Tout cela conduit à une augmentation du nombre de femmes enceintes atteintes de diabète de type 2.La grossesse chez la femme diabétique de type 2 est une grossesse à haut risque en raison des complications maternelles et fœtales qui lui sont inhérentes. Ces grossesses doivent donc être strictement encadrées et préparées afin d'assurer le bon déroulement de celles-ci. Ce travail est à la foisbibliographique et expérimental. Lapartieexpérimentale apour but de rapporterl'expérience de la Maternité Régionale de Nancy où sont prises en charge la majorité des grossesses diabétiques de Lorraine. Pour cela, la grossesse menée par 34 patientes diabétiques de type 2 entre 2006 et 2012 a été étudiée.Nous avons souhaité faire un état des lieux de la prise en charge et de l'issue de ces grossesses. Nous nous sommes également intéressés au traitement hypoglycémiant utilisé par ces patientes avant la grossesse ainsi qu'à son évolution au cours de la grossesse et après l'accouchement.Concernant la prise en charge de la grossesse, nos résultats sont en accord avec les données actuelles à ce sujet : seulement20,6 % des patientes ont bénéficié d'uneprise en charge pré-conceptionnelle, ce qui est très insuffisant. Hélas,certaines complications maternelles et fœtales en sont la conséquence : progression des rétinopathies diabétiques,néphropathies diabétiques et apparition de malformations fœtales par exemple. L'étude del'évolution du traitement antidiabétique avant, pendant et après la grossesse, a confirméun abandon desantidiabétiques orauxchez toutes les patientespendant la période gestationnelle.Un traitementpar insuline ou par mesures hygiéno-diététiques a alors été mis en place.Au fil des mois de grossesse, afin de compenser les modifications métaboliques maternelles,une complexification des schémas insuliniqueset une réévaluation quotidienne des doses d'insulineontété nécessaires.Que cela concerne la prévention, l'information oul'éducation,nous verrons quelle estla place du pharmacien d'officine dans cette prise en charge pluridisciplinaire.

Book excerpt: Objectif: Dresser un état des lieux du déroulement et de l'issue des grossesses chez les femmes diabétiques de type 1, au regard de l'équilibre glycémique préconceptionnel et de son amélioration éventuel pendant la grossesse. Patientes et méthode : Etude rétrospective portant sur une cohorte de 117 grossesses, menées chez 85 femmes, suivies à la Maternité Régionale de Nancy entre 2000 et 2007. Résultats : Les 117 grossesses ont abouti à la naissance de 100 enfants. 19 grossesses se sont arrêtées prématurément. 42% des grossesses étaient programmées. Une atteinte vasculaire obstétricale complique 22.2% des grossesses. La mortalité périnatale (4.3%), les malformations congénitales sévères (3.9%) et la prématurité (37%) demeurent majorées. Une HbAlc préconceptionnelle inférieure à 7%.est associée à une réduction significative de la prématurité. L'obtention de cette valeur d'HbAlc est liée à la programmation de la grossesse. Par contre, l'amélioration de l'équilibre glycémique pendant la grossesse ne réduit pas le taux de complications. Conclusion : 20 ans après la déclaration de Saint Vincent, les complications des grossesses chez les femmes diabétiques de type 1 sont toujours majorées et les grossesses insuffisamment programmées. Ce point est d'autant plus important que les répercussions du déséquilibre initial semblent irrécupérables.

Cette situation est fréquente à la Réunion qui se caractérise par une prévalence record du diabète de type 2 et un taux de natalité plus élevé. Après une revue des connaissances sur le sujet, nous avons partagé l'expérience du GHSR à travers une étude rétrospective évaluant la prise en charge et l'issue de 165 grossesses suivies de 2005 à 2010. Dans notre étude, les patientes cumulent de nombreux facteurs de risque tels l'âge maternel élevé, l'hypertension, la dyslipidémie, l'obésité. Les grossesses sont rarement programmées (4 %) et prises en charge tardivement (11,2 SA) avec un équilibre glycémique préconceptionnel médiocre (HbA1C à 7,3 %). Nous déplorons un taux de mortalité périnatale 2 fois plus élevé que dans la population générale (38,4 ‰ vs 15,4 ‰) et un risque accru de malformations congénitales.

