Abstract: The last oral health survey conducted by the government of Hong Kong in 2001 found that dental caries was common among the kindergarten children and that most of their dental caries was untreated. The overall aims of this research project were to find out the oral health status of the kindergarten children in Hong Kong and the outcomes of providing a private practice based dental service programme to these children. To achieve the first aim, an oral health survey was conducted in 2007. Through stratified simple random sampling, 12 kindergartens in Hong Kong Island were selected. All children attending grades 1 to 2, aged 3 to 5 years, in the kindergartens were invited to participate. Children with parental consent were clinically examined by one of two calibrated examiners. A questionnaire on the child's demographic background and oral health related behaviours was completed by the parents. Response rate of the survey was 89% and 1,343 children were examined. Their mean age was 52 months (SD=7) and 51.5% were boys. Around one third (35.3%) of the children had experienced dental caries. The mean dmft score was 1.5, increasing from 1.2 at age 3 to 2.0 at age 5 (ANOVA, p=0.016). Soft, cavitated caries (dt) accounted for 90% of the dmft score. The prevalence and severity of dental caries in the children were associated with the children's socio-demographic background, and their dietary and tooth-brushing habits. To find out the feasibility of implementing a private clinic based dental service program for the preschool children a total of 245 children attending grade 1 in 9 kindergartens were invited to participate. Baseline information of the children was collected through a clinical examination and a parental questionnaire at April 2007. After baseline data collection, the study children were randomly allocated into the test or the control group. Children in the test group were offered free dental services at regular intervals in 3 private dental clinics. Evaluation took place in July 2009 after the completion of the 24-month intervention programme through conducting clinical examinations and a parental questionnaire. After 24 months, 88% of the control and test group students remained in the study. The mean dental appointment attendance rate of the test group children was 53%. Individualized instructions on oral hygiene were provided to 82% of the children and 52% of the children had received topical fluoride application. Treatments were provided to more than 30% of the children and the mean number of dental fillings placed is 1.1. The mean total running cost per child was around HK$100 a year. The kindergarten children who were covered by the study dental service programme had better oral health than that of the children who were not. The study programme was effective in preventing development of new dental caries and in reducing the amount of untreated caries among the test group children. However, it was not effective in improving the oral health related behaviours and oral hygiene status of the children. The study programme was acceptable to the kindergartens, the parents and the children by the satisfaction study. Consumer satisfaction was reasonable high. It is considered as a major public health problem affecting pre-school children and it is the most common dental disease which affects 28% of children within two to five years of age. ECC not only induces pain and discomfort, but can also affect communication, nutrition status, learning abilities, speech and quality of life, which may progress into adulthood and pose a heavy burden on the healthcare system in long-term. In Hong Kong, over 50% of children were affected by dental caries in 2001 but over 70% of children had never been to a dentist for a regular check up at age five. Thus, much of the tooth decay was remained undetected and untreated. Dental caries can be preventable and achievable. Collaboration between families, early care and health care professionals is required to promote effective oral health care. Numerous studies have found that educational programs and workshops are effective in promoting oral health and can provide children a lifelong opportunity to be free from preventable oral disease. In order to promote oral health to achieve the mission of the Department of Health in Hong Kong- at least 65% of 5 years old children are free from caries by the year 2020, an evidence-based guideline for a community-based programme in oral health education was developed in the proposed setting after a critical appraisal of the reviewed evidence. The comprehensive intervention plan, including communication plan with stakeholders, training of staffs and pilot testing will be carried out to facilitate the implementation of the innovation. The oral health programme will be evaluated for its effectiveness in achieving the patient outcomes, health providers' outcomes and system outcomes in the proposed settings. Abstract: A review of available oral epidemiological studies among children in Hong Kong was conducted to evaluate trends in their oral health status. Since the 1960s, a decline in the prevalence and extent of dental caries experience was observed among school children and adolescents. However, among pre-school children, dental caries experience remains common and it appears that the extent of dental caries has increased. Over 90% of the dental caries experience among pre-school children is untreated. This high unmet treatment need suggests barriers to access of appropriate oral health care and this urgently warrants consideration. A random sample of 476 ( 25% of all) general dental practitioners (GDPs) and all 28 paediatric dentists (PDs) were recruited to participate in a survey in Hong Kong. Beliefs and attitudes towards providing care to pre-school children were determined using the Barriers to Childhood Caries Treatment (BaCCT) measure. In addition, dentists' treatment preferences for caries of different severities were determined. The response rate was 61.5% (310/504). There was a significant difference in BaCCT scores between GDPs and