Book excerpt: Introduction : L'incidence des états septiques graves est en constante progression dans le monde avec une mortalité élevée. La rapidité de leur prise en charge constitue un élément pronostique majeur. Nous avons étudié la prise en charge des états septiques graves aux urgences, son délai, ainsi que les facteurs associés à la mortalité à 28 jours. Méthodes : Etude épidémiologique prospective sur une durée de 6 mois, au service d'accueil des urgences adultes du CHU d'Angers. Résultats : Sur 88 dossiers analysés, le délai de reconnaissance du sepsis sévère était de 3h51min. L 'expansion volémique moyenne dans les 3 heures suivant la reconnaissance du sepsis était de 1 269 mL. L'initiation de l'antibiothérapie était réalisée dans l'heure dans 61,3 % des cas. La posologie des antibiotiques n'était pas conforme dans 23,5 % des cas. Le taux de mortalité était de 28,4 %. La surveillance de la diurèse et de la fréquence respiratoire était insuffisante. Discussion : Faible puissance de l'étude. Insister auprès des équipes pour la surveillance de la fréquence respiratoire et de la diurèse horaire. Nécessité de systématiser les schémas thérapeutiques pour l'antibiothérapie et l'expansion volémique en surveillant son efficacité clinique, notamment par la diurèse horaire. Le lactate capillaire est à évaluer à l'accueil des urgences, afin d'essayer de diminuer le délai de reconnaissance du sepsis sévère, influant sur le délai de prise en charge. Conclusion : La prise en charge thérapeutique optimale de l'état septique grave, enjeu majeur de santé publique, rend nécessaire d'améliorer le délai de sa reconnaissance au sein du service des urgences, et ce, dès l'admission.
