In such an endocytic process, cell surface receptors specifically bind protein ligands and localize them to specialized invaginations of the plasma membrane. These regions are coated pits, so named because they are lined on the cytoplasmic face with an ordered array of the protein, clathrin. It is this 'coat' which provides their characteristic electron microscopic image. Subsequently, these regions pinch off to form coated vesicles which rapidly lose their 'coat' and then fuse with other organelles or the plasma membrane. The hallmarks of ,this process are the specific receptors, coated pits, coated vesicles and an ordered sequence of transit events leading to delivery to selected locations. Receptor recognition, specific disposition of the endocytosed ligand and the existence of recep tor-ligand complexes at highest density in coated pits define the process as selective and concentrative. This topic has received ever increasing attention during the past few years. The evolving mechanisms are especially exciting because they come at a time when the conventional views based on thermodynamic arguments suggest that proteins should not be able to cross into the cell. Receptor-mediated endocytosis, however, reconciles the view that biological membranes should be impervious to macromolecules with the evidence that certain mac romolecules do gain entrance into the cell. During the last few years this field has been stimulated by studies on the uptake and processing of low density lipoproteins (LDL) by cells.

RME is based on exploitation of such receptors to achieve intracellular delivery. While targeted delivery can have manifold applications, in this book we focus on two major and challenging therapeutic areas; i) Cancer and ii) Infectious Diseases. Targeted Intracellular Drug Delivery by Receptor Medicated Endocytosis discusses the major receptors that are useful for targeted delivery for these afflictions. A major section of this book is dedicated to details regarding their occurrence and location, the recognition domain of the receptor, structure activity relationship of substrate /ligand for selective binding, ligands explored, antagonists for ligand binding and relevance of these aspects for therapy of cancer and infectious diseases. These facets are elucidated with the help of specific examples from academic research and also emphasize commercial products, wherever relevant. In vitro cellular models relied on for assessing receptor mediated cellular targeting and in vivo models depicting clinical efficacy are focused on in a separate section. Finally, we briefly discuss the regulatory and toxicity issues that may be associated specifically with the RME approach of intracellular drug delivery.

A major role of the plasma membrane is as a permeability barrier, keeping the inside of the cell inside and the outside, outside. Mechanisms must then exist to allow movement of material between the cell and its environment. One mechanism for export from the cell is by exocytosis, a process in which the membranes of secretory vesicles fuse with the plasma membrane releasing the contents of the vesicle into the extracellular medium. The process has been studied in particular depth for the release of neurotransmitters at the synapse. Import into the cell is possible by the process of receptor-mediated endocytosis in which selected plasma membrane proteins are internalizes; when these proteins are receptors for macromolecules, the result is uptake of the macromolecule. Transferring, the low-density lipoprotein, and asialoglycoproteins are all taken up into cells in this way. Phagocytosis, the ingestion of cells and cell fragments by neutrophils and macrophages, also involves receptors - on the phagocytic membrane - of which the best studied are those for the Fc domain of IgG, for the third component of complement, and for the mannose/fructose carbohydrates. Protection of a host against infection can also be achieved by damaging the integrity of the plasma membrane of the invading organism. This is the strategy evolved by the cytotoxic T lymphocytes, which produce a pore-forming toxin, perforin. Volume 4 of Biomembranes explores the structures and mechanisms involved in these biologically and medically important processes.

The unidirectional secretion of tTf by Sertoli cells was verified, in vivo, and it was shown that this protein is internalized by adluminal germ cells. It was further determined by Scatchard analysis that this internalization was mediated by high affinity transferrin binding sites on the surface of round spermatids, numbering 1453/cell and displaying a K$ sb{ rm d}$ = 0.6 $ times$ 10$ sp{-9}$ M. Northern blot analysis of RNA isolated from adluminal germ cells, namely spermatocytes, round spermatids and elongating spermatids, indicated that these cells expressed Tf receptor mRNA and ferritin mRNA in levels inversely related to their stage of maturation. Finally it was determined that following binding and internalization in round spermatids, Tf became associated with the endosomal compartment and was recycled back to the cell surface. Diferric tTf binds to Tf receptor on the surface of adluminal germ cells, is internalized by receptor-mediated endocytosis and the apotTf-Tf receptor complex is recycled back to the cell surface where apotTf is released into the adlumenal fluid." --
