Book excerpt: The purpose of this critical qualitative study was to understand how transition to adulthood for young people with developmental disabilities (DD) is constituted as a 'problem' in policies and practices across sectors in Ontario and the implications for these young people and their parents. This transition to adulthood has been identified as a policy problem in three provincial government sectors in Ontario, Canada-rehabilitation, education, and developmental services. The problem is predominantly framed as a service transition issue, particularly when young people 'age out' of pediatric health services by age 19 and public education by age 21, and seek adult-oriented programs and services. Transition policies have been developed in each sector, shaped by both explicit and implicit understandings of key concepts, such as disability and adulthood. These understandings function to constitute transition as a particular kind of problem, and play a key role in what is considered and proposed to address it, but also what is not considered or potentially neglected or even ignored. What is and is not considered has implications for the health and daily lives of young disabled people and their families. Guided by a problem-questioning approach to policy analysis proposed by Carol Bacchi, I used a multimethod design, including analysis of three policy documents and in-depth interviews with 13 parents, to examine how transition to adulthood is constituted as a problem. My analysis revealed that normative assumptions about ways of being, becoming, and acting as an adult shaped an implied problem of disabled children and their inadequate progression to socially valued adult roles and activities. Policies shaped by these assumptions had both positive (e.g., feelings of self-worth for achieving or approximating socially valued roles and activities) and unintended negative effects (e.g., social exclusion, stress or anxiety when adult roles were not achieved) on young disabled people and their parents. These findings highlight opportunities for rethinking the policy problem in ways that can mitigate unintended harmful consequences on young people with DD and their families and for improving their health and daily life circumstances through healthy public policy and cross-sector coordination.

In particular disabled children or children with unusual developmental patterns can find themselves surrounded by multiple practices through which they are examined. This rich book draws on a wide range of qualitative research to look at how disabled children have been cared for, treated and categorised. Narrative and longitudinal interviews with children and their families, along with stories and images they have produced and notes from observations of different spaces in their lives – medical consultation rooms, cafes and leisure centres, homes, classrooms and playgrounds amongst others – all make a contribution. Bringing this wealth of empirical data together with conceptual ideas from disability studies, sociology of the body, childhood studies, symbolic interactionism and feminist critical theory, the authors explore the multiple ways in which monitoring occurs within childhood disability and its social effects. Their discussion includes examining the dynamics of differentiation via medicine, social interaction, and embodiment and the multiple actors – including children and young people themselves – involved. A comprehensive overview of post-school outcomes, transition practices, special education law, social constructions of adulthood and disability, and parent/teacher perspectives on adulthood for students with profound disabilities is provided. Using data collected from interviews with eight parent and teacher pairs, special education artifacts, and community resources, this study uncovers the similarities and differences in parent and teacher thinking regarding a student's adult life; the factors that influence the content of student Individual Education Plans (IEPs) and Individual Transition Plans (ITPs); and the role of culture, context and artifacts in the parent, teacher, and institutional formulations of adult life for this student population. This research provides new information about the factors that influence the formulations of the adult lives of students with profound disabilities. These factors included community and institutional perceptions of adulthood; the role of context in thinking and decision making; the influence of social and cultural expectations; and the disparity in the mental models that parents and teachers hold regarding their children/students with profound disability. The study reveals that parents, teachers, and institutions have different perspectives regarding the adult lives of individuals with profound disabilities. Parent thinking is focused on securing a fulfilling and nurturing future for their child and teacher thinking on active participation in a range of activities in institutional settings, while institutional formulations of adulthood revolve around work and independence. Parents are better able to envision an adult life for their child when provided life planning support. Teachers are more concerned with disability label than parents, and are more focused on the role of institutional settings in a student's adult life. IEP and ITP goals are more consistent with teacher perspectives regarding a student's adult future than with parent or institutional perspectives. Context emerged as one of the most significant factors that influenced parent and teacher thinking regarding a child/student's life after graduation. This study supports the critical need for establishing model transition programs and providing education to parents and teachers regarding the options available to individuals with profound disabilities when they reach adulthood. Implications for parents, teachers, and institutions are reviewed and recommendations for further research are provided.
