Book excerpt: One in five people in the United States had a sexually transmitted infection (STI) on any given day in 2018, totaling nearly 68 million estimated infections. STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.

Book excerpt: Men who have sex with men are disproportionately affected by human immunodeficiency virus (HIV) compared to the general population in nearly all countries collecting reliable surveillance data. In low- and middle-income countries they have 19.3-fold greater odds of being infected with HIV compared with the general population. HIV prevalence among men who have sex with men across North, South and Central America, South and Southeast Asia and sub-Saharan Africa ranges from 14% to 18%. Even as HIV incidence is in decline worldwide, the rate of new HIV infections among men who have sex with men remains unchanged and is increasing in some high-income countries like the United States. In 2011 the World Health Organization (WHO), the Joint United Nations Programme on HIV/AIDS (UNAIDS), the United Nations Development Programme (UNDP) and The Global Forum on MSM & HIV (MSMGF) developed a guidance document on Prevention and treatment of HIV and other sexually transmitted infections among men who have sex with men and transgender people. The document sets out technical recommendations on interventions for the prevention and treatment of HIV and other sexually transmitted infections (STIs) among men who have sex with men. In 2014, WHO released the Consolidated guidelines on HIV prevention, diagnosis, treatment and care for key populations. These bring together all existing guidance related to key populations, including men who have sex with men, with updates on selected guidance and recommendations. The recommendations of these two publications are summarized in Table 1 at the end of this Introduction. Following the dissemination of the 2011 Recommendations and the 2014 Key Populations Consolidated Guidelines describing effective, evidence-based interventions (the what), a need was expressed for guidance focused on implementation (the how). This publication responds to that need by offering practical advice on implementing HIV and STI programmes for men who have sex with men, aligned with the 2011 Recommendations and the 2014 Key Populations Consolidated Guidelines. It contains examples of good practice from around the world that may support efforts in planning programmes and services, and describes issues that should be considered and how to overcome challenges. The health and prevention benefits of antiretroviral therapy (ART) in the management of HIV are now strongly supported by research. Behavioural prevention programmes including use of condoms and lubricant, early diagnosis, prompt linkage to sustained care and ART, and viral suppression constitute points along a comprehensive continuum of HIV-related services. When services are easily accessible, implemented effectively and delivered in close partnership with their intended beneficiaries, this comprehensive continuum of health services reduces morbidity, mortality and onward transmission of HIV. However, current service delivery models are not as effective as they should be in linking and retaining men who have sex with men to the services they need, resulting in a failure to fully realize the health and prevention benefits of all interventions currently at our disposal. The findings revealed a drop-off in service retention at every point along the continuum.

