In the past, management of diabetes mellitus has revolved around the use of Self-Monitoring Blood Glucose devices such as fingersticks. Today, advancement in diabetes technology has led to the innovation of Continuous Glucose Monitoring (CGM) devices, which has drastically influence how diabetes is treated. The leaders of this DNP project hoped to bring awareness of CGM to young people with diabetes and provide resources that would assist them in making informed decisions about how to manage this illness. Because CGM use continues to be the lowest among young people (Tanenbaum et al., 2018), an educational program titled Level-Up Workshop was developed to improve how this population perceives and uses CGM devices. The Level-Up Workshop was a virtual educational workshop designed to provide education on CGM use to youth with T1DM receiving diabetes-related care from a pediatric clinic. Workshop activities were geared towards improving problem-solving, trouble-shooting, and social support. Two workshop sessions were implemented and during each session, participants engaged in a series of activies led by a diabetic educator, a CGM device representative, and young people with T1DM. Recruitment to the workshops was challenging, and a total of 4 young people were eventually enrolled. Both patients and staff participants were positive about the experience. Despite the low number of participants, the format of this project shows promise as a means of educating young people with diabetes.

Book excerpt: Background and Aim:American Diabetes Association (ADA) recommends that all people with diabetes (PWD) should participate in diabetes self-management education and support (DSMES)(1). Despite this fact, there are limited educational programs in the Islamic Republic of Iran for PWD(2) and access to structured DSMES is limited to capital. Horigan et al. reported logistical and financial reasons are among the reasons why patients referred to diabetes education programs, choose not to attend such programs.(3)GDEA has started a program to make DSMES available all over the country, named peer support integrated tele-education for people with T1DM (PSITE-1). Method: All children and adults aged 30 and younger with an established T1DM diagnosis were enrolled by their physician. The course was offered to the patients regardless of age, time from diagnosis or level of glycemic control. No literacy or numeracy requirements were specified to attend PSITE-1 program. Since many people are not familiar with videoconferencing, we identified a role as the local diabetes education instructor. He or she is a nurse in hospital or a secretary in a clinic who works with local pediatric endocrinologists. All participants were informed and registered by a local diabetes education instructor. During one week, they will attend three sessions, with one spare day between sessions. Each session was three hours long, making it nine accumulative hours of group videoconferencing with specific topics, homework and activities to be covered. This method of delivery was selected to allow participants to apply skills learnt on the class in their daily life and discuss their challenges with diabetes educator and peer supporters during the week.Patients would gather at the regional center and the senior diabetes educator trained at GDEA would have a real-time contact with the group through skype videoconferencing. Personal feedback was provided by diabetes educators during sessions. Peer support was also provided online by a messenger application called Telegram. All participants and their family members would be added to the class group and they would share their experiences with each other and the peer supporter would moderate the conversation and give personal feedback to each participant. Results: In the first year, we implemented PSITE-1 courses in five centers in five different provinces. During the second and third years, we added six and ten more centers in different cities, respectively. (Figure 1) During 107 courses, 2198 people with T1DM and 1373 family members have received DSMES through PSITE-1 project. Discussion: By implementing PSITE-1 project, we made structured DSMES available in multiple centers in different cities for people with T1DM, so distance would be no longer a barrier to access structured DSMES.In the era of technology, we believe PSITE-1 model would help us expand structured DSMES access across country. A well-designed research is necessary to find out whether PSTIE-1 is effective in providing clinical, psychological, behavioral and care coordination outcomes including cost reduction in short and long-term intervals.
