Using data from the Insurance Component of the Medical Expenditure Panel Survey (MEPS-IC), this Statistical Brief discusses annual premiums, employer costs, and employee contributions for family coverage in private sector firms in 1996 through 2006. This period encompasses all the years for which the MEPS-IC has collected information on employer-sponsored health insurance coverage. Employee-plus-one coverage--which is limited to the employee and only one family member, and has a lower annual premium than comparable family coverage--had 18.0 percent of 2006 enrollment and is not analyzed here. In this Brief, family coverage premiums, employer costs, and employee contributions are analyzed for all firms, and for small (fewer than 50 employees) and large (50 or more employees) firms. Information relating to coverage for a family of four was collected in cases where multiple pricing levels were offered to employees. Note that while firm size is an important determinant of costs for employer-sponsored health insurance, other factors may come into play. Benefit packages offered can be more or less generous--through the absence or presence of deductibles and co-pays, for example. Demographic factors (such as age of enrollees or work schedule) and firm characteristics (such as industry or length of time in business) can also affect employer-sponsored insurance costs. Only differences that were statistically significant at the 0.05 percent significance level are noted in the text.

Although a large percentage of private sector employees worked where health insurance was offered in 2006, small private sector employers were much less likely to offer health insurance than other firms or governments. Among these small employers, the offer and enrollment rates varied considerably by industry.

In 2006, more private sector employees obtained non-single coverage health insurance (a plan covering the employee and at least one other family member) through their employers than single, self-only coverage. According to the Insurance Component of the Medical Expenditure Panel Survey (MEPS-IC), those employees with non-single coverage contributed both a larger dollar amount and a larger percentage of the total premium for their coverage than did employees with single coverage. Non-single health insurance plans encompass two types of coverage: family coverage and employee-plus-one coverage. Prior to 2001, MEPS-IC collected only information on family coverage plans. Information related to coverage for a family of four was collected in cases where multiple pricing levels were offered to employees. Beginning in 2001, separate data were also collected for employee-plus-one coverage. Not all employers who offer family coverage provide the alternative of an employee-plus-one policy for employees with just one family member to insure. However, when available for the same level of benefits, employee-plus-one plan premiums are less expensive than family plans. This Statistical Brief presents estimates on the selection and cost of single, employee-plus-one, and family health insurance coverage in 2006. Only differences that are statistically significant at the 0.05 significance level are discussed in the text.

Book excerpt: One approach to covering the uninsured that is frequently advocated by policy makers is subsidizing the employee portion of employer-provided health insurance premiums. But, since the vast majority of those offered employer-provided health insurance already take it up, such an approach is only appealing if there is a very high takeup elasticity among those who are offered and uninsured. Moreover, if plan choice decisions are price elastic, then such subsidies can at the same time increase health care costs by inducing selection of more expensive plans. We study an excellent example of such subsidies: the introduction of pre-tax premiums for postal employees in 1994, and then for the remaining federal employees in 2000. We do so using a census of personnel records for all federal employees from 1991 through 2002. We find that there is a very small elasticity of insurance takeup with respect to its after-tax price, and a modest elasticity of plan choice. Our results suggest that the federal government did little to improve insurance coverage, but much to increase health care expenditures, through this policy change.
