Book Synopsis Preventing Industrial Accidents by : Carsten Busch

Available in PDF, EPUB and Kindle. Book excerpt: Herbert William Heinrich has been one of the most influential safety pioneers. His work from the 1930s/1940s affects much of what is done in safety today – for better and worse. Heinrich’s work is debated and heavily critiqued by some, while others defend it with zeal. Interestingly, few people who discuss the ideas have ever read his work or looked into its backgrounds; most do so based on hearsay, secondary sources, or mere opinion. One reason for this is that Heinrich’s work has been out of print for decades: it is notoriously hard to find, and quality biographical information is hard to get. Based on some serious "safety archaeology," which provided access to many of Heinrich’s original papers, books, and rather rich biographical information, this book aims to fill this gap. It deals with the life and work of Heinrich, the context he worked in, and his influences and legacy. The book defines the main themes in Heinrich’s work and discusses them, paying attention to their origins, the developments that came from them, interpretations and attributions, and the critiques that they may have attracted over the years. This includes such well-known ideas and metaphor as the accident triangle, the accident sequence (dominoes), the hidden cost of accidents, the human element, and management responsibility. This book is the first to deal with the work and legacy of Heinrich as a whole, based on a unique richness of material and approaching the matter from several (new) angles. It also reflects on Heinrich’s relevance for today’s safety science and practice.

Book Synopsis Rethinking Bhopal by : Kenneth Bloch
Download or read book Rethinking Bhopal written by Kenneth Bloch and published by Elsevier. This book was released on 2016-05-27 with total page 510 pages. Available in PDF, EPUB and Kindle. Book excerpt: Rethinking Bhopal: A Definitive Guide to Investigating, Preventing, and Learning from Industrial Disasters is the go-to source for anyone seeking to learn how to improve process safety management (PSM) through applying fundamental asset reliability and incident investigation concepts. The seeds that unified PSM on a global scale were planted in Bhopal, India on December 3, 1984. Since then, considerable progress has been made to protect both workers and communities from catastrophic industrial failures. Industry acknowledges its responsibility to create value with accrued operating experience and that using information received from previous failures is a direct way to prevent future incidents. With this principle in mind, Bloch evaluates modern references related to the Bhopal Disaster, using recognized industrial asset reliability and incident investigation concepts. The practice of objective incident investigation offers a compelling insight into specific decisions and actions that resulted in history’s worst industrial disaster. Recording a fully transparent sequence of events promotes a personal sense of accountability for anyone involved in the manufacturing industry. Lessons learned can be immediately implemented by those with direct PSM, management, engineering, and operating responsibilities. Case histories demonstrate how patterns observed in the timeline leading up to the Bhopal Disaster can be detected in modern incidents and by recognizing these patterns in present-day processes avoids counterproductive operating decisions and unprecedented destruction. This text is instrumental in helping existing organizations re-evaluate their own exposures and risks, and would be a valuable read for any member of a process safety management team. Rethinking Bhopal: A Definitive Guide to Investigating, Preventing, and Learning from Industrial Disasters provides an expansion of knowledge and understanding for the novice in PSM while also providing depth and application considerations to challenge more experienced industry professionals. Note: All royalties from this book go to the Process Safety Heritage Trust Scholarship at Lamar University in Beaumont, Texas, USA. Learn how to improve Process Safety Management (PSM) performance by applying fundamental asset reliability and incident investigation concepts Understand your personal role in detecting and preventing Loss of Primary Containment (LOPC) incidents before they occur Take immediate action to stabilize processes under your control while promoting a systematic approach to eliminating persistent failure mechanisms Includes case histories to helpfully illustrate how to detect potentially destructive patterns in your own organization
Book Synopsis Lessons from Disaster by : Trevor A. Kletz
Download or read book Lessons from Disaster written by Trevor A. Kletz and published by IChemE. This book was released on 1993 with total page 196 pages. Available in PDF, EPUB and Kindle. Book excerpt: This series of essays on safety and loss prevention is aimed at helping the process industries avoid accidents and improve its public image. The central message is the apparent inability of organizations to learn, and retain in the long term, the lessons drawn from accidents. Thus incidents of a similar type recur within the same company at intervals of a decade or so, as personnel involved move on to other jobs.

Book Synopsis Risk Assessment for Industrial Accident Prevention by : United Nations
Download or read book Risk Assessment for Industrial Accident Prevention written by United Nations and published by UN. This book was released on 2024-03-26 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Risk assessments for industrial facilities are essential for the prevention of industrial accidents. The United Nations Economic Commission for Europe (UNECE) Convention on the Transboundary Effects of Industrial Accidents aims to help its Parties prevent, prepare for, and respond to industrial accidents, especially ones that can have transboundary effects. It also fosters transboundary cooperation among its Parties and beyond. As risk assessment is enshrined in the Convention's provisions, UNECE held a seminar on risk assessment methodologies (Geneva, 4 December 2018) to support countries in implementing the relevant provisions. The seminar resulted in conclusions and recommendations on the challenges in executing transboundary risk assessment for industrial facilities and the need for more information exchange on risk assessment methodologies used in the UNECE region, including available software.
