Outre les mesures hygiéno-diététiques, le traitement pharmacologique est souvent indispensable pour contrôler la glycémie, l'objectif thérapeutique étant de limiter le risque de complications chroniques. La pharmacothérapie du diabète de type 2 a longtemps été relativement pauvre avec essentiellement en dehors de l'insulinothérapie, deux classes de médicaments disponibles : les sulfamides hypoglycémiants et les biguanides (metfomine). Depuis une dizaine d'années, de nouveaux agents anti-hyperglycémiants ont été mis à la disposition des praticiens et parmi ceux-ci les inhibiteurs de l'enzyme dipeptidyl peptidase-4 (DPP4), également appelés gliptines. Compte tenu du problème de santé publique posé par le diabète de type 2 , nous avons souhaité étudier cette classe pharmacologique innovante dont le mode d'action est basé sur la majoration de l'effet incrétine, afin de savoir si elle peut répondre à un besoin médical et si elle offre des perspectives réellement prometteuses dans la prise en charge du diabète de type 2, tout en n'exposant pas les patients à des risques potentiels mal connus.

Les fruits des travaux des chercheurs conduisent à de nombreuses innovations thérapeutiques et techniques, dont certaines révolutionneront peut être la stratégie thérapeutique de la maladie. Dans le diabète de type 1, le principal espoir thérapeutique est le pancréas artificiel qui devrait être disponible sur le marché dans quelques années car des améliorations du système et des études complémentaires plus importantes sur la population diabétique restent encore à réaliser. Le perfectionnement de la technique de la greffe d’îlot de Langerhans, la thérapie cellulaire et la modulation de l’expression génique représentent également les principaux espoirs de traitement du diabète de type 1.Les pompes à insuline se révèlent également être de plus en plus perfectionnées performantes. Les chercheurs fondent beaucoup d’espoir sur la mise au point d’un vaccin, mais qui pour l’instant n’est pas encore au point. Dans le diabète de type 2, les stratégies thérapeutiques reposent de manière importante sur la lutte contre l’obésité qui est une des causes principales des troubles endocriniens observés. Pour satisfaire cet objectif la chirurgie par aspiration bariatrique s’est développée et se montre beaucoup moins invasive que les autres techniques chirurgicales actuellement utilisées. Une nouvelle classe d’antidiabétique oraux arrive sur le marché, les inhibiteurs du cotransport sodium/glucose au niveau du rein en association aux autres classes d’antidiabétique. Les résultats des études se montrent particulièrement intéressants. Le PaQ ® par Cequr pourrait aussi constituer un progrès thérapeutique intéressants pour les patients diabétique de type 2 pour sa simplicité d’utilisation et son efficacité. Dans le diabète de type 1 et 2, il existe une évolution permanentes des insulines avec des améliorations de leurs profils pharmacocinétiques et pharmacodynamiques. Une nouvelle forme d’insuline devrait faire son entrée sur le marché avec l’insuline inhalée. L’enregistrement continu du glucose par l’intermédiaire d’une électrode sous cutanée, comme l’évolution lecteurs de glycémie présentant de nombreuses fonctions représentent des avancées thérapeutiques majeures dans le suivi des patients diabétiques atteints d’un diabète déséquilibré nécessitant un suivi important.

Book excerpt: Contexte : L'éventail d'antidiabétiques oraux s'est récemment enrichi d'une nouvelle classe : les IDPP4. Ils connaissent un franc succès depuis leur mise sur le marché en 2006. Cependant, les dernières recommandations de la HAS (2013) les relèguent en seconde intention pour la bithérapie avec la metformine, après les sulfamides hypoglycémiants. Méthode : Cette étude qualitative a été réalisée par l'analyse thématique de 10 entretiens semi dirigés auprès de médecins généralistes. Elle cherchait à mettre en évidence les différents facteurs poussant les médecins à préférer les IDPP4 aux sulfamides. Résultats : En plus de critères quantitatifs facilement identifiables, l'approche qualitative à permis de découvrir d'autres facteurs plus subjectifs. Les médecins mettaient en balance les avantages et inconvénients de chaque classe (efficacité sur l'HbA1c, effet sur le poids, coût, effets secondaires). Leur critère principal semblait être l'observance, nettement améliorée par les associations fixes IDPP4/metformine. D'autres facteurs venaient influencer la prise en compte des premiers. Il s'agit des rapports qu'entretenait chaque médecin avec les différents acteurs de santé publique, que sont l'industrie pharmaceutique et les autorités sanitaires. Ces rapports sont eux mêmes influencés par le statut du médecin, son âge, ses sources d'information et par les récents retraits de molécules et autres scandales médiatiques survenus dans le domaine du diabète. Deux profils de médecins ont émergé : les premiers, souvent les plus jeunes, semblaient être réfractaires aux laboratoires, ils rejetaient les nouvelles molécules et préféraient suivre la HAS, tandis que d'autres plus âgés recevaient l'information des différents acteurs avec moins d'a priori et voulaient se forger leur propre opinion. Ils décidaient souvent de ne pas suivre ces recommandations estimant qu'une meilleure observance primait sur le coût du traitement. Conclusion : Les recommandations relèguent ces molécules en seconde intention principalement pour leur coût et le manque de recul sur leur sécurité. Depuis leur parution, certaines suspicions d'effets secondaires ont été en partie levées par de nouveaux essais cliniques. La recherche dans le domaine du diabète s'est considérablement accélérée, de nouvelles pistes sont à l'étude et l'éventail thérapeutique risque de s'élargir dans un futur proche. Les recommandations ainsi que l'opinion des médecins devront donc s'inscrire dans une constante évolution.

La connaissance d’un traitement est liée à son observance. L’éducation thérapeutique du patient (ETP) joue un rôle dans l’observance. L’objectif de ce travail est donc d’évaluer les connaissances et représentations des patients diabétiques de type 2 sur leur traitement. Matériel et méthodes Les données ont été recueillies de manière prospective à l’aide d’un questionnaire mixte téléphonique réalisé d’Août 2014 à Mai 2015 chez des patients diabétiques de type 2 choisis aléatoirement parmi les patients suivis au centre médical de santé de Champigny sur Marne. L’influence de plusieurs facteurs dont la réalisation d’une ETP ont été analysées à l’aide de tests univariés. Résultats 120 patients ont été inclus. 44.8% connaissaient très bien leur traitement antidiabétique oral et 66,7% leur traitement injectable. Les traitements antihypertenseurs et hypolipémiants étaient mal connus (respectivement 53,8 % et 48,3%). Il n’a pas été mis en évidence pas de lien significatif entre la connaissance des traitements et la réalisation d’une ETP. Les attentes des patients concernant leur traitement sont variées. 76.7% des patients déclaraient ne pas oublier leurs traitements. Les effets secondaires ne sont pas connus dans 85% des cas. Conclusion La connaissance des patients de leur traitement médicamenteux est partagée. Les traitements régulant la glycémie sont bien connus mais les anti-hypertenseurs et hypolipémiants le sont sont beaucoup moins. Ce différentiel s’explique probablement du fait que la mise en évidence de l’importance de ces traitements pour le daibète est récente et encore méconnue des médecins.
