Book excerpt: L'EPP est un entretien lors du suivi de grossesse très peu réalisé par les médecins généralistes et dont l'intérêt a déjà été souligné dans plusieurs études. Cet entretien correspond aux objectifs de la médecine générale. Objectif : Le but de l'étude est de réaliser une évaluation qualitative portant sur le vécu pour les femmes de la prise en charge psycho-sociale dans l'EPP lorsqu'il est réalisé par un médecin généraliste, une sage-femme ou un gynécologue libéral. IL s'agit également d'évaluer la relation avec le professionnel suite à l'EPP. Résultats : les entretiens ont été réalisés entre octobre 2017 et février 2018 au près de femmes en période de post-partum. Les résultats témoignent des multiples thèmes d'ordre psychologique et sociale abordés par les femmes. L'ouverture à cette dimension semble avoir un impact sur le vécu de la grossesse et du post-partum par l'appropriation de certains enjeux, la réflexion du couple et de la femme. Enfin, la relation au soignant ayant réalisé l'EPP est désigné par les femmes comme une relation de confiance, de disponibilité et d'écoute. Conclusion : L'EPP est l'occasion d'ouvrir le dialogue avec la femme et d'enclencher une réflexion sur le vécu de la grossesse et de la parentalité. Il se présente comme un outil de prévention et d'humanisation. Il demande au professionnel une disponibilité et une écoute différente de la consultation médicale exploratrice. Il s'agit d'accompagner le patient dans ce processus de transformation qu'est la grossesse et la parentalité.

Book excerpt: Le vécu d'une première grossesse engendre différentes modifications et bouleversements, aussi bien sur le plan physique que psychique. Nous avons réalisé une étude permettant d'évaluer en quoi le vécu d'une première grossesse entraîne la quête d'un réaménagement identitaire chez la primipare, avec pour objectif d'ouvrir une piste de réflexion sur l'ensemble des phénomènes mis en jeu au cours d'une première grossesse et ainsi permettre aux professionnels d'être en mesure d'accompagner au mieux les femmes. Une étude transversale descriptive et multicentrique a été réalisée dans ce sens. Le recueil de données a été effectué à partir de 155 questionnaires distribués aux primipares dans les services de suites de couches physiologiques de trois maternités. L'étude montre que la perception de soi est fortement modifiée pour une majorité de femmes durant la grossesse, et que cette période est marquée par une transparence psychique traduisant un sentiment de vulnérabilité plus prégnant. Dans ce cadre, une grande majorité des femmes est en attente de divers types de soutiens afin de vivre au mieux cette période de transition vers la maternité. Le corps médical revêt une importance majeure auprès des femmes et constitue un réel soutien pour celles-ci. La création de l'Entretien Prénatal Précoce apparaît comme une mesure essentielle permettant d'introduire la dimension psychosociale au sein du suivi de grossesse. Cette mesure serait cependant à consolider afin qu'une généralisation de sa proposition soit mise en place par tous les professionnels de la périnatalité, et que toutes les femmes le désirant puissent en bénéficier.

Trente-trois pour cent des médecins interrogés connaissent l'existence de l'EPP. 10,81% le proposent systématiquement contre 78,77% qui ne le proposent jamais, principalement par méconnaissance de cet entretien. Huit pour cent des médecins interrogés réalisent eux-mêmes l'entretien. Méconnu par les médecins généralistes, l'EPP n'est pas adapté à la l'exercice de la médecine générale puisqu'il nécessite une organisation particulière et l'appartenance à un réseau de périnatalité. Malgré cela, les médecins généralistes réalisent un EPP officieux tout au long des consultations de suivi de grossesse, le contenu de l'EPP étant abordé au fur et à mesure de la prise en charge. La place accordée aux futurs pères par le médecin généraliste lors des consultations de suivi de grossesse pourrait être explorée par une autre étude.

Book excerpt: This book is devoted to the description of agenesis of the corpus callosum, a congenital malformation of midline structures in the brain that may be regarded as a natural model of the "split-brain. " First reported by Rei! in 1812, this anomaly has since been described by several investigators. Interest in this malformation was revived in the 1970s by studies of interhemispheric transfer in Bogen and Vogel's commissurotomized patients and the subsequent findings by Sperry that acallosal patients were devoid of the typical disconnection deficits found in patients with surgical transection of the corpus callosum. Since this seminal work, the bulk of neuropsychological research on callosal agenesis has focused on the particulars of interhemispheric transfer and integration. An ever-growing literature has emerged on the subject, attempting to specify the extent and limits of neural plasticity in a nervous system that has evolved in the absence of the most important interhemispheric pathway. Whilst callosal agenesis proves to be an excellent model of cerebral plasticity, it has to be pointed out that this anomaly is often associated with other malformations and neurological diseases that may result in different degrees of mental retardation or other cognitive and sensorimotor deficits. In this context, neurological research on callosal agenesis has concentrated on the description of various syndromes associated with this pathology as well as on the attempt to specify its neurobehavioral manifestations.
