Book excerpt: Many health problems that healthcare practitioners encounter derive from or are complicated by alcohol use disorders (AUD). Four approved medications that treat AUD make treatment in primary care a viable adjunct or alternative to specialty care, with many potential advantages. Screening for and providing brief interventions to treat AUD in general medical settings promote healthy life choices and increase the likelihood of recovery, especially for patients who have not yet progressed to chronic alcohol dependence, those with comorbid medical disorders being treated in these settings, and those who otherwise would not seek or receive treatment for their AUD. This report provides a resource to assist the health-care provider in this effort. Illus.

The underlying objective is to expand access to information about the effective use of these medications, not only in specialty substance abuse treatment programs but also in physicians' offices and other general medical care settings. The TIP includes discussions of acamprosate, disulfiram, oral naltrexone, and extended-release injectable naltrexone. The U.S. Food and Drug Administration has approved these medications for treating alcohol use disorders. The TIP explains each medication's history, the reasons for its use, how to use it, who should use it, and other clinical information about the medication. Most of the fundamental research that forms the evidence basis for this TIP is not provided in the TIP itself.

Book excerpt: The United States is in the midst of the largest military demobilization in its history. This is leading to an increase in the demand for mental health clinicians who can provide services to hundreds of thousands of military veterans and members of the military. Nearly two million Americans have been deployed to the wars in the Middle East, and thousands of them have been deeply affected, either psychologically, physically, or both. Projections suggest that 300,000 are returning with symptoms of PTSD or major Depression; 320,000 have been exposed to probable Traumatic Brain Injuries; and hundreds of thousands are dealing with psychological effects of physical injuries. Other veterans and members of the military without injuries will seek treatment to help them with the psychological impact of serving in the military, being deployed, or transitioning and reintegrating back into the civilian world. As an example, hundreds of thousands of service members are also leaving the armed forces earlier than they anticipated and will need to quickly adjust to life as civilians after assuming that they would have many more years in the military. Many will be leaving the military because of demobilizations and downsizing due to budget cuts. Current proposed cuts will shrink the military force to the same size it was in 1940. The Pew Center reports that 44% of veterans from the current wars are describing their readjustment to civilian life as "difficult," and many of them are and will be turning to civilian mental health and primary care clinicians for assistance. The Handbook of Psychosocial Interventions for Veterans and Service Members is a "one stop" handbook for non-military clinicians working with service members, veterans, and their families. It brings together experts from the Department of Defense, the Department of Veterans Affairs, veteran service organizations, and academia to create the first comprehensive guidebook for civilian clinicians. In addition to covering psychiatric disorders such as depression, anxiety, and PTSD, this book also offers information about psychosocial topics that impact military personnel and their loved ones and can become part of treatment (e.g., employment or education options, financial matters, and parenting concerns), providing the most recent and cutting-edge research on the topics. Chapters are concise and practical, delivering the key information necessary to orient clinicians to the special needs of veterans and their families. The Handbook of Psychosocial Interventions for Veterans and Service Members is an essential resource for private practice mental health clinicians and primary care physicians, as well as a useful adjunct for VA and DOD psychologists and staff.

Book excerpt: Using the latest clinical research and diagnoses, Principles and Practice of Psychiatric Nursing, 10th Edition provides a holistic, biopsychosocial approach to psychiatric nursing care. It follows the popular Stuart stress-adaptation framework and includes comprehensive coverage to simplify important nursing and medical concepts, promote quality and safety in care, and address psychobiology and psychopharmacology topics integral to today’s psychiatry. New to this edition is a chapter on psychiatric care of military personnel, plus the latest on health care reform, prescription abuse, and obesity issues. Written by psychiatric nursing expert Gail W. Stuart, this market-leading text makes it easy to apply classroom theory to clinical practice. An easy-to-follow writing style makes it easy to understand both simple and complex topics. A well-rounded, collaborative approach provides coverage of all major psychiatric disorders from nursing and medical perspectives. The Stuart Stress Adaptation Model of health and wellness provides a consistent nursing-oriented framework, with clear explanations of biological, psychological, sociocultural, environmental, and legal-ethical components. An evidence-based practice approach bridges the gap between clinical research and everyday practice. Learning from a Clinical Case boxes begin disorders chapters with thought-provoking questions and end chapters with answers and feedback. Summarizing the Evidence boxes in the disorders chapters examine the research and findings that support psychiatric nursing care. A Patient Speaks and A Family Speaks boxes present short vignettes with the patient’s and family’s perspectives of the caregiving process. Competent Caring: A Clinical Exemplar of a Psychiatric Nurse boxes feature the experiences and personal insights of practicing psychiatric nurses. Medical and Nursing Diagnoses boxes and Detailed Diagnoses tables emphasize the interdisciplinary approach to patient care by presenting NANDA diagnoses relevant to specific disorders and describing the essential features of the related DSM-IV-TR diagnoses. Nursing Treatment Plan Summary tables present care plans including patient goals with nursing interventions and rationales. Patient Education Plan and Family Education Plan tables include key information that you need to share with the patient and his or her family to facilitate shorter hospital stays and more outpatient care. Therapeutic Dialogue boxes offer examples of nurse-patient interactions. Clinical examples include selected nursing diagnoses. Focus Points provide a comprehensive, point-by-point review of the important information in each chapter.

Its focus is on the practical application of information covered in the two previous volumes of the series, Comprehensive Addictive Behaviors and Disorders. Readers will find information on treatments beyond commonly used methods, including Internet-based and faith-based therapies, and criminal justice interventions. The volume features extensive coverage of pharmacotherapies for each of the major drugs of abuse—including disulfiram, buprenorphine, naltrexone, and others—as well as for behavioral addictions. In considering public policy, the book examines legislative efforts, price controls, and limits on advertising, as well as World Health Organization (WHO) efforts. Interventions for Addiction is one of three volumes comprising the 2,500-page series, Comprehensive Addictive Behaviors and Disorders. This series provides the most complete collection of current knowledge on addictive behaviors and disorders to date. In short, it is the definitive reference work on addictions. Includes descriptions of both psychosocial and pharmacological treatments. Addresses health services research on attempts to increase the use of evidence-based treatments in routine clinical practice. Covers attempts to slow the progress of addictions through prevention programs and changes in public policy.
