Available in PDF, EPUB and Kindle. Book excerpt: The Facts of Prostate Cancer 2/3 of men over age 50 have low grade tumors or potential cancer cells in their prostate. Only 10% will develop clinically aggressive prostate cancer. These non-invasive exams tell if a tumor is aggressive or not with an accuracy exceeding traditional biopsy results and correlate well with prognosis according to the latest international cancer conferences. The PSA test has a poor correlation with cancer detection. The digital rectal exam is more accurate in detecting high grade tumors than the PSA level. Biopsies are random, invasive and may spread tumor cells. New non-invasive treatments are guided by advanced radiologic imaging technologies. The therapeutic effect is serially studied by MRI and 3-D sonography. While generally successful, interval exams may document an unsatisfactory response, allowing "standard" treatments to be started, if necessary, in a timely manner.

Book excerpt: An indispensable map through the medical minefield of prostate cancer, revised and updated with the latest developments in treatment options. Every year almost a quarter of a million confused and frightened American men are tossed into a prostate cancer cauldron stirred by salespeople representing a multibillion-dollar industry. Patients are too often rushed into a radical prostatectomy, a major operation that rarely prolongs life and more than half the time leaves them impotent. Invasion of the Prostate Snatchers argues that close monitoring—active surveillance rather than surgery or radiation—should be the initial treatment approach for many men at the low- and intermediate-risk stages. In a unique collaboration, doctor and patient provide a wholly new perspective on managing this disease. Ralph Blum’s account of his personal struggle, together with Dr. Mark Scholz’s presentation of new scientific advances, offers convincing evidence that this noninvasive approach can be crucial in preventing tens of thousands of men from being overtreated each year. This revised and updated second edition: -Highlights the latest prostate cancer treatment options that preserve erectile and urinary function while prolonging lifespan -Establishes updated protocol for how to accurately interpret high prostate-specific antigen (PSA) while avoiding the invasive, dangerous random biopsy -Recounts true patient stories that accurately represent the hurdles today’s patients face while navigating a prostate cancer diagnosis -Introduces the latest evidence in the radiation vs. radical prostatectomy debate -Presents the latest thinking on diet and exercise for men with prostate cancer

Book excerpt: Although it is the most frequently diagnosed cancer in men, prostate cancer is highly misunderstood. The disease has little effect on many, yet it is deadly to others. The book is a form of expedition-an expedition to explore the scientific wildernesses of biology, chemistry, and physics of the mechanisms that promotes and fuels the deadly form of Prostate Cancer. Readers will journey through the modern understanding of the disease that physicians use to exploit its weaknesses, and that result in therapies to reduce or mitigate its devastation. The expedition is for those who want to dig deeper into the subject of advanced prostate cancer. The account is written in understandable terms for a layman, yet retains some scientific rigor without the numbing jargon used in evidence-based scientific and medical journals. The terrain covered is not the initial treatments of prostate cancer-watchful waiting or active surveillance, proton therapy, brachytherapy, external beam therapy, or radical prostatectomy. There are many good books on these topics. This book is primarily about when prostate cancer takes a turn for the worse, and becomes advanced. Most books stop, or say little, on this topic. We pop the hood and take a look at the inner workings of the disease-the science of the disease, its weak points that therapies attempt to exploit, and we examine some of the experiments that got us to our current understanding. This information is difficult to find in an easily digestible form. Books are less likely to be found on these topics as it is not "mass market," and frankly are much more difficult to write. Hundreds of scientific papers have to be researched to keep up with developments. It should appeal to those with the curiosity of a cat, or the inquisitiveness of a scientist. The magic of several beautiful experiments is explained. As such, prostate cancer patients, scientist, engineers, and biotechnologists who want to get an overview of the science applied to this important health menace will benefit from the expedition. Advanced prostate cancer is a disease that must be fought, and fought hard. Like a military fight, its afflicted must become warriors against the disease. Part of the war chest of a warrior is intelligence (information). We need information to synthesize if we are to produce wisdom. And an abundance of wisdom (and luck) is needed to be successful in the fight. It is not just a patient's physicians that lead the prostate cancer journey that a prostate cancer warrior takes, but instead it is a partnership between patient and physician. The expedition for the reader results in providing current information about the science of the disease so that the prostate cancer warrior becomes a kind of Captain Clark, who works side by side with his co-expedition physician leader Captain Lewis to explore the forbidding landscape of this disease.

Book excerpt: Image-Guided Prostate Cancer Treatments is a comprehensive reference and practical guide on the technology and application of ultrasound and MRI in the male pelvis, with special attention to the prostate. The book is organized into three main sections, the first of which is devoted to general aspects of imaging and image-guided treatments. The second section provides a systematic overview of the application of ultrasound and MRI to the diagnosis and treatment of diseases of the lower urinary tract. Performance of the ultrasound and MRI studies is explained, and the normal and abnormal pathological anatomy is reviewed. Correlation with the ultrasound in the same plane is provided to assist in understanding the MRI sequences. Biopsy and interventional procedures, ultrasound-MRI fusion techniques, and image-guided therapies, including focused ultrasound, photodynamic therapy, microwave and laser ablation, are all fully covered. The third section focuses on securing treatment effectiveness and the use of follow-up imaging to ensure therapeutic success and detect tumor recurrence at an early stage, which is vital given that prompt focal treatment of recurrence is very successful. Here, particular attention is paid to the role of Doppler ultrasound and DCE-MRI technologies. This book, containing a wealth of high-quality illustrations based on high-end equipment, will acquaint beginners with the basics of prostate ultrasound and MRI, while more advanced practitioners will learn new skills, means of avoiding pitfalls, and ways of effectively relating the imaging and image-guided treatments to the clinical situation. The information provided will permit a tailored approach in dealing with specific pathologic issues.

Available in PDF, EPUB and Kindle. Book excerpt: This book offers a comprehensive evaluation of the use of stereotactic body radiosurgery (SBRT) for the treatment of prostate cancer. The rationale, selection criteria, and treatment planning for prostate SBRT are explained. Important imaging and anatomic considerations are discussed, and detailed consideration devoted to organ motion and tumor tracking during SBRT. Outcomes of therapy are then examined, with thorough appraisal of side effect profiles and quality of life impacts. Clear guidance is provided on how to deliver the therapy in a way that minimizes the risk of long-term urinary and rectal toxicities. Stereotactic radiosurgery for prostate cancer is an increasingly used form of treatment. Retrospective investigations have demonstrated the safe application of high-dose treatments, with 5-year results comparable to those achieved with protracted external beam radiotherapy. Prospective studies are underway comparing SBRT with more traditional forms of image-guided and intensity-modulated radiotherapy. In offering in-depth guidance on safe delivery of prostate SBRT, this book will be of value for students of radiation oncology, more experienced practitioners, and medical physicists.
