Policy makers have responded with a variety of state and federal policies to reduce infections by increasing the visibility and accountability of hospital performance. One policy initiative that has gained momentum is state-level legislative mandates requiring hospitals to report HAI-related performance data, which often includes public release of this data. These reporting mandates have produced mixed results, however, regarding their impact on healthcare organizational processes, patient outcomes or consumer decision-making.This dissertation comprises three papers that explore the relationship between HAI public reporting, organizational climate and HAI infection rates. The first paper presents a conceptual framework derived from an integrative review of the HAI reporting literature that proposes new pathways for testing these relationships. The second paper uses data from the Prevention of Nosocomial Infections and Cost-Effectiveness Refined (P-NICER) survey, specifically the Leading a Culture of Quality for Infection Prevention (LCQ-IP) instrument, to examine differences in infection prevention organizational climate between hospitals in states with and without HAI reporting mandates. Bivariate and multivariate analysis revealed no statistically significant associations between key climate domains supporting infection prevention and state reporting mandates, despite finding noted associations with other hospital characteristics. The final paper uses National Healthcare Safety Network (NHSN) surveillance data, from the P-NICER survey, to conduct a quasi-experimental longitudinal analysis examining the impacts of reporting mandates on Clostridium difficile (C. difficile) infections in U.S. hospitals. Poisson regression models yielded no statistically significant differences in incidence rate ratios for C. difficile at varying time intervals before and after implementation of reporting mandates. Sensitivity analysis showed similar findings, with no differences in rates of infections over time between hospitals in reporting and non-reporting states. This dissertation provides a well-circumscribed analysis of varying organizational factors and patient outcomes thought to be impacted by mandatory HAI reporting. Findings are used to propose new directions for nursing research and public policy.

Book excerpt: Healthcare-associated infections (HAIs), also known as hospital-acquired infections or nosocomial infections, exact a significant toll on human life. They are among the leading causes of death in the United States, accounting for 99,000 deaths annually. HAIs affect patients, health care systems and society by increasing the cost of treating infections and causing greater disability and death. Since 2005, the number of states with laws requiring health care facilities to report HAIs has grown from six to 27. At the heart of public reporting is the belief that promoting transparency will improve quality of care, expand and improve infection prevention measures, reduce the morbidity and mortality associated with HAIs and cut costs. Research about outcomes from public reporting is in its infancy and more work will need to be done to assure that reporting meets its goals of higher quality, lower costs, and a more informed public. Setting up a reporting program is complex and time-consuming, and a successful program must have skilled staff and adequate, sustainable financing. This report examined reporting laws from nine states and gathered information about the challenges, successes and 'lessons learned' from states that have pioneered public reporting.

These infection rates were hypothesized to be decreased due to mandatory public reporting required by state legal statute. Archival data of 145 hospitals were analyzed. These hospitals had been grouped into four different peer groups according to the level of complexity of services, number of patients treated, and percentage of surgical procedures performed. Results demonstrated that health care associated infections in Pennsylvania hospitals did not decrease as the legislation had intended. The four most common types of hospital acquired infections: Surgical site infection, blood stream infection, ventilator assisted pneumonia, and urinary tract infection had not decreased in 2007, compared with the first reporting period (in 2005) subsequent to legislature enactment (in 2004). However, a small decrease of surgical site infection rate occurred compared to the 2005 baseline. Possible explanations were discussed.

At any one time, up to 7% of patients in developed and 10% in developing countries will acquire at least one HAI. These infections also present a significant economic burden at the societal level. However, a large percentage are preventable through effective infection prevention and control (IPC) measures. These new guidelines on the core components of IPC programmes at the national and facility level will enhance the capacity of Member States to develop and implement effective technical and behaviour modifying interventions. They form a key part of WHO strategies to prevent current and future threats from infectious diseases such as Ebola, strengthen health service resilience, help combat antimicrobial resistance (AMR) and improve the overall quality of health care delivery. They are also intended to support countries in the development of their own national protocols for IPC and AMR action plans and to support health care facilities as they develop or strengthen their own approaches to IPC. These are the first international evidence-based guidelines on the core components of IPC programmes. These new WHO guidelines are applicable for any country and suitable to local adaptations, and take account of the strength of available scientific evidence, the cost and resource implications, and patient values and preferences.
