Book excerpt: Contexte : en France et dans le monde, la prévalence de la maladie rénale chronique (MRC) augmente, notamment chez les personnes âgées qui ne bénéficient d'aucune recommandation spécifique de prise en charge. Populations et méthodes : notre étude épidémiologique, descriptive a été réalisée en 2 parties : l'une quantitative à partir de laquelle 197 dossiers médicaux de patients âgés de 75 ans ou plus présentant une MRC modérée à sévère ont été inclus. L'autre partie, qualitative par auto-questionnaires administrés à 18 médecins généralistes des Alpes-Maritimes. Les objectifs de notre étude étaient : de caractériser sur le plan médical la personne âgée atteinte d'une MRC en soins primaires ; de décrire et d'évaluer sa prise en charge et enfin d'identifier les difficultés rencontrées par les médecins dans cette pratique. Résultats : nous avons identifié certaines lacunes dans la prise en charge : une sous-utilisation de l'échographie (38%) ou de la recherche de protéinurie (21%), ainsi qu'un contrôle insuffisant de la pression artérielle (43%). La majorité des médecins (83%) estiment que la prise en charge de la maladie rénale chronique de la personne âgée est difficile principalement en lien avec la polypathologie (66.7%). Alors que la majorité des médecins (78%) ont un correspondant néphrologue avec lesquels plus de la moitié (57%) estiment avoir de bonnes relations ; peu ont eu recours à son avis (11%) et 38% des patients ont débuté un suivi néphrologique en «référence tardive » aux stades 4 et 5 de la MRC dont 5% ont été pris en dialyse en urgence. Les difficultés sont rencontrées principalement lors du contrôle des prescriptions et l'ajustement thérapeutique (67%) ; dans le choix et la mise en place du traitement néphroprotecteur (55%) et dans le suivi (50%). D'autres part, 56% des médecins interrogés ne connaissent pas les recommandations sur la prise en charge de la MRC (HAS 2012) et seulement un tiers (27.8%) ont assisté à une FMC sur ce thème. 43% estiment que les relations peuvent être améliorées notamment sur le plan de la communication ainsi que sur le retour d'information. Conclusions : pour améliorer cette prise en charge il semble donc nécessaire de développer des formations auprès des médecins généralistes, de diffuser des recommandations adaptées à la pratique quotidienne concernant la personne âgée polypathologique, d'améliorer la communication et le partage d'information médecin généraliste - néphrologue. Le dispositif des réseaux de santé peut constituer une piste de réponse particulièrement adaptées aux besoins des patients atteints de MRC ainsi que des médecins.

Since the author and his colleagues performed the first ABO-incompatible kidney transplantation in Japan in 1989, 400 such transplantations have been performed in 41 hospitals in Japan and this practice has contributed to a number of new developments. One is a clearer and more suitable model for conceptualising the mechanism of humoral immune response which enables identification of antigens and antibodies and a therapeutic strategy against rejection. The rejection mechanisms are discussed not only from the perspective of immunology but also viewed from different angles, including anatomy, microscopic and macroscopic pathology, molecular biology and haematology. Immunosuppressive therapy is discussed, divided in four categories: extracorporeal immunomodulation with removal of humoral antibodies; drug therapy to suppress cellular immunity; splenectomy; and anticoagulation therapy. Surgical procedures for kidney transplantation and splenectomy are treated, including discussion of the best timing for the latter. Son coût humain est particulièrement élevé en termes de morbi-mortalité, d'impact sur la qualité de vie. Le coût de la prise en charge de la MRC est considérable. Toutes les études s'accordent à dire qu'il existe une réelle insuffisance de diagnostic et de prise en charge en médecine générale. Pourtant, le médecin généraliste joue un rôle déterminant dans le parcours de soins des patients ayant une MRC. Aussi, l'objectif de cette étude était d'explorer les pratiques et les difficultés des médecins généralistes dans le dépistage et la prise en charge de la maladie rénale chronique en soins primaires. Méthode et matériel: Nous avons réalisé une étude qualitative, par entretiens individuels semi directifs, auprès de quatorze médecins généralistes installés dans la ville de Nice, d'Avril 2013 à décembre 2013. Une vignette présentant un cas clinique typique de découverte d'une insuffisance rénale chronique chez un patient diabétique hypertendu servait de support à ces entretiens. Le choix des médecins interrogés a été fait par échantillonnage aléatoire. De plus, Les verbatim ont été analysés par approche herméneutique par théorisation ancrée. Le recueil et l'analyse des données, quant à eux, ont été faits de façon itérative, jusqu'à la saturation des données. Résultats: Notre étude met en évidence les principaux obstacles et les vraies difficultés rencontrés chez les médecins généralistes dans le dépistage et la prise en charge de la MRC. Nous les avons classés en quatre grands thèmes : des difficultés liées à la représentation de la MRC, estimée grave, méconnue, silencieuse ; des difficultés liées à une thérapeutique essentiellement non médicamenteuses, difficile à mettre en place en soins primaires ; des difficultés liées à l'absence de réseaux de soins et à une mauvaise collaboration avec les néphrologues ; et enfin des difficultés liées au profil des patients souvent âgés et poly pathologiques. Conclusion: Cette étude qualitative nous permet de proposer deux grandes pistes de travail pour un meilleur parcours de soins des patients ayant une MRC. La première est destinée au médecin généraliste pour mieux l'aider et l'accompagner. Et la deuxième est destinée au patient pour le sensibiliser et l'impliquer réellement dans la prise en charge de sa maladie.
