Book excerpt: Les réadmissions précoces aux urgences font partie des évènements indésirables potentiels dans le parcours de soins de tout patient âgé, d’autant plus que ce dernier est fragile. Le but principal de notre étude est de rechercher dans les dossiers médicaux d’une population de patients de 75 ans ou plus, des différences significatives en terme de facteurs corrélés à une éventuelle réadmission précoce. L’objectif secondaire est d’évaluer, sur le plan gériatrique, la qualité du dossier médical utilisé aux urgences. Nous avons réalisé une étude rétrospective, comparative, monocentrique, sur une période de trois mois (octobre à décembre 2008) au sein du service d’accueil des urgences (SAU) du CHU de Nice. Nous avons comparé les données présentes dans les dossiers médicaux de 408 patients répartis en 2 cohortes: l’une de 204 patients réadmis aux urgences à moins d’un mois de leur première admission, l’autre de 204 patient non réadmis. Nous avons retrouvé 3 facteurs significativement liés à la réadmission précoce : l’identification d’un médecin traitant (p=0,001), l’admission pour motif social (p= 0,04) et la notion d’appel ou de courrier au médecin traitant (p=0,0001) dans les suites de la consultation aux urgences. Nous avons également mis en évidence le fait que les dossiers médicaux du SAU du CHU de Nice n’étaient pas complètement adaptés à la prise en charge des patients âgés. Nos résultats confirment ce que beaucoup d’études mettent en évidence : le phénomène de réadmission chez les patients âgés est multifactoriel et complexe et le médecin traitant y joue un rôle essentiel. Le dossier patient utilisé au SAU du CHU de Nice pourrait être complété ou modifié afin d’obtenir un document plus « gériatrique » dont le but serait de favoriser une meilleure prise en charge multidisciplinaire du patient et ainsi limiter autant que possible la survenue d’évènements indésirables, notamment de réadmissions précoces, dans le parcours de soins de la personne âgée.

They frighten the patients who experience them; they frighten those who witness them; they also frighten many physicians who have to deal with them. Most individuals with seizures present to family physicians or to emergency room physicians. However, despite the fact that seizures are among the most common neurological conditions, most general practitioners, family practice specialists, and intemists do not see large numbers of patients with seizures. Given the apoplectic appearance of generalized tonic clonic convulsions, it is not difficult to understand why they arouse such emotional responses in those that experience them, those that witness them, and those whose care is sought for them. Seizures are symptoms of something wrong with the brain. Many different kinds of perturbations in brain anatomy, chemistry, or physiology can produce seizures. For many individuals, seizures occur in the context of an acute illness and will not recur once that illness is treated. These individuals do not have epilepsy. They have transient disturbances in brain function attributable to systemic medical conditions. It is important to recognize these issues, because, first, the seizure may be the initial, or even only, manifestation of the underlying medical problem and this needs to be recognized.
