Pour cela, nous nous sommes appuyé sur le cas de Mr M., qui présente dans la phase aiguë de la maladie, une aphasie à prédominance réceptive dont il ne semble pas avoir conscience. Après plusieurs semaines de prise en charge au CMPR de Château Rauzé, il présente une amélioration simultanée de ses capacités langagières et de sa reconnaissance de son trouble. L'étude de son cas permet de mettre en avant des mécanismes physiologiques, cognitifs et psychologiques susceptibles d'expliquer cette évolution.

Cette revue systématique a recensé des données épidémiologiques et sémiologiques au sujet de l'aphasie du traumatisé crânien grave, afin d'établir les caractéristiques de ces troubles du langage, de dégager des recommandations de prise en charge et d'identifier les facteurs de récupération. L'aphasie traumatique est ainsi une aphasie à part entière, caractérisée par une réduction de l'expression et un déficit de la disponibilité lexicale avec manque du mot et anomie. Les tableaux traumatiques des aphasies anomiques et de Broca sont les plus fréquents. La gravité du tableau sémiologique, très varié est essentiellement due aux lésions axonales diffuses. Après l'accompagnement lors de la phase d'éveil, l'orthophoniste procède à la rééducation du langage. Les thérapies spécifiques à l'aphasie traumatique étant inexistantes à ce jour, les protocoles de l'aphasie vasculaire peuvent être utilisés, en l'absence de troubles cognitifs. Le cas échant, les situations PACE et/ou des outils de compensation peuvent être mis en place. La rééducation doit être précoce et très intensive. Les perspectives d'évolution des troubles du langage des patients traumatisés crâniens sont meilleures que celles des troubles du langage des patients vasculaires. La prise en charge des troubles phasiques est compliquée par la présence des troubles cognitifs et comportementaux. L'effet de l'âge et du coma sur la récupération de l'aphasie traumatique est controversé. Au contraire, le niveau d'études et la capacité de compréhension verbale initiale jouent un rôle majeur dans la récupération des troubles phasiques traumatiques.

Notre démarche s'inscrit dans le courant de la psycholinguistique cognitive qui envisage la compréhension de texte comme une construction de la signification associée à l'intégration des connaissances du lecteur. Ces remarques nous ont amenées à formuler l'hypothèse selon laquelle le patient traumatisé crânien présente une altération du traitement de haut niveau lors de la lecture de texte. Nous avons donc réalisé une étude statistique afin de comparer les performances d'un sujet traumatisé crânien à celles d'une population contrôle en situation de compréhension de texte. Puis, nous avons approfondi notre étude de cas afin de déterminer les procédures cognitives en cause lorsqu'il y a échec du traitement de l'information. Notre protocole nous a permis de mettre en évidence chez le sujet traumatisé crânien étudié : - une compréhension fine de texte altérée, en lien avec un déficit attentionnel et des capacités mnésiques à court terme dans les limites inférieures de la norme, - des difficultés pour construire et/ou exprimer sa pensée.