PDs (PIn collaboration with the Hong Kong Society of Paediatric Dentistry (HKSPD), clinical guidelines for 'Dental caries prevention and management by caries risk assessment for pre-school children in Hong Kong' were developed using the ADAPTE process and the Delphi consensus technique. The draft guidelines were reviewed by 36 members of HKSPD with a response rate of 52.9% (36/68). Seventy out of 91 draft recommendations were retained where there was consensus among members. The HKSPD adopted the guidelines. At a teaching hospital, the feasibility of implementing the guidelines and the effectiveness in changing clinical practice was determined over a 6-month period. Clinical records of preschool children treated at the hospital over a 3-month period prior to implementing the guidelines were reviewed (N = 237). The guidelines were implemented over a one month period and clinical records for the following 3-month period were reviewed (N = 147). Book excerpt: BACKGROUND: In 2014, the U.S. Preventive Services Task Force (USPSTF) found insufficient evidence to assess the benefits and harms of screening for dental caries, but recommended that primary care clinicians prescribe oral fluoride supplementation to preschool children starting at age 6 months whose primary water source is deficient in fluoride and apply fluoride varnish to the primary teeth of all infants and children starting at the age of primary tooth eruption. PURPOSE: To systematically review the current evidence on primary care screening for and prevention of dental caries in children younger than 5 years old. DATA SOURCES: We searched the Cochrane Central Register of Controlled Trials and Cochrane Database of Systematic Reviews (through April, 2021), and MEDLINE (2013 to April, 2021); with surveillance through July 23, 2021, and manually reviewed reference lists. STUDY SELECTION: Randomized controlled trials (RCTs) and controlled observational studies on benefits and harms of screening versus no screening and referral to dental care from primary care versus no referral; studies on the diagnostic accuracy of oral examination and risk assessment by primary care clinicians; RCTs on benefits and harms of oral health education and preventive interventions; and systematic reviews on risk of fluorosis associated with early childhood ingestion of dietary fluoride supplements. DATA EXTRACTION: One investigator abstracted data and a second investigator checked data abstraction for accuracy. Two investigators independently assessed study quality using methods developed by the USPSTF. DATA SYNTHESIS (RESULTS): Thirty-three studies (reported in 36 publications) were included in this update (19 RCTs, four non-randomized trials, nine observational studies, and one systematic review [19 studies]). Seventeen studies were newly identified as part of this update and 16 studies (including the systematic review) were carried forward from the previous review. No randomized trial or observational study compared clinical outcomes between children younger than 5 years of age screened and not screened by primary care clinicians for dental caries. One good-quality cohort study (n=258) found primary care pediatrician examination following 2 hours of training associated with a sensitivity of 0.76 (95% confidence interval [CI], 0.55 to 0.91) for identifying a child with one or more cavities and 0.63 (95% CI, 0.42 to 0.81) for identifying children younger than 36 months of age in need of a dental referral, compared with a pediatric dentist evaluation. One study (n=697) found a novel risk assessment tool administered by home visitor nurses associated with suboptimal accuracy for predicting future caries in children 1 year of age. The prior USPSTF review found oral fluoride supplementation associated with reduced caries incidence versus no supplementation in children younger than 5 years of age in settings with inadequate water fluoridation, though only one trial was randomized; we identified no new trials. The prior USPSTF review included a systematic review of observational studies which found an association between early childhood ingestion of systemic fluoride and enamel fluorosis. Topical fluoride (all trials except for one evaluated varnish) associated with decreased caries increment (13 trials in updated meta-analysis, N=5733, mean difference in decayed, missing, and filled teeth or surfaces −0.94, 95% CI, −1.74 to −0.34) and decreased likelihood of incident caries (12 trials, N=8177, RR 0.80, 95% CI, 0.66 to 0.95; absolute risk difference −7%, 95% CI, −12% to −2%) versus placebo or no varnish, with no increase in risk of fluorosis or other adverse events. Almost all trials of topical fluoride were conducted in higher risk populations or settings. Evidence on other preventive interventions was limited (xylitol) or unavailable (silver diamine fluoride). Evidence on educational or counseling interventions is very sparse and no studies directly evaluated the effectiveness of primary care referral to a dentist versus no referral. LIMITATIONS: Only English-language articles were included. Graphical methods were not used to assess for publication bias, due to diversity in populations, settings, and outcomes, and substantial statistical heterogeneity. Statistical heterogeneity was present in pooled analyses of fluoride varnish and not explained by stratification on a variety of factors. Studies conducted in resource-poor settings may be of limited applicability to screening in the United States. Most studies had methodological limitations. CONCLUSIONS: Dietary fluoride supplementation and fluoride varnish appear to be effective at preventing caries outcomes in higher risk children younger than 5 years of age. Dietary fluoride supplementation in early childhood is associated with risk of enamel fluorosis, which is usually not severe. More research is needed to understand the accuracy of oral health examination and caries risk assessment by primary care clinicians, primary care referral for dental care, and effective parental and caregiver/guardian educational and counseling interventions.
