Available in PDF, EPUB and Kindle. Book excerpt: Prognosis and Outcomes in Surgical Disease is the definitive source for clinical results presented in a pragmatic, user-friendly way! In today's managed-care environment, it's becoming more and more common: Your patient wants the least traumatic therapy; however, his/her health plan wants the least expensive, and you want the one most likely to produce a favorable long-term outcome but lack the time and resources to research success rates for every available intervention. In the end, do you rely on your experience? and perhaps a hasty online search? to arrive at the right course of action, then call on your powers of persuasion to get everyone else on the same page. If this scenario sounds all too familiar, Prognosis and Outcomes in Surgical Disease is an indispensable reference that will not only help you save time and optimize outcomes, but also facilitate the informed consent process with surgical patients under your care. Edited by Drs. Daniel P. McKellar, Richard B. Reiling, and Ben Eiseman, this book is unique for both its comprehensive scope and diagrammatic approach. In a single, convenient volume, you gain immediate access to concise, algorithmic presentations of expected outcomes for the surgical treatment of more than 150 different conditions, covering 99 percent of all conditions a surgeon may encounter. Information that used to take hours to uncover is now at your fingertips and accessible at a glance. Furthermore, it is supplied by more than 150 world-renowned experts. Just open the text to the appropriate section, scan the algorithm to assess your options, and consult the shaded outcomes to weigh the pros and cons of the various treatments at your disposal. For easy reference, each disorder commands its own two-page spread, allowing you (and your patient) to compare and contrast therapies quickly, clearly, and objectively.

Available in PDF, EPUB and Kindle. Book excerpt: Emergency General Surgery has become a cornerstone of Acute Care Surgery over the past decade. Once the scope of community general surgeons, the growing complexity of patients with acute surgical diseases has increasingly driven their care to tertiary referral centers. The aging population confounds this problem further as these patients present with more complex comorbidities, life-threatening physiology, and progressively severe anatomic disease severity. To ensure better outcomes, the practice has evolved to focus on evidence-based practice management guidelines in order to standardize care and optimize outcomes. The purpose of this text is to be a comprehensive volume for patients with acute surgical diseases. There will be three sections, each comprised of multiple chapters. The first section will be a comprehensive analysis of the background of Emergency General Surgery. The goal of this section will be to provide the reader background into the acute surgical practice and to introduce the reader to the important concepts discussed in the remainder of the textbook. The second section will be the largest and will discuss specific acute surgical diseases. Individual chapters will outline the diagnostic approach, current treatment standards, operative approaches, and expected outcomes with an emphasis on practice management guideline implementation into practice. As acute surgical diseases and surgical critical care are integral to each other, the last section will be Critical Care for the Acute Care Surgeon. These topics will focus on the critical care concepts essential to surgeons who care for acute diseases. This comprehensive text is targeted to any surgeon who takes emergency call for any of the listed diseases including community and referral general and vascular surgeons. In addition, this would be an excellent resource for all general surgery residents. Lastly, this would also serve as a resource to intensivists who care for acute surgical patients.

Book excerpt: Infections that occur in the wound created by an invasive surgical procedure are generally referred to as surgical site infections (SSIs). SSIs are one of the most important causes of healthcare-associated infections (HCAIs). A prevalence survey undertaken in 2006 suggested that approximately 8% of patients in hospital in the UK have an HCAI. SSIs accounted for 14% of these infections and nearly 5% of patients who had undergone a surgical procedure were found to have developed an SSI. However, prevalence studies tend to underestimate SSI because many of these infections occur after the patient has been discharged from hospital. SSIs are associated with considerable morbidity and it has been reported that over one-third of postoperative deaths are related, at least in part, to SSI. However, it is important to recognise that SSIs can range from a relatively trivial wound discharge with no other complications to a life-threatening condition. Other clinical outcomes of SSIs include poor scars that are cosmetically unacceptable, such as those that are spreading, hypertrophic or keloid, persistent pain and itching, restriction of movement, particularly when over joints, and a significant impact on emotional wellbeing. SSI can double the length of time a patient stays in hospital and thereby increase the costs of health care. Additional costs attributable to SSI of between £814 and £6626 have been reported depending on the type of surgery and the severity of the infection. The main additional costs are related to re-operation, extra nursing care and interventions, and drug treatment costs. The indirect costs, due to loss of productivity, patient dissatisfaction and litigation, and reduced quality of life, have been studied less extensively.

Available in PDF, EPUB and Kindle. Book excerpt: Gastric cancer has been one of the great malignant scourges affecting man kind for as long as medical records have been kept. Until operative resection pioneered by Bilroth and others became available, no effective treatment was feasible and death from cancer was virtually inevitable. Even with resection by total gastrectomy, the chances of tumor eradication remained small. Over recent years, however, the situation has been changing. Some changes have resulted from better understanding of the disease, early detec tion, and better management techniques with applied clinical research, but the reasons for other changes are poorly understood. For example, the incidence of gastric cancer is decreasing, especially in westernized societies, where it has fallen from one of the most common cancers to no longer being in the top five causes of cancer death. Still it remains the number one killer of adult males in Japan and Korea. Whether the reduced incidence in western societies is a result of dietary changes or methods of food preservation, or some other reason, is as yet uncertain. Improvements in outcome have been reported from mass screening and early detection; more refined techniques of establishing early diagnosis, tumor type, and tumor extent; more radical surgical resection; and resection at earlier stages of disease.

Book excerpt: Crohn's disease is a chronic illness that afflicts millions of patients worldwide. Despite its increasing recognition as a medical specialty within gastroenterology, there has been little attention devoted to Crohn's from a surgical perspective. This volume fills that void and acts as the first catalogue to address the surgical management of Crohn's disease. The goal of this work was to provide surgeons with the latest medical and surgical information to manage patients with Crohn's disease effectively. In this book, contributions from both gastroenterologists describe the history, diagnosis, nutrition, medical management, and surveillance of Crohn's disease. Chapters from general and colorectal surgeons detail common clinical techniques, minimally invasive surgical approach and management of perianal disease. In addition, surgical complications specific to Crohn's disease are also reviewed. This book provides all of the essential information needed in the surgical management of Crohn's disease in one simple volume. It aims to be a useful resource for practicing surgeons and surgical residents as well as gastroenterologists. The authors wish readers to understand the complex decision making and sophisticated surgical techniques that these patients often demand.
