The use of ether for the relief of pain has become one of those twice told tales which it is a pleasure to recall and an honor to relate. The story itself, however, full as it is of human interest and dramatic elements, will not here be repeated, for it seems more to the purpose to consider some of the progress which in no small part was made possible by the introduction of the experimental method as exemplified in the discovery of ether and its application as an anesthetic. A Constructive Program. Formerly hygienic and sanitary measures were based almost solely upon a negative program. This was necessarily so because we had to await the exact observations of Pasteur before the sanitary sciences could be placed upon a sound basis of fact. The old books of hygiene were full of fads and fancies. By the old books I do not mean ancient tomes, but the books which were regarded as authority when I was a medical student. The whole sanitary system was then a vague or confused jumble and an uncertain guide. The system was an assemblage of sanitary dont's. We were told what not to do, in order to avoid infection, or to improve our bodily vigor. When the sanitarian of yesterday did depart from this negative program and ventured to give counsel of a positive character, it turned out to be mostly guesswork and very often wrong. When I was a boy they told me not to catch cold, but they could not tell me what to do in order not to catch cold, and if they did it was mostly poor advice. None of these contracted small-pox, but one of them did catch typhus. So, also, during the same year, the Medical Officer of Health of Gateshead reported that every nurse who has been more than a fortnight in the typhus wards has suffered from typhus; on the other hand, the only officers who took small-pox were two kitchen girls whom I neglected to vaccinate. This was a local, nu organized, and non-uniform movement, too often influenced by commercial motives and civic pride, but the seed of preventive medicine thus sown took root, and one of the first bills introduced into the First Congress of the United States provided for the assump tion of the quarantine power by the federal govern ment. But the time was not ripe, and we passed through a long period of state and local quarantine, and not a few devastating epidemics, before the citizen ship had evolved to the point where it would impose these duties on the general government. Even then we were unprepared for the step, and the short-lived national Board of Health came and went, but not before it had touched with prophetic finger the heart of the problem of the public health, the state and local boards of health. In a republic where the right to govern rests on the consent of the governed, where public senti ment very largely influences the enforcement of sanitary law, where people think before they obey, the comer stone of the whole public health fabric is the state board of health, and its efficiency determines in large measure the efficiency of the whole. Coincident with the development of the state boards came the improvement of the federal health agencies.
