Book excerpt: In the sixth edition of this work John Fry has reverted to the format of the earlier editions published in 1965, 1970 and 1973 and has presented statistical data drawn widely from many sources. General practice in the United Kingdom and elsewhere throughout the world has undergone many changes in the past decade. This new edition brings the available statistical informa tion up to date and indicates the way ahead. General practice, the College and the National Health Service have been closely linked in effecting change. The membership of the College con tinues to grow and its influence in changing the face of general practice in the United Kingdom is significant. John Fry's new book presents that characteristic blend of fact and personal opinion which was so successful in earlier editions and so valuable to general practice and others in primary health care. Once again he has shown what can be achieved by one man based on his own work and those of others in general practice. John Lawson RCGP, 1983 President of the Royal College of General Practitioners vi Preface The Present State and Future Needs series had its roots in the dark days of the early 1960s when morale in general practice was low and when numbers of new entrants were actually going down. They were the days of mass emigration of doctors from Britain and the National Health Service. The fourth and fifth editions were entitled Trends in General Practice (1977 and 1979).

They performed their own surgery, and were accustomed to treating a wide variety of illnesses and symptoms. Specialists were few in number, tended to practice in large towns, and were often associated with major hospitals. But rapidly changing medical institutions and services in the twentieth century have compelled specialization even among more modest doctors and hospitals.

Horder, President, 1980-82 The first 30 years of the College have been an exciting experience for those most closely involved. Some have already passed on, but this account has been written soon enough for many of the actors to be historians. Future members of the College will be grateful to them for what they have written, as well as for what they did as a remarkably determined and harmonious team. Students of twentieth century medicine in this country will also be grateful for a first-hand account of the development of an institution which has been closely associated with, and partly responsible for, important changes in medical care and education. Those who read these pages may wonder how the builders of this young College could have found time to do much general practice. They did. The three editors of this history, which covers 25 years, and the general practitioner members of the Steering Committee all ran large practices, in which they worked very hard throughout that time. Most of their work for the College was done during off-duty hours, weekends and holidays. The College could not have developed as it did, had they not been personally concerned with the practical problems and needs of clinical medicine. This is also true of many of the contributors. It is impossible to mention everyone who deserves credit. The editors hope that they may be forgiven for any serious omissions.

Yet we still lack a clear historical account of how this psychological sensibility came to be established. The Transformation of the Psyche in British Primary Care, 1880-1970 will remedy this situation by demonstrating that there is nothing inevitable about the current connection between health, identity and personal history. It traces the changing conception of the psyche in Britain over the last two centuries and it demonstrates how these changes were rooted in transformed patterns of medical care. The shifts from private medicine through to National Insurance and the National Health Service fostered different kinds of relationship between doctor and patient and different understandings of psychological distress. The Transformation of the Psyche in British Primary Care, 1880-1970 examines these transformations and, in so doing, provides new critical insights into our modern sense of identity and changing notions of health that will be of great value to anyone interested in the modern history of British medicine.

Available in PDF, EPUB and Kindle. Book excerpt: JOHN S. MILLIS In 1966 the Citizens Commission on Graduate Medical Education observed that the explosive growth in biomedical science and the consequent increase in medical skill and technology of the twentieth century had made it possible for physicians to respond to the episodes of illness of patients with an ever-increasing effectiveness, but that the increase in knowledge and technology had forced most physicians to concentrate upon a disease entity, an organ or organ system, or a particular mode of diagnosis or therapy. As a result there had been a growing lack of continuing and comprehensive patient care. The Commission expressed the opinion that "Now, in order to bring medicine's enhanced diagnostic and therapeutic powers fully to the benefit of society, it is necessary to have many physicians who can put medicine together again. "! The Commission proceeded to recommend the education and training of sub stantial numbers of Primary Physicians who would, by assuming primary responsi bility for the patient's welfare in sickness and in health, provide continuing and comprehensive health care to the citizens of the United States. In 1978 it is clear that the recommendation has been accepted by the public, the medical profession, and medical education. There has been a vigorous response in the development of family medicine and in the fields of internal medicine, pediatrics, and obstetrics. One is particularly impressed by the wide acceptance on the part of medical students of the concept of the primary physician. Dr. John S.

Available in PDF, EPUB and Kindle. Book excerpt: In the early 1970s general practitioners were well aware that they were being asked to deal not only with physical illness in their patients but also with the stresses relating to social and emotional problems. Increasingly often they were working together with health visitors and social workers in attempting to respond more effectively to these demands. Originally published in 1972, this study describes the attachment of a social worker to a group general medical practice in London, indicating why, for all social groups, the general practice is an appropriate point at which psychosocial problems may be identified and treated. The authors describe the nature and range of patients’ problems that come to light in the consulting room; how patients present their problems to the social worker; and the kind of help the social worker is able to offer. They explore the extent to which the general practice setting provides opportunities for preventive therapy and further describe how social work in general practice can most effectively be related to existing social services in the community, particularly to the reorganised personal social services. Their findings are supported throughout by illuminating case studies. The book also discusses the integration of the social worker into the general practice team, the problems that have to be solved and the mutual enlightenment that results. This emerges as an extremely encouraging and instructive experiment, which will immediately interest social service departments and social workers, doctors and nurses, both students and those in practice. The wide spectrum of social problems encountered and dealt with by the social worker in a general practice make it a particularly stimulating account.
