The treatment of the purchaser-provider split will be diveded into two parts. One is a more general preamble, just giving the principles. The other, the core of this paper, will concentrate on four cases, four "models" as they are known in Sweden. Some topics will be particularly explored; main tasks for the purchaser, freedom of choice for the patient and the relationship between purchaser and provider. From these examples some of the key features of the reforms will be touched upon in a more problem oriented and critical way; the competence and status of the purchaser, the responsibility for the health of the population, the control over development of total expenditures and quality assurance.
