Book excerpt: The pioneering work by Pierre Janny and Nils Lundberg added a new tool to the investigative armamentarium of the neurosurgeon: continuous monitor ing of intracranial pressure (ICP). A small group of basic and clinical sci entists recognized the importance of this method and in 1972 an International Symposium was organized by Mario Brock in Hannover, Germany. The workers "of the first hour" had high expectations and thought that the problem of intracranial hypertension could finally be dealt with. The Symposium con tinued to grow during subsequent meetings in Lund (1974), Groningen (1976) and Williamsburg (1979). However, the advent of the modern imaging tech niques of CT and MRI have dramatically changed the scene in clinical neurology and neurosurgery. These new methods as well as other monitoring techniques have put ICP in its proper place as just one of the many aspects, albeit an important one, in the management of patients with intracranial space-occupying lesions. The symposia in Tokyo (1982), Glasgow (1985) and Ann Arbor (1988) confirmed the steady interest in ICP recording, both as a research tool and as a clinical method. and yielded many new developments in the field of ICP. This book comprises the proceedings of the Eighth International Symposium on Intracranial Pressure that was held in Rotterdam, The Netherlands, from June 16th to 20th, 1991.

Book excerpt: In the last few decades an abundance of publications have accumu lated on the clinical implications of subarachnoid haemorrhage (SAH) due to aneurysmal rupture. In contrast, until about 6 years ago, SAH due to traumatic head injury mainly, if not only, drew the attention of the forensic pathologist. In their analysis of the data from the American Traumatic Coma Data Bank, Eisenberg et al. concluded in 1990 that the presence of SAH on the initial computed tomographic scan (tSAH) had an un favourable effect on outcome. This conclusion has since been con firmed in other series. Dr. Kakarieka, the author of the present monograph, has been fascinated by tSAH since his participation in 1989 as International Trial Coordinator of the large European Trial on the effect of nimodipine on outcome after severe injury (HIT 2). This study re vealed a statistically significant, favourable effect in the subgroup of patients with tSAH, a result which warranted a further trial on the efficacy of nimodipine administration in patients with tSAH. This trial, the so-called German trial which was completed in 1995, not only revealed that tSAH is an important and independent factor predicting unfavourable outcome in head injury, but also showed convincingly the statistically significant, favourable effect of nimodipine adminis tration in patients with tSAH.

These giant aneurysms present many therapeutic difficulties and, because of their unique anatomical features and size, may present in a multitude of ways. With the advent of specialized imaging techniques such as computed tomography (CT), mag netic resonance imaging (MRI) and selective angiography, preoperative diag nosis today is most often accomplished without difficulty. However, com pletely thrombosed giant aneurysms may mimic other lesions with mass effect (such as basilar meningiomas, chordomas or chondromas) and their true anatomical shapes and relations to other cranial structures can only be ascer tained by direct operative inspection. Due to their morphological features (thrombosed, nonthrombosed, par tially thrombosed, fusiform), anatomical variations and difficult locations, giant aneurysms present new challenges for the modern neurosurgeon. Al though microsurgical techniques have rendered direct surgical treatment of giant intracranial aneurysms safer, elimination of the aneurysm without dis turbing the hemodynamics continues to be problematic. Some of these lesions have relatively small necks and can therefore be clipped fairly easily. Others have large necks, are fusiform, or contain perforators; how best to treat these lesions is a question still unresolved by presentday neurosurgery.
