Les indicateurs sont des outils qualité permettant d'évaluer le niveau de performance et la qualité des prestations d'un organisme. Dans une démarche d'amélioration continue de la qualité, une réflexion a été initiée pour valider la pertinence des indicateurs du secteur de l'Assistance Pharmaceutique et en proposer de nouveaux. La première étape a consisté en l'analyse des lois, des référentiels normatifs et professionnels en vigueur dans le domaine de la pharmacie hospitalière en Suisse et en France. Une liste de 86 indicateurs a été créé à partir des indicateurs actuels, de la littérature et d'une enquête menée auprès de différents hôpitaux suisses. Les indicateurs transposables aux activités du secteur ont été évalués selon leur pertinence et leur mesurabilité. Nous avons sélectionné 34 indicateurs, 26 pour être mesurés systématiquement et 8 pour soutenir temporairement le développement de projet et négocier des moyens humains ou matériels. La méthodologie de cette étude a été construite afin d'éviter les freins rapportés dans la littérature. Pour améliorer la pertinence des indicateurs, il a été décidé de choisir leurs destinataires et s'adapter à leur point de vue. La participation de tous les pharmaciens a été sollicitée afin de faciliter leur mise en place. Les besoins spécifiques des pharmaciens ont été recueillis et une liste de 9 indicateurs contextuels pourra être ajoutée dans le logiciel de documentation des activités cliniques. La mise à jour des indicateurs de l'Assistance Pharmaceutique donne de la visibilité aux activités et peut faire évoluer la pratique au sein de l'établissement. L'utilisation de l'approche processus a permis de s'éloigner de l'approche par activité pour se concentrer sur la valeur ajoutée du pharmacien clinicien dans la prise en charge pluridisciplinaire du patient. Cette liste d'indicateurs élaborée à partir d'une revue de la littérature et des expériences rapportées par les hôpitaux suisses pourrait servir de base à une démarche nationale.

Book excerpt: La politique d'évaluation imposée aux établissements de santé par la loi du 31 juillet 1991, a pour objet de garantir « la qualité et l'efficience » de la prise en charge du patient. Il est en particulier indispensable d'obtenir une répartition des ressources selon les besoins des différents acteurs du système de santé, besoins qu'il est donc nécessaire d'évaluer précisément, nécessité renforcée par l'ordonnance du 24 avril 1996 et les contrats d'objectifs internes et externes. Dans ce contexte, il est indispensable pour les pharmaciens de créer un outil de comparaison externe ( « Benchmarking ») et interne des activités réalisées au sein d'une Pharmacie à Usage Intérieur et que cet outil soit un instrument de mesure représentatif de la quantité d'activités réalisées mais également de la qualité de ces activités. Nous avons tenté, par le biais d'une gestion par processus des activités exercées au sein d'une Pharmacie à Usage Intérieur, de mettre à disposition des pharmaciens du Département de Pharmacie du CHU de Grenoble, des tableaux de bord, véritables outils de pilotage, permettant une mesure quantitative et qualitative des activités pharmaceutiques hospitalières. Pour l'ensemble des neuf processus pharmaceutiques fondamentaux retenus, ont été élaborés des tableaux de bord rassemblant un nombre acceptable d'indicateurs d'activités, indicateurs sélectionnés à partir de listes exhaustives issues de la littérature française, au moyen d'un « Delphi modifié », par un échantillon représentatif des différents corps de métier impliqués au coeur de ces processus. Un consensus sur la pertinence d'un certain nombre de ces indicateurs a ainsi pu être établi, pertinence traduisant notamment la parfaite représentativité des activités qu'ils sont sensés mesurer, leur facilité de mise en place et d'interprétation par les acteurs du Département de Pharmacie qui seront amenés au final à les manipuler. Ces tableaux de bord doivent être finalisés, validés et instaurés officiellement au sein du Département de Pharmacie, avant d'envisager de tester ultérieurement leur comparabilité externe. Il restera également à envisager l'application de notre méthodologie à la discipline de la Pharmacie Clinique. Dans le contexte de financement de l'activité hospitalière, évoluant vers la tarification à l'activité, il est essentiel que la pharmacie, encore considérée comme un support logistique et non comme un service participant aux soins, puisse faire la preuve d'un service rendu efficient.

