Book excerpt: Developed within the context of the expansion of the Canadian welfare state in the years following the Great Depression, the present organization of Canadian health care delivery is now in serious need of reform. This book documents the causes and effects of changes made in this century to Canada's health care policy. Particular emphasis is placed on the decades following 1940, the years in which Canada moved away from an individualistic entrepreneurial medical care system, first toward a collectivist biomedical model and then to a social model for health care.

Available in PDF, EPUB and Kindle. Book excerpt: The rise of the welfare state threatens the autonomy and survival of nonprofit voluntary agencies as providers of social services. Or does it? In this cross-national, empirical study of the workings of voluntary agencies, Ralph M. Kramer cuts through the conceptual confusion surrounding voluntarism and the boundaries between the public and private sectors. He draws on a survey of voluntary agencies helping disabled people in four welfare democracies (the United States, England, Israel, and the Netherlands) to explain the virtues and flaws of different patterns of government-voluntary relationships in coping with the growing demand for human services. Kramer concludes that many of the most cherished beliefs about the voluntary sector have little basis in fact. The most innovative agencies, for example, are not the smallest, but rather among the largest, most bureaucratized, and most professionalized. Government funding does not necessarily constrain agency autonomy. And giving voluntary agencies the primary responsibility for social services can reduce, not increase, citizen participation. This comparative analysis of the distinctive competence, vulnerability, and potential of the voluntary agency should replace some of the myths that guide public policy and the day-to-day activities of social service agencies. With contributions from scholars and patients across the globe, this collection brings together a comprehensive overview of major achievements in patient engagement, analysing political, organizational and clinical contexts. By understanding the concept of care partnership, the authors explore how this patient revolution could transform, improve and innovate the ways in which care services are organized and delivered. Looking closely at the role of new technologies, this timely book will undoubtedly be of use to patients, managers and professionals within the healthcare industry, as well as those researching health policy and organization.

Available in PDF, EPUB and Kindle. Book excerpt: Why has health care reform proved a stumbling block for provincial governments across Canada? What efforts have been made to improve a struggling system, and how have they succeeded or failed? In Paradigm Freeze, experts in the field answer these fundamental questions by examining and comparing six essential policy issues - regionalization, needs-based funding, alternative payment plans, privatization, waiting lists, and prescription drug coverage - in five provinces. Noting hundreds of recommendations from dozens of reports commissioned by provincial governments over the last quarter century - the great majority to little or no avail - the book focuses on careful diagnosis, rather than unplanned treatment, of the problem. Paradigm Freeze is based on thirty case studies of policy reform in Alberta, Saskatchewan, Ontario, Quebec, and Newfoundland and Labrador. The contributors assess the nature and extent of healthcare reform in Canada since the beginning of the 1990s. They account for the generally limited extent of reform that has occurred, and identify the factors associated with the relatively few cases of large reform. An insightful new perspective on a problem that has plagued Canadian governments for decades, Paradigm Freeze is an important addition to the field of health policy. Contributors include John Church (University of Alberta), Michael Ducie (Alberta Health and Wellness), Pierre-Gerlier Forest (Pierre Elliott Trudeau Foundation), Stephen Tomblin (Memorial University), Jeff Braun Jackson (Ontario Professional Firefighters Association, Burlington, ON), Marie-Pascale Pomey (Université de Montréal), John N. Lavis (McMaster University), Harvey Lazar (Queen's University), Elisabeth Martin (Université Laval),Tom McIntosh (University of Regina), Dianna Pasic (McMaster University), Neale Smith (University of British Columbia), and Michael G. Available in PDF, EPUB and Kindle. Book excerpt: Health rights are a common but controversial legal phenomenon. Every country is signatory to a treaty that incorporates health rights, yet existing health rights do not fit easily into the traditional "claim right" model, and questions remain over how to theoretically incorporate health rights into domestic systems. The Pluralist Right to Health Care addresses this incongruity between theory and practice with an account of the right to health care that is both philosophically and practically sound. Utilizing a pluralist framework, Michael Da Silva argues that the right to health care is best understood as a set of claims to related ends: the goods necessary for a dignified existence, procedural fairness in determining what other goods to provide and in the provision of goods, and a functioning health care system. Through philosophical reasoning, analysis of relevant international human rights law, and a close study of the Canadian case, The Pluralist Right to Health Care provides crucial insight into the potential of law and policy to improve health care systems in Canada and beyond.

Available in PDF, EPUB and Kindle. Book excerpt: Located at the junction of the St. Lawrence and Ottawa rivers, Montreal Island is the main contact point between French and English Canadians. Prior to Quebec's "Quiet Revolution" of the 1960s, local governments in Montreal both reflected and perpetuated the mutual isolation of French and English. Residential concentration in autonomous suburbs, together with self-contained networks of schools and social services, enabled English-speaking Montrealers to control the city's economy and to conduct their community's affairs with little regard for the French-speaking majority. The modernization of the Quebec state in the 1960s dramatically challenged this arrangement. The author demonstrates how the English-speaking politicians in cooperation with certain French-speaking allies have succeeded in preventing the wholesale adoption of ambitious schemes for metropolitan reorganization. He describes the workings of a society divided by language and ethnicity, where the pervasiveness of the politics of language impedes all plans for comprehensive metropolitan reform. Available in PDF, EPUB and Kindle. Book excerpt: Canada’s fragmented healthcare system is one of the most expensive among the OECD countries, yet the quality of its performance is mediocre at best. Canada lacks a system-wide healthcare strategy that brings together many individual federal, provincial, and territorial strategies into a comprehensive and coherent whole. Managing a Canadian Healthcare Strategy is a collection of ten policy research essays by leading Canadian and international scholars who address three important questions. First, if Canada had a unifying strategy, how would the country measure its success and monitor its performance? Second, who are the agents of change to bring about a Canadian system-wide strategy? Third, how can the jurisdictional realities of Canada’s political system be managed to bring about strategic reform? The final section in the volume explores ways to overcome the barriers and impediments that preoccupy Canadians’ concerns about healthcare. A companion volume to Toward a Healthcare Strategy for Canadians, the contributors to Managing a Canadian Healthcare Strategy turn to the critical importance of how necessary healthcare changes can be best implemented.
