Available in PDF, EPUB and Kindle. Book excerpt: Colorectal cancer (CRC) is the second most frequent cause of cancer-related death worldwide. Detecting CRC in a premalignant or early stage can improve survival of patients. When the CRC is metastasized (mCRC) to other organs patients are offered systemic treatment to prolong survival. However, not all patients respond to the available treatment while they do suffer from treatment related toxicities. In this thesis we studied the effects of CRC screening programmes on incidence and mortality. In addition, we studied tumor genomics and clinicopathological factors for the prediction of response to systemic therapy in patients with mCRC. The first chapter of this thesis described the effects of CRC screening on incidence and survival of patients with CRC. It was expected that screening for CRC would result in the detection of 1600 additional stage I and II CRCs per year in the first few years after its introduction in The Netherlands. This increase in detected stage I and II resulted in a decrease in the proportion of CRCs diagnosed at stages III or IV from 47% to 20%. Studies described in the next chapters aimed to improve treatment of patients with mCRC and to reduce unnecessary treatment related toxicity by patient selection for systemic therapy. Patient selection can be done with predictive biomarkers based on tumor genomics. A large number of genomic tumor characteristics can be analysed. This high dimensional data makes statistics challenging, as corrections for multiple testing are crucial to avoid overoptimistic results. Moreover, stable estimation of parameters is crucial for determining reproducible biomarkers. In chapter 2 we described a method with multi-parameter shrinkage options to overcome these statistical challenges. In chapter 3 we demonstrated that the miRNA expression profiles of metastases closely resemble that of their corresponding primary CRCs (pCRC).

They could lead to a better understanding of the causes of treatment failure and disease recurrence and thus to new possibilities for patient stratification. Liquid biopsy is currently an effective strategy to capture tumor heterogeneity in non-invasive monitoring of disease progression and recurrence.

It does not literally mean that a pharmaceutical company makes a drug for an individual patient for consumption and treatment but rather means the ability to stratify (or classify) individuals into sub-populations that differ in their responsiveness to a specific drug. However, the recognition that prognostic factors often have a greater impact on outcome than available therapies and the proliferation of biochemical, molecular, and genetic markers have resulted in renewed interest in this field. The outcome in patients with cancer is determined by a combination of numerous factors. Presently, the most widely recognized are the extent of disease, histologic type of tumor, and treatment. It has been known for some time that additional factors also influence outcome. These include histologic grade, lymphatic or vascular invasion, mitotic index, performance status, symptoms, and most recently genetic and biochemical markers. It is the aim of this volume to compile those prognostic factors that have emerged as important determinants of outcome for tumors at various sites. This compilation represents the first phase of a more extensive process to integrate all prognostic factors in cancer to further enhance the prediction of outcome following treatment. Certain issues surround ing the assessment and reporting of prognostic factors are also considered. Importance of Prognostic Factors Prognostic factors in cancer often have an immense influence on outcome, while treatment often has a much weaker effect. For example, the influence of the presence of lymph node involvement on survival of patients with metastatic breast cancer is much greater than the effect of adjuvant treatment with tamoxifen in the same group of patients [5].
