Yet 75-80% of trainees, health workers, and faculty working in global health are female. This gender disparity hinders efforts to improve health outcomes, particularly for women and children, worldwide. We conducted a multi-country study to evaluate these gender-based challenges experienced by female health trainees and professionals, and to identify data-driven solutions to promote their retention and rise to leadership roles. Methods: We conducted a mixed-methods study utilizing surveys, in-depth interviews, and focus group discussions at four academic health centers in Haiti, India, Tanzania and the United States. Based on prior formative research, we collected data on three barriers: work-life balance, gender discrimination, and sexual harassment/assault. We additionally sought womenu2019s input for solutions to address these barriers. We used a mixed model with site as a random effect to determine the mean of the means across sites for each barrier. We performed interpretive phenomenological analysis of qualitative data using a thematic survey. Results: Across institutions, 346 women completed surveys, and 85 participated in qualitative discussions. Overall, 291 (84%) women reported at least one challenge: 72% with work-life balance (95% CI: 44-90), 46% with gender discrimination (95% CI: 37-56), and 38% with sexual harassment/assault (95% CI: 25-53). Qualitative discussions revealed major themes: fear, futility, and resignation. Women expressed resigned acceptance about handling work and disproportionate household responsibilities, declined leadership opportunities due to family obligations, and described the lack of safe and unbiased systems for reporting harassment/assault. To address these barriers, women recommended public promotion of gender equality, formal discrimination and harassment policies with clear reporting methods, peer-mentorship groups, and gender-specific leadership training.Conclusion: At all sites, the high prevalence of discrimination and sexual harassment/assault, as well as of other gender-specific barriers to career advancement was alarming. Low-cost, practical solutions may have major impact. Gender balance helps institutions better reflect the populations they serve, and improves outcomes. Building on solutions suggested by women themselves, we must commit to providing a safe environment for everyone or risk losing a generation of female health workers who play a crucial role in improving health globally.
