Méthodes : Nous avons réalisé une revue narrative de la littérature internationale de janvier 1990 à décembre 2018 incluant les articles traitant de la formation à la communication chez les étudiants en médecine. Résultats :46 articles ont été inclus, majoritairement de littérature anglo-saxonne dans des spécialités médicales diverses.l'enseignement interactif est supérieur à l’absence de formation et à l’enseignement traditionnel. Le jeu de rôle avec pairs et les patients simulés sont les plus utilisés sans supériorité démontrée de l’un ou l’autre. Les autres méthodes sont les vidéos, les débats et le théâtre avec peu de données d’évaluation notamment comparatives. Les enseignants spécifiquement formés et l’usage d’une rétroaction semblent utiles. Discussion/conclusion : L’intérêt de former les étudiants en médecine à la communication est confirmé. Le jeu de rôle avec pairs et les patients simulés sont les références. D’autres méthodes existent mais méritent plus d’études pour constituer des alternatives. Plus d’études comparatives inter-méthodes ou sur les bénéfices à long terme de cette formation sont nécessaires. Les patients simulés sont applicables à Créteil.

Book excerpt: Suite à l'arrêté du 8 avril 2013 relatif au régime des études médicales, la formation initiale des étudiants en médecine en France doit maintenant être complétée par un apprentissage par compétence. Alors que de nombreux pays ont déjà intégré l'acquisition de compétences dans leurs objectifs de formation, en France existe une certaine crainte du changement de modèle pédagogique. Ainsi, nous avons cherché à reconnaître les représentations du concept de compétence chez les étudiants et enseignants, mais aussi celles de 'collaboration-coopération', de 'communication' et d''érudition' (référentiel CanMEDS 2015). Nous avons réalisé un travail qualitatif constitué d'une recherche bibliographique et d'entretiens collectifs (focus group). Des étudiants volontaires des promotions de la 2ème à la 6ème année d'études médicales de la Faculté de Médecine Lyon Est, ainsi que leurs enseignants ont participé aux entretiens. Un groupe par promotion et par compétence était prévu. Les entretiens furent retranscrits intégralement et analysés thématiquement. Treize entretiens ont été réalisés avec 40 étudiants et 15 enseignants entre le 24 mars et le 26 juillet 2016 à la Faculté de Médecine Lyon Est. Pour les participants, la compétence semblait nécessaire à l'exercice de la profession et fait le lien entre la théorie et la pratique pour répondre aux attentes du patient et de la société. La 'collaboration-coopération' mettait en jeu une équipe dans toute sa complexité. Elle était très en lien avec la 'communication' qui passe par un dialogue impliquant le temps, l'empathie et la bienveillance. Enfin, l'érudition représentait un savoir évoluant, basé sur la médecine factuelle, en lien avec la pratique. La mise en oeuvre de toutes ces compétences permettait un gain d'efficacité dans le soin. Les définitions et les attentes des étudiants et enseignants se rapprochaient souvent de celles incluses dans les définitions canadiennes et françaises. Ils semblent avoir validé et accepté le concept de compétence tel que défini par les experts. Cela permet d'envisager une possible intégration d'un tel paradigme d'apprentissage dans les études médicales françaises. Il apparaît nécessaire de développer des travaux de recherche sur les modes d'enseignement et d'évaluation des compétences afin de prolonger cette évolution de la formation médicale aux étudiants et enseignants du 3ème cycle.

Résultats et discussion : la communication en santé s'enseigne et a prouvé son efficacité auprès des étudiants en santé. Les enseignements efficaces mêlent les techniques d'enseignement théoriques, pratiques (entretiens simulés) associés à une supervision individuelle utilisant la vidéo, ou réalisée par un médecin sénior entrainé. Les grilles d'évaluations sont trop nombreuses et évaluent les mêmes compétences, témoin d'un manque d'homogénéité des enseignements. Les compétences communicationnelles les plus difficiles à acquérir sont la capacité d'écoute du patient et l'empathie. Conclusion : l'enseignement de la communication aux étudiants en médecine est crucial. Il doit être de qualité, proposant plusieurs dispositifs pédagogiques (théoriques et pratiques), bien encadré (supervision) par des professionnels formés. Il doit être reproductible et évaluable facilement par des grilles validées selon les compétences que l'on souhaite évaluer.

Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. The overview chapter is followed by eight empirical findings, each dealing with phenomena that are significant in Japanese pragmatics. They target selected features of Japanese pragmatics and investigate the learners' use of them as an indicator of their pragmatic competence. The target pragmatic features are wide-ranging, among them honorifics, speech style, sentence final particles, speech acts of various types, and indirect expressions. Each study explicitly prompts the connection between pragmalinguistics (linguistic forms available to perform language functions) and sociopragmatics (norms that determine appropriate use of the forms) in Japanese. By documenting the understanding and use of them among learners of Japanese spanning multiple levels and time durations, this book offers insight about the nature and development of pragmatic competence, as well as implications for the learning and teaching of Japanese pragmatics.