Les indicateurs quantitatifs sont également intéressants dans la mesure de la qualité car ils traduisent les moyens mis à disposition pour l’exercice de la pharmacie clinique. L’impact clinique est bien représentatif de la qualité du travail du pharmacien clinicien mais reste difficile à mesurer. Enfin les indicateurs de satisfaction permettent d’orienter les mesures à prendre pour améliorer la qualité future. Une fois un degré satisfaisant de qualité atteint, le maintien de ce niveau de qualité des activités de pharmacie clinique peut être contractualisé avec les services de soins afin de valoriser le travail réalisé et la profession de pharmacien clinicien. Le contrat établi précise les engagements et contraintes de chaque partie. Parmi les engagements de la pharmacie, le suivi d’indicateurs qualité est garant du maintien du niveau de qualité proposé dans l’exercice des missions du pharmacien clinicien. Le développement d’autres activités de pharmacie clinique, en collaboration avec le département d’information médical, pourrait être à l’origine de la construction d’autres indicateurs économiques valorisant les avantages financiers qui pourraient être réalisés grâce aux activités de pharmacie clinique. La construction d’indicateurs qualité en pharmacie clinique n’est donc qu’une étape dans l’amélioration continue de la prise en charge pharmaceutique des patients et dans la valorisation des activités de pharmacie clinique et de la profession de pharmacien hospitalier.

Actuellement, la traçabilité des activités de pharmacie clinique est souvent réalisée à l'aide de tableaux Excel® propres à chaque établissement. Au Centre Hospitalier de Valenciennes, une équipe de pharmaciens cliniciens a créé une application qui permet de tracer toutes les activités pharmaceutiques. L'objectif principal est d'identifier les indicateurs utilisés dans la littérature pour valoriser la pharmacie clinique et d'évaluer la possibilité de calculer ces indicateurs à partir de données structurées (issues de Valopharm ou d'autres. L'objectif secondaire est d'appliquer ces indicateurs aux données de traçabilité des pharmaciens cliniciens du pôle de Cancérologie et spécialités médicales. Méthode : dans un premier temps, une revue narrative de la littérature a été réalisée pour identifier les indicateurs les plus fréquemment mesurés lors d'études d'impact clinique et/ou médico-économique des activités de pharmacie clinique. Dans un second temps, les conseils et interventions pharmaceutiques tracés en 2019 et 2020 ont été analysés et certains indicateurs identifiés ont été calculés. Résultats : la revue de littérature a mis en évidence 70 indicateurs à travers 41 publications. Ces indicateurs ont été classés par type selon la catégorisation proposée par l'ANAES : 15 indicateurs d'activité, 51 indicateurs de résultat et 4 indicateurs de processus. Parmi ces indicateurs, 15 étaient directement calculables avec l'application de traçabilité, 18 à la faveur des données récupérables d'une autre source grâce à une clé commune, et 37 n'étaient pas calculables automatiquement. Un calcul d'économie de coût et d'évitement de coût à partir des indicateurs retrouvés dans la littérature a été simulé sur 3 interventions pharmaceutiques d'impact majeur. L'économie de coût en résultant variait d'une perte de 25 347 € à un gain de 12 388 €. L'évitement de coût était, lui, évalué entre 11 490 € et 81 848 €. Discussion-conclusion : le calcul d'indicateurs à partir de données de traçabilité est complexe. Pour trouver des indicateurs permettant une réelle valorisation de la pharmacie clinique, il est nécessaire de cibler des situations dans le cadre de sa propre pratique, en travaillant sur des données de bonne qualité.