Per-capita estimates reveal approximately 68 million prevalent and 26 million incident STIs nationally. Gonorrhea, syphilis, and chlamydia—the three reportable STIs—reached levels not seen in the last fifty years and this resurgence is concurrent with increasing antimicrobial resistance and a dearth of viable candidates in the vaccine pipeline. A seminal report by the National Academies of Sciences, Engineering, and Medicine, Sexually Transmitted Infections: Adopting a Sexual Health Paradigm, confirms that STIs rank among the most pressing and intractable public health threats. Furthermore, rising rates of STIs exact a substantial societal, medical, and economic burden that strain public health capacity, which has been substantially debilitated in the wake of the Covid-19 pandemic. STIs can have serious consequences for sexual, reproductive, and overall health. Untreated syphilis, for instance, is directly implicated in neurological, cardiovascular, and dermatological disease. Human papillomavirus is a known cause of cervical cancer and is the most common cancer among women globally. Hepatitis B increases risk for cirrhosis and primary liver cancer. Despite reductions in HIV transmission and improvements in prevention and treatment, infections among women, girls, adolescents, and mother-to-child transmission remain unacceptably high. Marginalized racial/ethnic minorities, LGBTQ persons, vulnerable at-risk adolescents and young adults, and other underrepresented populations are more susceptible to STIs as they negotiate an array of factors that can delay and even preclude access to preventive interventions. The United States spends substantially more on STI prevention and treatment yet it consistently bares a disproportionate burden of sexually transmitted infections compared to other Western industrialized nations. Access to healthcare, erosion and diversion of public health capacity, racism, discrimination, stigma, substandard education, and poverty have all been identified as important contributors to the trajectory of acute sexually transmitted infections. Furthermore, upstream drivers such as national, state, and local public health policies have been associated with population-level STI risk, prevention, and treatment and as such offer opportunities for ecological, observational, and multi-level analyses to assess their direct and indirect impact on sexual health outcomes. In this thematic collection, we aim to present an interdisciplinary collection of high-quality articles centered on the premise that the rise of emerging and re-emerging STIs can be attributed, in part, to a complex interaction of sociomedical factors beyond individual behavioral risk profiles. This Research Topic welcomes a variety of manuscript formats including original research, brief reports, systematic reviews, and perspective manuscripts involving sociomedical factors, social determinants of health, health disparities, infectious disease epidemiology, and other drivers including health policies that act as barriers and facilitators of effective STI prevention and treatment. Manuscripts that examine structural, community, institutional, interpersonal, and broad individual factors linked to STIs are very well suited for this collection. Topic areas include, but are not limited to: • Structural racism, discrimination, and stigma and their influence on prevention, treatment, and support services among vulnerable and marginalized populations; • Housing and income insecurity that impede sexual, reproductive, and overall health; • Biological factors that affect the spread of STIs including their asymptomatic nature and the influence of sex as a biological variable; • Intimate partner violence, harassment, and intimidation; • Substance abuse, sex work, sexual networks, and normative sexual attitudes and beliefs that impede the adoption of preventive health-promoting behaviors; • Priority populations including adolescents, men who have sex with men, youth who are LGBTQ, and the incarcerated; • Impact of HIV pre-exposure prophylaxis (PrEP) uptake on STI incidence; • Culturally-specific biopsychosocial, behavioral, and community-based STI interventions to treat and support those afflicted with STIs; • Community mobilization and community-based organization to reduce STIs; • Community-level prevalence of infectious agents; • Local and holistically-integrated STI clinics; • Public health STI workforce capacity; • Health in All Policies (HiAP) approaches to STI prevention and treatment. However, in the South-East Asia Region, the majority of men who have sex with men and transgender persons are highly stigmatized and discriminated against. There are an estmated 4-5 million men who have sex with men; among the transgender population, the number is less clear. Many of them are involved in high risk sexual behaviors that put them at risk for HIV infection, resulting in a high and increasing HIV prevalence in several countries of the Region. Control of HIV infections among these populations is thus an urgent public health priority. The countries included in this review are Bangladesh, India, Indonesia, Maldives, Myanmar, Nepal, Sri Lanka, Thailand and Timor-Leste. Though most of them have some form of interventions for men who have sex with men and transgender populations, the majority of these populations do not have access to various HIV services due to widespread stigma and discrimination, and punitive laws in most countries. This report provides information on the status of the epidemic among these populations in the South-East Asia Region. It highlights the need for improved advocacy efforts and a greater national response to save the lives of these populations who are at risk for HIV infection.

The Health of Lesbian, Gay, Bisexual, and Transgender People assesses the state of science on the health status of LGBT populations, identifies research gaps and opportunities, and outlines a research agenda for the National Institute of Health. The report examines the health status of these populations in three life stages: childhood and adolescence, early/middle adulthood, and later adulthood. At each life stage, the committee studied mental health, physical health, risks and protective factors, health services, and contextual influences. To advance understanding of the health needs of all LGBT individuals, the report finds that researchers need more data about the demographics of these populations, improved methods for collecting and analyzing data, and an increased participation of sexual and gender minorities in research. The 2011 WHO guidance, Prevention and treatment of HIV and other sexually transmitted infections among men who have sex with men and transgender people, did not separate recommendations for transgender people from those for men who have sex with men. The 2014 WHO Consolidated guidelines on HIV prevention, diagnosis, treatment and care for key populations acknowledge that the 'high vulnerability and specific health needs of transgender people necessitates a distinct and independent status in the global HIV response' (p. xiii) and consider transgender people as a separate key population. However, to date no global WHO document has addressed these needs exclusively. To fill that gap, this technical brief highlights WHO recommendations on HIV prevention, diagnosis, treatment care and needs of transgender people. Transgender-specific HIV data are limited. The majority of the published literature focuses on transgender women, given the documented heavy burden of HIV disease they bear. While this document will address HIV among transgender people, it contains limited information about transgender men, reflecting the imbalance of HIV burden and data on transgender women. While many transgender people identify as either male or female, some individuals express gender identities that do not fit within this binary. HIV data on people with non-binary identities are lacking, and, therefore, are not discussed separately in this technical brief. A technical brief on HIV among transgender youth, published in 2014, is referenced in this document where relevant. This technical brief is intended as a resource for governments, donors and implementers to help them identify what needs to be done to address HIV among transgender people. It begins with a summary of the epidemiologic data and then moves directly to a consolidation of recommendations pertaining to transgender people and HIV. All recommended interventions for transgender people should be voluntary, without coercion or force, and informed consent ensured. It is informed by a values and preferences study of transgender people themselves, and it includes illustrative case examples. This brief concludes with a discussion of the gaps in evidence and suggested steps to fill those gaps.
