Book excerpt: Dans les pays développés, la transmission mère-enfant du VIH (TME) est virtuellement éliminée par l'administration d'antiretroviraux pendant la grossesse et l'accouchement, la césarienne et l'allaitement artificiel. Des interventions faisables en Afrique sub-saharienne se sont montrées efficaces, mais toutes nécessitent que les femmes connaissent leur statut VIH. La mise en place du test à grande échelle du test VIH est difficile à réaliser. A Mombasa, Kenya, nous avons testé l'efficacité du lavage vaginal avec de la chlorhexidine pendant le travail pour réduire la TME, intervention qui ne nécessite pas de test anténatal et qui peut apporter un bénéfice général en réduisant les infections néonatales. Cette intervention s'est révélée inefficace pour réduire la TME. Nous avons cherché à mieux comprendre la physiopathologie de la TME, et avons retrouvé que la charge virale maternelle, la détection du VIH dans la filière génitale et dans la bouche de l'enfant étaient associées à un risque plus élevé de TME, en faveur d'une acquisition oro-digestive de l'infection par l'enfant. Le fait de connaître leur statut VIH expose les femmes infectées à un risque accru de violence domestique. Ceci s'est confirmé lors de notre étude. Ceci souligne l'importance de l'implication du père dans les interventions de prévention de la TME. La TME vient allonger la liste des pathologies délétères sur la santé néonatale en Afrique sub-saharienne. Des interventions simples pour réduire la prévalence d'autres maladies meurtrières ne sont pas encore mises en place. De nombreuses questions de recherche technique et opérationnelle demeurent pour obtenir des interventions de prévention de la TME efficaces, simples, sûres, et acceptables en Afrique sub-saharienne. Ces interventions qui doivent être intégrées dans les autres programmes existants, permettent l'ide,tification de mères et de familles infectées et affectées par le VIH et des liens avec les services de prise en charge doivent être créés.
