La dernière décennie a été marquée par l’avènement des inhibiteurs de point de contrôle immunitaire (IPCIs), grâce notamment aux essais cliniques randomisés (ECRs). La mesure de la toxicité est un point-clé de l’appréciation de la balance bénéfice/risque de ces traitements. L’objectif de cette revue systématique était d'évaluer la qualité de la restitution des toxicités des IPCIs dans les ECRs de phase 3 chez les patients atteints de cancer. Méthode. Les essais de phase 3 impliquant un IPCI en oncologie publiés jusqu’en septembre 2019 ont été inclus. Deux évaluateurs ont déterminé en double aveugle la concordance de la description de la toxicité avec les recommandations internationales TRIO et 2004 CONSORT. Résultats. Au total, 46 essais avec 30 615 patients ont été inclus. Parmi eux, 74 % des études présentaient l’incidence des effets indésirables (EIs) de grade 3 et 4 ensemble. Les toxicités de grade 2 n’étaient jamais décrites séparément. Le délai d’apparition, la durée et le taux de guérison des EIs étaient rapportés dans 30 %, 28 % et 26 % des études, respectivement. Les EIs dont l’incidence était inférieure à 10 % n’étaient présentés que dans 35 % des publications. L’évaluation de la tolérance par le patient lui-même n’était faite que dans 17 % des articles. Enfin, 8 études définissaient le profil de toxicité comme « maîtrisable », « tolérable », « bien toléré » ou « favorable » malgré un taux d’EIs de grade 3 ou 4 supérieur à 33 %. Conclusion. La toxicité est insuffisamment rapportée dans les essais cliniques d’oncologie impliquant les IPCIs. De nouveaux outils d’évaluation pourraient permettre une amélioration en ce sens.

La recherche clinique en oncologie est en plein essor depuis l'avènement de thérapies telles que les thérapies moléculaires ciblées ou l'immunothérapie. Le nombre de nouveaux médicaments en développement en oncologie a augmenté de façon exponentielle depuis les années 2000. Il en découle un accroissement important du nombre de publications d'études en oncologie. L'analyse d'une étude n'est possible que si son schéma expérimental, sa réalisation et ses conditions d'analyse sont clairement décrits dans l'article publié. Le reporting est le fait de décrire dans une publication toutes les actions, notamment d'ordre méthodologiques, réalisées au cours de l'étude. 5 revues systématiques de la littérature ont été réalisées afin d'analyser le reporting des articles d'études en oncologie. La première étude a objectivé que la littérature était pauvre en articles analysant le reporting des études observationnelles (2% des publications) et que le reporting intrinsèque des revues systématiques récentes portant sur le sujet était exhaustif (score OQSR moyen (2010-2014) = 10, écart-type =2). 826 articles d'études ont été analysés dans la deuxième revue systématique, dont 84.5% d'études observationnelles. Les reporting des variables et du critère de jugement principal étaient clairs, respectivement dans 100% et 85.2% des cas. Une discordance de reporting pour le test statistique principal utilisé, entre les parties Méthodes et Résultats, a été identifié dans 23 articles. La troisième revue systématique a mis un évidence un manque de données sur le reporting des essais de phase II en oncologie (13 articles) et 3 scores d'évaluation du reporting avaient été décrits : score OQS, score KMS et score d'index de qualité. 557 publications d'essais de phase II ont été étudiées dans la quatrième revue systématique. Le score de reporting KMS était égal à 0, 1, 2, 3 respectivement pour 3,9%, 21,4%, 48,5% et 26,2% articles. Ovarian epithelial cancer represents approximately 90% of all ovarian malignant tumours. The most usual pattern of spread and probably the earliest kind of metastasis of ovarian cancer is intraperitoneal spread, caused by apoptosis of viable cancer cells, even in cases where the ovarian surface has no visible rupture. Approximately 70% to 75% of all ovarian cancers are being diagnosed with peritoneal carcinomatosis and ascites in advanced stages III and IV. Peritoneal carcinomatosis from ovarian cancer may occur either concomitantly with the primary tumour or as a recurrence pattern of spread. Standard treatment consists of surgical debulking and postoperative systemic chemotherapy. Response rates to first-line chemotherapy with platinum/taxane combinations are high, about 70-80%, but the vast majority of patients will relapse. Peritoneal seeding is a major cause of surgical treatment failure among those patients, even after optimal debulking. However, progressing ovarian cancer tends to remain within the peritoneal cavity or, better-expressed, ovarian cancer is a cancer of the peritoneum. The administration of cytotoxic agents directly into the peritoneal cavity as treatment for malignancy was initially evaluated more than 40 years ago. Over the past two decades regional therapy of ovarian cancer has evolved from just an interesting pharmacokinetic concept into a viable management option for women with advanced disease. Several authors, and among them some of the world's leading experts on this field present the current knowledge and their experience on peritoneal carcinomatosis from ovarian cancer.
