Book excerpt: La stérilisation des dispositifs médicaux réutilisables en milieu hospitalier est une activité à risque. C'est un procédé complexe et réglementé qui implique la mise en place d'un système d'assurance qualité. Les différents textes réglementaires sont régulièrement mis à jour et de plus en plus exigeants notamment en termes de traçabilité. L'objectif de cette thèse est de préparer la mise en place de la traçabilité individuelle à l'instrument au CHU de Bordeaux. Actuellement nous réalisons une traçabilité au contenant, c'est-à-dire que nous traçons dans le dossier patient informatisé l'utilisation de chaque composition contenant des dispositifs médicaux réutilisables. Afin de répondre aux objectifs de sécurité sanitaire pour le patient, nous nous sommes engagés dans la mise en place d'une traçabilité individuelle à l'instrument. Pour cela, nous avons rencontré différents fournisseurs et centres hospitaliers afin d'étudier les différentes solutions de mise en place de ce type de traçabilité. Ce travail nous a permis de définir le périmètre de traçabilité individuelle à l'instrument, de choisir un bloc opératoire test, d'étudier les différents modes d'identification des instruments, les différentes méthodes de marquage, les différents modes de lecture ainsi que les prérequis informatiques nécessaires. Nous avons également réalisé différentes étapes préliminaires indispensables telles que l'estimation du taux de marquage des instruments chirurgicaux, l'ajout des compositions dans notre logiciel métier et la réalisation de tests de reconstitution. A la suite de ces résultats nous avons pu définir les futures étapes nécessaires au bon déroulement de la mise en place de la traçabilité individuelle à l'instrument au CHU de Bordeaux.
