Après la définition de la notion de qualité de vie et la présentation des données bibliographiques sur la question il décrit l'enquête menée auprès de 146 médecins de campagne du département de la Dordogne et les résultats obtenus. Elle a pour objectif de recueillir l'avis subjectif des médecins de campagne sur leur propre qualité de vie à l'aide d'un questionnaire. J'ai élaboré le questionnaire moi-même en m'inspirant des données de la bibliographie. Il comporte 11 questions ouvertes portant sur des renseignements personnels et professionnels, 15 affirmations portant sur la qualité de vie professionnelle, 5 sur la qualité de vie familiale et 7 sur la qualité de vie personnelle. Les médecins devaient donner leur degré d'accord vis-à-vis des affirmations en entourant la proposition qui correspondait le plus à leur situation personnelle. Pour 12 affirmations de qualité de vie une bonne majorité des médecins se sont prononcés positivement à leur égard. Pour 8 affirmations une bonne majorité des médecins se sont prononcés négativement à leur égard. Pour sept affirmations du questionnaire aucune majorité claire ne s'est dégagée.

De ce fait, lors d'une période d'absence, ce dernier va rechercher un remplaçant afin de permettre une continuité des soins pour ses patients. Mais cette recherche semble de plus en plus difficile à satisfaire pour les médecins généralistes français. OBJECTIF :l'objectif de notre étude est de mettre en évidence les attentes respectives des médecins installés et des médecins remplaçants, vis-à-vis du déroulement d'un remplacement en médecine générale. MÉTHODE : une revue de littérature a été réalisée par un seul chercheur sur le sujet, en suivant les lignes directrices du guide de rédaction PRISMA. Les bases de données de recherches utilisées sont : PubMed/Medline, SUDOC, Ulysse, BDSP et LiSSa. RÉSULTATS : ur 1200 résultats de recherche, 27 sources ont été incluses. Différents critères, ayant une tendance à influencer le bon déroulement d'un remplacement, ont été mis en évidence : la relation confraternelle, le professionnalisme, l'adaptabilité, la traçabilité, le relationnel et le mimétisme pour les médecins installés. Des critères géographiques, d'organisation, de rentabilité, d'autonomie, de similitude des pratiques professionnelles pour les médecins remplaçants. La tendance des médecins installés à ressentir une difficulté dans la recherche d'un médecin remplaçant apparaît dans notre analyse.DISCUSSION : les différents critères retrouvés dans notre étude concordent avec le fait que les jeunes médecins remplaçants recherchent une qualité de vie et de travail. L'exercice du remplacement leur autorise une liberté contrairement à l'installation qui leur apparaît plus restrictive et contraignante. CONCLUSION : dans le but de maintenir une continuité des soins pour ses patients, il existe actuellement une difficulté pour les médecins installés à trouver un remplaçant. Ces derniers, du fait d'une actuelle balance démographique en leur faveur, vont sélectionner leur remplacement en fonction de critères qui permettront, au mieux, un confort de vie et un confort de travail.

Leur origine rurale est souvent la raisons principale qui preside au choix de leur installation. La moindre concurrence qu'il rencontre et les connaissances souvent limitees des populations rurales dans le domaine medical sont des elements qui interviennent egalement dans leur decision. Les docteurs en medecine, issus le plus souvent de la bourgeoisie, accedent a une carriere medicale apres de longues etudes couteuses. Ils s'installent eux aussi volontiers a la campagne notamment lorsqu'ils y ont des attaches familiales. La notoriete de leur famille leur fait esperer la constitution rapide d'une clientele et l'exploitation du domaine familial represente en outre une source de revenus appreciable. Aujourd'hui les jeunes medecins hesitent a s'installer en milieu rural et l'on constate une sous-medicalisation importante dans ce secteur. La charge de travail, les nombreuses gardes a assumer au sein de secteurs sanitaires importants rebutent bon nombre de candidats. La feminisation importante de la profession, la volonte de vouloir preserver une qualite de vie et la peur de l'isolement contribuent eux aussi a expliquer le desinteret actuel pour la medecine rurale. Les projections dans l'avenir d'une telle situation risquent d'aboutir a l'apparition de zones de "non-droit-medical". Aussi l'installation de jeunes medecins en secteur rural doit etre encouragee : une meilleure connaissance de la medecine rurale au travers de la mise en place de stages chez des praticiens exercant a la campagne ; l'aide a la creation de "Maisons Medicales" regroupant les urgences d'une meme zone et ou les medecins du meme secteur interviennent a tour de role, representent des voix de recherche a prendre en compte pour eviter la perennisation de cette demedicalisation en secteur rural. Dans cette optique ont peut se demander si l'assistance des medecins de campagnes, par des officiers de sante exercant sous l'autorite d'un medecin, apres avoir recu une formation adaptee ne pourrait pas representer l'un des moyens meritant d'etre etudie pour eviter la desertification medicale du secteur rural.

Book excerpt: Articulation ou conciliation, conflit ou harmonisation, la question de l'équilibre entre vie personnelle et vie professionnelle chez le médecin généraliste installé en libéral ou en salariat est cruciale et suit les mutations que la société française a connues depuis les cinq dernières décennies. En lien direct avec le syndrome d'épuisement professionnel, l'attractivité de la profession est en jeu. L'objectif de cette étude est de s'intéresser à la représentation de cet équilibre par les médecins généralistes installés en analysant leurs différentes perceptions et les déterminants d'un équilibre entre vie personnelle et vie professionnelle jugé satisfaisant. Etude qualitative par entretiens semi-dirigés auprès de 12 médecins généralistes installés en libéral ou salariat. La stratégie d'échantillonnage en "variation maximale" a été utilisée pour la sélection des participants. Les entretiens étaient enregistrés, puis retranscrits et anonymisés. Une analyse thématique transversale par encodage sémantique a ensuite été réalisée. Le bon équilibre est perçu comme étant l'absence, ou du moins le délicat contrôle, des contraintes dans l'interaction mouvante et évolutive d'une accumulation de facteurs, pour une personne évoluant à un moment donné, dans un environnement donné, avec une histoire personnelle particulière. Face à ces contraintes rencontrées, les médecins généralistes ont développé des stratégies différentes qui vont déterminer, conditionner le "bon" équilibre qui leur convient. Savoir-agir complexe, cet équilibre nécessite que le médecin entre dans un processus réflexif dès le début de sa carrière, puis pendant toute la durée de son activité professionnelle. Une tendance est observée : la vie personnelle, notamment familiale, constitue la base d'une pyramide sur laquelle repose la vie professionnelle. Une vie personnelle solide, développée et entretenue est indispensable. Le "bon" équilibre entre vie professionnelle et vie personnelle est perçue conformément au paradigme du conflit, approche dominante de la recherche sur les relations entre travail et hors-travail, avec un objectif de segmentation entre vie personnelle et vie professionnelle. Des mesures protectrices sont développées et nécessitent un sentiment de liberté d'action. Les relations entre travail et hors-travail peuvent être vues selon un autre angle, celui du paradigme de l'enrichissement : l'un bénéficie de la présence de l'autre, entretenant une relation symbiotique. Il serait intéressant d'étudier ultérieurement comment renforcer la réconciliation entre vie personnelle et vie professionnelle dès le début de la formation initiale du médecin généraliste, et comment optimiser cet enrichissement réciproque.

The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