Actuellement, 0,4% à 1,5% des femmes enceintes sont diabétiques de type 1 ou de type 2. Par ailleurs, 3 à 6% des grossesses sont compliquées d’un diabète gestationnel, trouble de la tolérance glucidique qui apparait pendant la grossesse et qui disparait après l’accouchement. Grossesse et diabète forment une situation métabolique à risque, tant pour la mère que pour l’enfant à naître. L’hyperglycémie est pathogène pour le fœtus: le risque de malformations, de macrosomie et de complications obstétricales est accru. Le statut hormonal particulier peut également être néfaste pour la mère et déséquilibrer un diabète pré-existant. Une prise en charge multidisciplinaire et un suivi diabétologique rigoureux permettent de réduire de manière significative le risque de complications. Très récemment, le Collège National des Gynécologues Obstétriciens de France a élaboré de nouvelles recommandations sur le dépistage, le diagnostic et la prise en charge du diabète gestationnel: c’est la référence actuelle pour les praticiens.

Une prise en charge adaptée et rigoureuse s'avère nécessaire afin de limiter les risques materno-fœtaux. Une corrélation est en effet désormais établie entre l'hyperglycémie maternelle et le risque de survenue de complications pour la mère et son enfant. Dans un premier temps, des mesures hygiéno-diététiques sont instaurées afin de tenter de normaliser les glycémies. En cas de non atteinte des objectifs, une insulinothérapie sera envisagée dans un second temps. Nous avons réalisé une étude observationnelle à la Maternité Régionale Universitaire de Nancy, portant sur le protocole d'insulinothérapie par analogue rapide, dans le diabète gestationnel, mis en place depuis 2002 au sein de l'établissement. La singularité du protocole réside dans l'instauration d'un délai de 30 ou 45 minutes entre l'injection de l'analogue rapide et la prise alimentaire. Le traitement doit tenir compte des particularités physiopathologiques de la grossesse et notamment de l'insulinorésistance croissante. Le délai établi permettrait de lutter contre cette sensibilité diminuée à l'insuline.Dans l'étude, nous nous sommes intéressés à deux paramètres: une diminution des glycémies maternelles après mise sous insuline et la survenue éventuelle d'hypoglycémies précoces ou tardives, constituant un frein potentiel à la régulation glycémique.

Since the United Nations General Assembly held in New York in 2011, many publications have emphasized the close link between NCDs and nutrition. The NCDs epidemic forces us to reconsider the public health perspectives. Many governments, non-governmental organizations and other institutions are actively involved in educational nutrition programs and campaigns; however their efforts seldom obtain the results hoped for. It is extremely difficult to induce changes in lifestyle and behavior that have built up over a long period of time. However, it becomes urgent to adapt to our changing life-environment where traditional wisdom and intuitive choices are giving way to individual thinking and search for (often uncontrolled) information. This engenders a number of unprecedented challenges and it calls for a re-appraisal of the existing paradigms to achieve an adequate management of the upstream determinants of health instead of a (pre)dominant medical and hospital-centric approach. In the era of personalized healthcare, it is time to empower policy makers, professionals and citizens for achieving an evidence-based change in the health-disease interface and decision-making process for public health interventions. The scientific and professional society Health Technology Assessment International (HTAi) has recognized nutrition as a health technology (preventative) by creating a Interest Group (IG) dedicated to research on methodologies and assessments of nutrition-related public health, while taking into account contextual factors (ethical, legal, social, organizational, economic, …) in order to generate meaningful outcomes for establishing evidence-based health policies. This Research Topic aims to elaborate on some of the potential hurdles or guarantees which have to be overcome for the sake of sustainable healthcare provisions anywhere in the world, such as shortcomings in methodological approaches, regulatory frameworks, gaps between evidence, its hierarchy and final recommendations for public health management.
