Clinical and radiographic assessment of periodontal disease remains the basis for patient evaluation. In general clinical parameters including bleeding on probing, plaque index and radiographic loss of alveolar bone are used to assess disease severity. Occasionally, monitoring of microbial infection and analysis of host response in the gingival crevicular fluid are utilized in an attempt to identify individuals at risk for future periodontal breakdown. However till date no clinical or laboratory test is routinely employed in the monitoring of patients with periodontal disease. Saliva can be collected easily and less invasively than blood, therefore interest in saliva as a diagnostic fluid has received increased attention during the past two decades. It has been discussed lately as an important biological material to the purpose to introduce new diagnostic tests which may contribute to making a diagnosis of periodontal diseases.

Study showed the presence of elevated levels of C-Reactive protein in gingival crevicular fluid of patients with periodontitis. Non-surgical periodontal treatment was effective in reducing the levels of C-Reactive protein in gingival crevicular fluid. This study data support the hypothesis that levels of gingival crevicular fluid biomarkers specific for three aspects of periodontitis-inflammation, collagen degradation and bone turnover-correlate with the clinical features of periodontal disease and suggest that elevated gingival crevicular fluid levels of C-Reactive protein are candidate biomarkers of periodontal disease. Hence gingival crevicular fluid CRP determination should always be considered as inflammatory marker before treatment of Periodontitis.

Book excerpt: The purpose of this study was to quantify glycosaminoglycans (GAGs) released into the gingival crevicular fluid (GC) during health, gingivitis and adult periodontitis. This investigation tested the hypothesis that increased amounts of GAGs can be measured in GCF associated with adult periodontitis and gingivitis. An individual patient's sampling sites were assigned to either teh health (control) group or one of three experimental groups (gingivitis, periodontal "maintenance" (perio-M) or periodontal "non-maintenance" (perio-NM)) according to standard clinical criteria, i.e., pocket depth, bleeding on brobing, radiographic and/or clinical evidence of bone loss. The perio-M group was defined as a periodontal patient that had received a dental prophylaxis and /or root planing within six months prior to GCF collection. The perio-NM group had received no periodontally related therapy during the previous six months. GCF was collected by a microcapillary pipette held at the gingival margin subsequent to air-drying and isolation. All fluid samples were digested overnight at 37°C with 25 [micrograms] of papain and analyzed for GAG content using a chondroitin-4-sulfate standard. Utilizing data generated from safranin "O" dye binding assays of GAGs revealed 4.41 ± 9.82 ng GAG from the helath (control) group (n=23); the gingivitis group (n=13) shoed 15.23 ± 11.95 ng GAG/sample; perio-M group (n=11) showed 23.64 ± 12.98 ng GAG/sample and the perio-NM group (n=12) exhibited 119.08 ± 33.14 ng GAG/sample.
