The theoretical development is traced through Freud, Klein and Bion, along with contemporary Kleinian authors. An important role is granted to psychic pain as the cornerstone of psychopathology, and particularly to the psychotic patient's difficulties in dealing with it. Bion's distinction between "feeling psychic pain and suffering it" is considered an indicator when evaluating the patient's ability to cope with psychoanalytic treatment. The author's experience with a schizophrenic patient is related in detail, offering a view of the patient and her relationship with the analyst from various different angles, and showing how the psychoanalytic method can be used to treat psychosis.

John Steiner, an experienced psychoanalyst, uses new developments in Kleinian theory to explain how this happens. He examines the way object relationships and defences can be organized into complex structures which lead to a personality and an analysis becoming rigid and stuck, with little opportunity for development or change. These systems of defences are pathological organisations of the personality: John Steiner describes them as 'psychic retreats', into which the patient can withdraw to avoid contact both with the analyst and with reality. To provide a background to these original and controversial concepts, the author builds on more established ideas such as Klein's distinction between the paranoid-schizoid and depressive positions, and briefly reviews previous work on pathological organizations of the personality. He illustrates his discussion with detailed clinical material, with examples of the way psychic retreats operate to provide a respite from both paranoid-schizoid and depressive anxieties. He looks at the way such organizations function as a defence against unbearable guilt and describes the mechanism by which fragmentation of the personality can be reversed so the lost parts of the self can be regained and reintegrated in to the personality. Psychic Retreats is written with the practising psychoanalysts and psychoanalytic psychotherapists in mind. The emphasis is therefore clinical throughout the book, which concludes with a chapter on the technical problems which arise in the treatment of such severely ill patients.

This book reviews in detail the neuroscience of brain and mind development, including the neuroscience of psychoanalytic concepts, both for normal and disordered personalities. In contrast to the current static classifications of personality pathology, the authors' approach yields a dynamic and personalized diagnosis within a 3D diagnostic space in which each individual is uniquely positioned. In this model, two intersecting dimensions, one vertical, representing the person's qualitative level of mental functioning (the "how" of personality), and the other horizontal, representing his or her adaptive style (the "what" of personality) are cross matched in the unit of time. Such dynamic nosology is inherently sensitive to fluctuations in mental functioning over time and context, and gives the clinician precise milestones for monitoring progress in therapy. In this book, the authors analyze the impact of social transitions on adaptive tasks, personality and psychopathology. They argue that the conservative society, with strict socio-religious norms, favored the psychopathology of neuroses centered around guilt, including guilt for not fitting the preapproved norms. Ausubel's central thesis is that the most significant and crucial factors predisposing persons to mental disorders are critical developmental defects that arise at vital transitional phases of ego development. He sees both normal and pathological personality functioning as most cogently explained in terms of an individual's ego structure at a given point in the life cycle. Ausubel relates his developmental theory to the phenomenology and related clinical problems of psychiatric diagnosis. He classifies mental disorders in terms of their developmental history. Such factors, in his opinion, offer the most precise delineation of etiological, functional, and phenomenological similarities and differences among the various psychiatric syndromes. He provides an overview of ego development, as well as major variants of the norm. He also discusses development of conscience and moral values, as well as psychopathological considerations that follow from deficiencies, defects, failure, and distortions in ego development. Ausubel is careful to point out that ego development is not the only significant determinant of normal and aberrant personality. Genetic predispositions, situational stress, and sociocultural factors must always be taken into consideration since mental disorder is always a product of multiple causality. However, he believes ego development is by far the most critical factor, and hence offers the most for classification of mental illness. This intriguing study will be of interest to professionals as well as educated and concerned practitioners in the fields of psychology, psychiatry, psychoanalysis, child psychotherapy, and social work.
