase understanding of patient outcomes. For the purposes of this guide, a patient registry is an organized system that uses observational study methods to collect uniform data (clinical and other) to evaluate specified outcomes for a population defined by a particular disease, condition, or exposure, and that serves one or more predetermined scientific, clinical, or policy purposes. A registry database is a file (or files) derived from the registry. Although registries can serve many purposes, this guide focuses on registries created for one or more of the following purposes: to describe the natural history of disease, to determine clinical effectiveness or cost-effectiveness of health care products and services, to measure or monitor safety and harm, and/or to measure quality of care. Registries are classified according to how their populations are defined. For example, product registries include patients who have been exposed to biopharmaceutical products or medical devices. Health services registries consist of patients who have had a common procedure, clinical encounter, or hospitalization. Disease or condition registries are defined by patients having the same diagnosis, such as cystic fibrosis or heart failure. The User’s Guide was created by researchers affiliated with AHRQ’s Effective Health Care Program, particularly those who participated in AHRQ’s DEcIDE (Developing Evidence to Inform Decisions About Effectiveness) program. Chapters were subject to multiple internal and external independent reviews.

Book excerpt: Most industries have plunged into data automation, but health care organizations have lagged in moving patients' medical records from paper to computers. In its first edition, this book presented a blueprint for introducing the computer-based patient record (CPR). The revised edition adds new information to the original book. One section describes recent developments, including the creation of a computer-based patient record institute. An international chapter highlights what is new in this still-emerging technology. An expert committee explores the potential of machine-readable CPRs to improve diagnostic and care decisions, provide a database for policymaking, and much more, addressing these key questions: Who uses patient records? What technology is available and what further research is necessary to meet users' needs? What should government, medical organizations, and others do to make the transition to CPRs? The volume also explores such issues as privacy and confidentiality, costs, the need for training, legal barriers to CPRs, and other key topics.

Available in PDF, EPUB and Kindle. Book excerpt: Patient-reported outcomes (PROs) are measures of how patients feel or what they are able to do in the context of their health status; PROs are reports, usually on questionnaires, about a patient's health conditions, health behaviors, or experiences with health care that individuals report directly, without modification of responses by clinicians or others; thus, they directly reflect the voice of the patient. PROs cover domains such as physical health, mental and emotional health, functioning, symptoms and symptom burden, and health behaviors. They are relevant for many activities: helping patients and their clinicians make informed decisions about health care, monitoring the progress of care, setting policies for coverage and reimbursement of health services, improving the quality of health care services, and tracking or reporting on the performance of health care delivery organizations. We address the major methodological issues related to choosing, administering, and using PROs for these purposes, particularly in clinical practice settings. We include a framework for best practices in selecting PROs, focusing on choosing appropriate methods and modes for administering PRO measures to accommodate patients with diverse linguistic, cultural, educational, and functional skills, understanding measures developed through both classic and modern test theory, and addressing complex issues relating to scoring and analyzing PRO data.

Book excerpt: Complementing existing literature on measuring health outcomes that is largely conceptual, this book focuses on simple, practical advice for measuring outcomes in a variety of settings. Written in an engaging conversational tone, readers will learn why measuring health outcomes is necessary in clinical practice and how these measures may vary between people and across care structures. Covering how to identify measurements as well as collect and analyze the data, the chapters lead readers through a series of logical steps to scaling up a measurement program. The workbook style allows readers to record their own notes and thoughts throughout the book, while the list of action steps at the end of chapters are tangible starting points for developing a measurement program of their own. Explores how to measure and think about outcomes in a way that sees the whole person, not just the medical or behavioral condition they have.

Book excerpt: This report presents the findings from a study designed: (1) to determine the current availability and utility of student outcome statistics in the nation; and (2) to examine the feasibility and desirability of nationwide institutional reporting of student outcome data. Following an introductory section on the purposes of the report and the approaches taken, section II addresses the desirability of student outcome data, focusing on the utility of the information and the necessary properties of outcome statistics. Section III explores sources of available data on postsecondary student outcomes and describes current capabilities for reporting institutional-level student outcomes, examining both institutional and state capabilities as well as other sources. Section IV discusses problems with institutional-level student outcome reporting, examining the quality of data that are currently available and the current and prospective uses of these data. Section V identifies those aspects of student outcomes statistics that require definitional specificity and describes data collection procedures and methodologies that could minimize the various problems of student-outcome reporting and produce useful and meaningful institution-level data. Finally, section VI summarizes the report and formulates conclusions concerning the desirability and feasibility of different approaches for obtaining student outcome information. Suggestions for specific actions that must be initiated if meaningful institution-level student outcome data are to be a reality for all postsecondary institutions in the nation are provided. Appendices include technical notes and summaries of definition working group meetings. (14 references) (GLR)

Nursing Outcomes Classification (NOC), 7th Edition standardizes the terminology and criteria needed to measure and evaluate outcomes in all care settings and with all patient populations. A total of 612 research-based nursing outcome labels — including 82 that are NEW to this edition — provide clinically useful language to help you deliver treatment and document outcomes. Specific indicators are included to make it easier to evaluate and rate the patient in relation to outcome achievement. Written by an expert team of authors led by Sue Moorhead, this book is also ideal for healthcare administrators seeking to improve billing, recordkeeping, and cost containment. 612 research-based nursing outcome labels provide standardized terminology for individual, family, or community outcomes. Overview of the use of NOC within the nursing process introduces the importance of measuring outcomes of nursing care, and describes linkages with other classifications. Outcomes structured with a label name include code, definition, set of indicators with codes, five-point Likert measurement scales, publication facts lines, and selected references. Core outcomes are provided for an expanded list of nursing specialties. Linkages between NOC knowledge-focused outcomes and NOC behavioral outcomes focused on the concept or condition are examined. NEW! 82 new outcomes are added to the Classification, allowing you to better define patient outcomes that are responsive to nursing care. NEW! 402 existing outcomes are reviewed or revised based on research-based outcomes. NEW! A new section focused on resources supports research, implementation, and educational strategies. NEW! Revised taxonomic structure includes two new classes and expanded family and community outcomes.
