Book excerpt: This interesting book offers an analysis of man-made catastrophes and asks why they continue to occur. 87 catastrophes or near-catastrophes, including high profile cases such as the Bhopal gas disaster, Grenfell Tower, Shoreham Air Show crash, Brumadinho dam collapse and Fukushima Daiichi, are described together with the reasons why they occurred and why over 50 different safety management approaches and techniques failed to prevent them. Featuring 63 eye opening stories from the author’s own personal experience and over 200 pitfalls in safety management approaches, this title is illustrated by 24 hypothetical cases in which the reader is asked to consider the approach they would take. Safety management techniques discussed include operating practices, personnel selection and emergency response. Safety management approaches including safety governance in organisations, along with the role of government and local authorities using the instruments of the law are extensively discussed. The work concludes with imaginative and creative ways forward with the aim to make considerable progress and to potentially eliminate man-made catastrophes for good. This title will be an ideal read for safety managers and engineers, community leaders in civic duties or labour union roles and professionals tasked with stopping and mitigating the impacts of man-made catastrophes, along with non-technical readers who are curious and concerned.

Book excerpt: Ross P. Cameron argues that the flow of time is a genuine feature of reality. He suggests that the best version of the A-Theory is a version of the Moving Spotlight view, according to which past and future beings are real, but there is nonetheless an objectively privileged present. Cameron argues that the Moving Spotlight theory should be viewed as having more in common with Presentism (the view that reality is limited to the present) than with the B-Theory (the view that time is just another dimension like space through which things are spread out). The Moving Spotlight view, on this picture, agrees with Presentism that everything is the way it is now, it simply thinks that non-present beings are amongst the things that are now some way. Cameron argues that the Moving Spotlight theory provides the best account of truthmakers for claims about what was or will be the case, and he defends the view against a number of objections, including McTaggart's argument that the A-Theory is inconsistent, and the charge that if the A-Theory is true but presentism false then we could not know that we are present. The Moving Spotlight defends an account of the open future--that what will happen is, as yet, undetermined--and argues that this is a better account than that available to the Growing Block theory.

Patient safety is a relatively new field of study, but the field is expanding and there is now better understanding of what is needed to measure and achieve safety for patients. The Handbook of Patient Safety will empower doctors, nurses and other professionals to be able to develop safe clinical processes that allow proactive management and minimisation of risk, so that people are not harmed when they receive clinical care. It gives the rationale for patient safety, the theories behind the science of patient safety and then the practical methods that frontline staff can use on a daily basis to decrease harm. Pocket sized and practical, this handbook is the ideal guide to support frontline staff and trainees, as well as all allied professionals in the name of patient safety. It reflects the World Health Organization's Patient Safety Curriculum and is written by international experts in their field who have specialist interests and direct expertise in dealing with patient safety issues. This book will demystify what is often seen as a complex topic, helping doctors understand the methods needed to provide safe care.

Available in PDF, EPUB and Kindle. Book excerpt: Even for violent crime, justice should mean more than punishment. By paying close attention to the relational harms suffered by victims, this book develops a concept of relational justice for survivors, offenders and community. Relational justice looks beyond traditional rules of legal responsibility to include the social and emotional dimensions of human experience, opening the way for a more compassionate, effective and just response to crime. The book’s chapters follow a journey from victim experiences of violence to community healing from violence. Early chapters examine the relational harms inflicted by the worst wrongs, the moral responsibility of wrongdoers and common mistakes made in judging wrongdoing. Particular attention is paid here to sexual violence. The book then moves to questions of just punishment: proper sentencing by judges, mandatory sentences approved by the public, and the realities of contemporary incarceration, focusing particularly on solitary confinement and sexual violence. In its remaining chapters, the book looks at changes brought by the victims' rights movement and victim needs that current law does not, and perhaps cannot meet. It then addresses possibilities for offender change and challenges for majority America in addressing race discrimination in criminal justice. The book concludes with a look at how individuals might live out the ideals of a greater—relational—justice. Available in PDF, EPUB and Kindle. Book excerpt: Graft is a common and persistent social pathogen that afflicts the developed and developing world in equal measure. This book describes, through the medium of international case studies, how graft undermines public safety and how, following a near-miss, incident or accident, investigators can use actor-network theory (ANT) to ascertain to what degree and through what mechanisms graft contributed to the event. The book introduces the reader to graft through a variety of case studies and explains how graft works against the public interest. The relatable case studies include the 1989 Hillsborough football stadium disaster, 2007 Adam Air crash, 2015-ongoing Volkswagen diesel emissions scandal and 2020 Beirut ammonium nitrate explosion (Lebanon). It demonstrates the threat graft poses to public safety, economic success and corporate and national reputation. By the end of the book the reader will understand the nature and extent of the problem of graft, how graft undermines safety, confidence and reputation, and how ANT can be used to identify and quantify graft in respect of the governance of technological systems and to ascertain to what degree and through what mechanisms graft contributed to a near-miss, incident or accident. Primarily aimed at an academic audience, this book will offer essential insights to students, researchers and faculty within the fields of risk, crisis and disaster management, as well as corporate governance and safety. The accessible nature of the book will also appeal to safety practitioners, risk managers and accident investigators.
