This report evaluates four recent Air Force Major Defense Acquisition Programs (MDAPs) with low cost growth and compares and contrasts their key characteristics to the six programs evaluated with extreme cost growth from the earlier report. The purpose is threefold. First, we seek to determine whether or not the key characteristics identified in the programs with extreme cost growth are present in the programs with low cost growth and, if not, why. If those characteristics are not present, we assume that this finding reinforces our view that the key characteristics of the extreme cost-growth programs that were identified are likely the root causes of extreme cost growth. Second, we seek to determine the common characteristics of the low cost-growth programs and whether such characteristics can be incorporated into future Air Force MDAPs. Finally, we revisit the main recommendations from our earlier report regarding approaches to mitigating extreme cost growth and, based on our findings from the low cost-growth programs, determine whether those recommendations are still valid and broadly applicable to future Air Force MDAPs. Book excerpt: The U.S. Defense Department suffers from persistent, but not certain, cost growth within major acquisition programs. Over the past few decades, scores of empirical studies have examined the causes and consequences of cost growth and have shed light on characteristics of programs that fail to meet cost-performance goals. The question motivating the present study is: given the materiality of the problem (hundreds of billions of dollars), the number of times it has been studied, the attention of the highest levels of government, and numerous attempts to reform acquisition, why does the problem persist? What have the studies missed? Two recent and important deviations from the norm of past studies focused less on the acquisition program and more on the decisions made by officials about the program. This paper was motivated by those studies and was further informed by various literatures, including behavioral finance, group decision-making psychology, and organizational failure. The paper makes the case for the application of a framework taken from the worlds of political science and sociology to gain a better understanding of the problem of cost growth. The Institutional Analysis and Diagnosis (IAD) framework has been used to study the dynamics of complex decision-making processes involving collective resources. The data set is a collection of studies of defense acquisition cost growth from government organizations, academics, and think tanks. Using a software tool, the content of those reports is analyzed according to the IAD framework. The report then proposes a stream of research using the IAD framework -- in conjunction with theories of behavior finance, group decision-making, and organizational failure -- to improve our understanding of the dynamics and factors that result in cost growth. This framework has been successfully employed in other contexts to perform both qualitative field research and laboratory experimentation. (13 briefing charts are included).

All of these people have a vested interest in the cost of DoD programs and most would like to see those costs decrease; as such, we need additional tools to help combat cost growth. Previous research creates the foundation for the use of a two-step methodology to help predict cost growth, which we follow closely. First, utilizing logistic regression we analyze whether specific program characteristics predict cost growth within the Engineering and Manufacturing Development (EMD) phase for combined RDT&E and procurement budgets. The second step uses this answer (i.e., a positive response) to find cost growth predictor variables. Specifically, we perform a multiple regression analysis and determine the amount of cost growth incurred by these DoD programs. Through these two steps, we seek to unearth any predictive relationships within the data in order to build a predictive cost growth model. The final models predict whether a program will have cost growth and what the potential amount of the cost growth will be for the combined RDT&E and procurement budgets within the EMD phase of acquisition.

Total medical expenditures are rapidly approaching 20 percent of the gross domestic product and are crowding out other priorities of national importance. The use of increasingly expensive prescription drugs is a significant part of this problem, making the cost of biopharmaceuticals a serious national concern with broad political implications. Especially with the highly visible and very large price increases for prescription drugs that have occurred in recent years, finding a way to make prescription medicinesâ€"and health care at largeâ€"more affordable for everyone has become a socioeconomic imperative. Affordability is a complex function of factors, including not just the prices of the drugs themselves, but also the details of an individual's insurance coverage and the number of medical conditions that an individual or family confronts. Therefore, any solution to the affordability issue will require considering all of these factors together. The current high and increasing costs of prescription drugsâ€"coupled with the broader trends in overall health care costsâ€"is unsustainable to society as a whole. Making Medicines Affordable examines patient access to affordable and effective therapies, with emphasis on drug pricing, inflation in the cost of drugs, and insurance design. This report explores structural and policy factors influencing drug pricing, drug access programs, the emerging role of comparative effectiveness assessments in payment policies, changing finances of medical practice with regard to drug costs and reimbursement, and measures to prevent drug shortages and foster continued innovation in drug development. Book excerpt: This report is one of a series from a RAND Project AIR FORCE project, "The Cost of Future Military Aircraft: Historical Cost Estimating Relationships and Cost Reduction Initiatives." The purpose of the project is to improve the tools used to estimate the costs of future weapon systems. It focuses on how recent technical, management, and government policy changes affect cost. This report focuses on the accuracy of cost estimates. For our analysis, we used a very specific sample of Selected Acquisition Report (SAR) data, namely only programs that are complete or are nearly so. The analysis indicates a systematic bias toward underestimating the costs and substantial uncertainty in estimating the final cost of a weapon system. In contrast to the previous literature, the cost growth was higher than previously observed. We also found few correlations with cost growth, but observed that programs with longer duration had greater cost growth and electronics programs tended to have lower cost growth. Although there were some differences in the mean cost growth factors among the military departments, the differences were not statistically significant. While newer programs appear to have lower cost growth, this trend appears to be due to factors other than acquisition policies.
